Blame it on the metric system, who thinks up 5.0mm and 5.5mm bolts in the same bins? That's only 20 thousandths of an inch difference, but all the difference in the world to the pilot flapping out in the breeze.
Blame it on a system that makes sense? No pull out a tape measure. Bloody Americans are full of it.

As much as the mechanic and engineer are at fault, if the pilot did not have his lap belt and shoulder straps on,
sinched tight, he is very much culpable.
He ain’t culpable for the windscreen blowing out.
His behavior was perfectly normal and not a mistake. It was on auto pilot, safe to unbuckle in any non emergency situation. HTF was he supposed to know the windscreen would blow out?
