- I try… Tyrvaya! This is a brand new dry eye medication that stimulates basal reflex tearing via a nasal spray. As a dry eye patient myself, I was naturally curious about this medication and wanted to offer my honest and completely non-sponsored thoughts and experience.

Update: I was most likely coughing and having some throat irritation because some of the medication went into my throat.
According to the company, you should be breathing normally (or even holding your breath) upon administration. One tip they gave me is to place the tongue on the roof of the mouth when administering to prevent the spray from entering the nasopharynx. Hope that helps :)
I'm a COA and was so excited when we got samples of this. Of course I ripped it open and just skimmed the instructions. I took it just like I would Nasonex. Ooopssss!! Immediately sneezed and coughed. Boy did it burn!! 🤣🤣🤣🤣
But once I actually learned how to use it properly, it's not nearly as bad. I still sneeze about five times after every dose which can be annoying. I much prefer it to the battery acid feel of Xiidra though!
For reference, I'm pretty dry. I usually have about 1+ inf PEE and use scleral cls because I cannot tolerate or see well in soft cls. I'm curious to see how well this works. It's exciting particularly for patients who are averse to using gtts!!

Sorry I was disrespectful but I am very cynical. Sure I can believe you are not being paid but I do think you should mention the very high price. Not sure what the exact price will be but I read anywhere from $10 to 30 to 400 a month.

The pricing depends 100% on your insurance plan and pharmacy so I can't quote any pricing. I would not spend an exorbitant amount on this medication but if you struggle with dry eye and would like to try this, I would recommend giving it a shot. Ask your eye doctor of they have a sample you can try and whether they know of a local pharmacy that can give you a fair price.

I am in my young 40s but have had dry eyes issues since my mid-late 20s. For some reason this winter my eyes have been at their worst. I’ve tried Restasis, Xiidra and Cequa over the years with no luck. I just recently stopped Cequa after a week or two of use as it worsened my symptoms. By the evening, my eyes were so dry that my vision was very blurry. It made my eyes red and the sensation of having something in it. I believe in the studies with those cyclosporine drops, only about 17% see any improvement (if instead the study correctly - that just seems so incredibly low to me). Anyways, I’m also sensitive to medication side effects and hate taking prescriptions but Tyrvaya was my next option. I followed the instructions exactly as the manufacturer described and it did not burn or really even make me sneeze. I’ve only used it a few times though. I may have some throat irritation but can’t tell if it’s the spray or just my reflux acting up and causing the feeling. Since it’s only been a couple of days, I have not noticed any dry eye relief but believe it needs to be taken at least for four weeks to get the full benefit (also knowing it won’t work for everyone but I’m hopeful it will work on me). Even lubricating eye drops evaporate quickly and I’ve tried so many different options there. It’s frustrating to keep having to try things and not get any relief from dry eye. The blurry vision and glare from poor tear quality is really bothersome at times. I will try to remember to come to this channel and give an update once I give this more time. I will say my husband even commented that my eyes seemed whiter today than they have been so that was nice to hear. Not sure if it’s because I stopped Cequa which did worsen my redness or if it’s the Tyrvaya. TBD…
I have dry eyes and was using Restasis and eye drops with no real relief. I saw 100 percent improvement in 4 days. Stopped using Restasis after one week. I now sometimes forget to use the eye drops in. Don't breathe deep when using and don't swallow and post nasal mucus. Yes it burns a little, but better than having the feeling of sand in my eyes.

I tried Tyrvaya for my dry eyes; and this medication irritated my throat; dried out my skin on my face that made my eyes sunken. Also made my neck drier. Gave me cough from irritation. Sneezing galore! Never again! Made me look older! My eye doctor switched me to Xiidra; also burns a little but the immediate relief compensates. I also use Refresh Optive Mega-3 lubricant eye drops - they are wonderful! And Alrex eye drops by prescription; also very soothing.

Hmm. My eye doctor gave me a one month sample and I'm not feeling anything in my eyes, but my nose, the sneezing and a sore throat, absolutely. I was contacted by the pharmacy that can provide it, but even with my insurance, nope. Too expensive. Sure I've got the money, but I'm not paying that every month. He offered some other solution to my issue, but honestly, I don't feel any eye doctor is truly listening to me. I guess I'll just have to poke my eye out. Not really of course, but it would be nice to have my issue actually addressed. Sure, I've got dry eye, I've never debated that. But first, there is 100% something either on the surface of my eye, under an eyelid or totally underneath my eye. And no one seems to be able to see it. Extremely frustrating.
Try a good Atlantic or Norwegian cold pressed fish oil.
Dr Lee, you used Tyrvaya improperly.
You sprayed it up your nose like an allergy spray which is not how the instructions for use are stated.
You are to spritz once in each nostril towards the outside of the nose pointing to the area of where a nose ring would be, towards the top of the ear. Have a tissue in your hand ready to wipe up any spray that may run out of your nose. If administered properly the only side effect should be sneezing and it’s mild and should resolve within one minute.
Tyrvaya is dosed one spritz in each nostril, two times per day spaced 12 hours apart.
You also referred to Tyrvaya as a drop. Tyrvaya is not a drop it is a nasal spray for dry eye.

Thank you for your feedback! You are absolutely right. This is a nasal spray (I promise I haven't been putting it in my eye or telling patients to do so!), and I was aiming towards my ear but ended up spraying a lot up my nose in my first use video here. I've been using it since and I have not had coughing or headaches.
I just got it…and googled and saw the chantex ingredient causes so many side effects…is this true?
I took chantex at 18 and had weird health issues at the time
I suffer from post accutane health issues

I'm sorry you had such bad side effects from Chantix. This spray is very dilute (0.03mg) per spray versus 0.5mg or 1mg tablets you take orally so it would be suprising if you have the systemic side effects from Chantix. I would make sure you speak to your eye doctor to make sure you would benefit from this medication, especially if you are worried about side effects.

I’m using it and so far not bad
I tried it for two solid months and finally gave up. I followed the directions but it always felt like I was shooting pepper spray up my nose. It burned and made me sneeze over and over. I just couldn't deal with it anymore and I didn't experience any benefit that I could detect. I am still using Xiidra twice a day (which burns but I can handle it) and I just started IPL treatments for my dry eyes. I've only had one treatment so far, but I think it helped. I will have five IPL treatments in total. I also use a device called NuLids which helps to gently clean my upper and lower lids and I think that is helping as well. I go back to my opthomologist in a few weeks to see if anything is really making a difference. Even though this medication didn't work for me, I hope some people find it gives them some relief as I know how hard it is to treat dry eyes effectively.
I'm so sorry this was irritating and did not give you reilef! As you mention, this medication is not for everybody. I hope you and your ophthalmologist can find a treatment and/or regimen that helps you feel better ❤️

I guess this product is not for me. I experienced a severe burning sensation which was extremely uncomfortable. I use nasal sprays for my allergies and have no problems with this type of administration of meds. But the burning sensation was much too strong, similar to inhaling bleach or other potent solutions.
What a bummer :( Did you try holding your breath to mimimize that burning sensation? Hope you find relief with something else!

@@user-gh6dv6cl7d It works well for some and not for others. It partly depends on what kind of dry eye you have and why you have dry eye. If you suffer from dry eye and have not found relief with other treatments, this is very different kind of treatment that you can ask your eye doctor about trying.

Tyrvaya unfortunately is a total nothing burger for me. I get some reflexive tears but I can barely perceive any improvement with dry eye symptoms an hour after usage. I was really looking forward to it too. Intro pricing is just $10 for a monthly supply, and I like spicy things... even up the nose, apparently, but the novelty is wearing off for me.
I'm sorry it didn't work for you :( Unfortunately we don't have a majgic cure for dry eye yet, and this medication won't be helpful for everybody. Glad you were able to give it a try for a low intro price, and I hope there is another treatment out there that will be helpful for you!
@@MicheleLeeMD Thanks. Fortunately my symptoms are easily managed with reduced CL usage and OTC Retaine PRN. But I'm anticipating moving to cyclosporine drops or punctal plugs if it progresses.

I'd use every other treatment available before using tyrvaya. It's awful! It's irritating and didn't even seem to work.
Only worked for two weeks for me. Have to move on to finding something else 😢😢😢

M'am, dry eye is curable.... Both myself and my sister cured our dry eyes. Please educate yourself beyond what you were taught! Medical school needs to be updated with current information!

@@DoggyDog420 ..ok so we both at separate times in our lives went to the eye doctor (I went to at least three) and eventually we both got the prescription cyclosporine drops but neither of us used it. I think we both hoped for natural resolution. And we got it. My sister said she had dry eye for 2-3 months and it eventually went away on its own ... probably because her body had health issues in other body parts and focused on those instead.
I had it over a year... probably longer. What I did was I use a wet cold washcloth to apply to my eyes with lids closed. This gave me a bit of temporary relief. And maybe it helped with the cure.
BUT the main cure is that for me, I discovered the muscles in my eyelids were tense. That is it! Ridiculous. One morning when I awoken...I realized my eyelids were really tense... And so over the next days and weeks I really focused on relaxing my eyelid muscles and keeping them as relaxed as I possibly could, which can be a challenge. But like they were probably tense overnight as well when I was sleeping.
So just wanna say there are certain pharmaceuticals that can exacerbate muscle tension in the body even tho they are supposed to get rid of muscle tension.
You can also gently massage your eyelids when they are closed ..and like massage all around and even get a scalp massage or whole body massage...well there are so many different techniques for body muscle tension.
So please try this out and get back to me!

if they have a good response in general to neurostimulation, a lot of patients we talk to use both tyrvaya and itear100. tyrvaya for quick hit and twice per day and itear for longer use during day with unlimited usage during the day
