Hello! Thanks for being a member! Since I can't hear you or see how you are singing, I'll list some general reasons as to why you might still have trouble singing smoothly over the passaggio. 1. Either your chest voice or head voice registers are not strong enough. When one register is stronger than the other, it makes it hard to blend. Most singers need to work on strengthening their head voice the most, so that means practicing exercises that will work on better cord closure in the head voice. 2. Your breath control may be off. You may be pushing too much air when trying to go over the passaggio, so it's a matter of knowing how to manage your breath, but that gets better when your head voice becomes stronger to resist airflow, rather than letting all that air escape the minute you try to enter the head voice register. For now, look up my head voice routine video, there's a 20 minute version and work on strengthening that up first before trying to learn how to mix. Hope that helped!
