    He lived fast, and died young. Looked like he loved what he was doing, got to enjoy the fruits of his labors, and he was entertaining, funny, knowledgeable, just a great guy that wasn't afraid to tell the world what he was doing and how to do it. Totally preventable death. RIP Rich.

    Not to be an ass but you're wrong about "vascular injury" As a registered vascular specialist for 15 years I want to clear something up. #1 arteries don't have "flaps" (They are called valves), veins do. The bulging veins is the correct term however, I'm guessing he had some cardiomegely with right heart failure/overload which can also contribute to the bulging veins. Stroke is a broad term. It doesn't mean "vascular damage" in every case. It can be embolic, hemorrhagic due to trauma, aneurysmal due to genetics etc. It seems like you're trying to be honest and put out a good video. I'd lay off the medical terms because this can cause more confusion and mis information than is already surrounding his death.

      I truly appreciate your comments and criticisms. This video is intended for non-medical professionals - if i had prepared this presentation for the medical community, it would have lost the viewers in the first 2 minutes. I do, however, feel it necessary to include some medical terminology to support my statements. I can't please everyone, this is UA-cam, after all, not the NIH.

      I have elevated blood factors 7,8,10,11 and 12. This happened seemingly out of nowhere. It has been determined that a genetic mutation that exists was triggered by testosterone. I will have these elevated blood factors for life now. It's possible the amount of anabolic use Rich put himself through would have had his PTINR count real low, meaning his blood would be thick. This caused by red blood cell production at the hands of increased test levels. This would be an additional factor alongside the other possibilities which would compound any problems he had with his cardiovascular system. Essentially speeding up potential of death by forcing the heart to work at a heavy rate. I'm no where near Rich's size, not even close. I have to monitor my heart closely due to my blood thickness. I'm at risk of death daily because of blood alone. That doesn't include stress from Rich's body mass, bulging veins, low oxygen distribution from weight on his lungs etc. I guess what I'm getting at is that I'm an expert on cardiovascular/hemo issues, because i have to be, and I don't believe it's any one thing that killed Rich but more of a compounding of problems due to his extensive long term usage of androgenic substances on top of his work ethic. The horse basically raced himself to death. Also, i don't care what anyone says, knowing how this works, his BP had to be out of this world high.

      @@mrrooster4876 rich was sniffing caffeine powder and to much caffeine will speed up your heart and that's what caused his heart attack!!!!! I new other people that did the same thang!!!!!! Caffeine powder made his blood pressure sky rocket and his heart started racing and that's what caused his heart attack!!!!! He was sniffing caffeine powder every morning and his heart gave out!!!!!! Caffeine powder is made to put in liquid and drink not sniff!!!! If you sniff caffeine powder it goes straight into the blood stream and your hearts starts racing to much then you have a heart attack!!!!!!

      Wrong idiots!!!! Rich overdosed on caffeine powder!!!! He was sniffing caffeine powder and to much got into his blood stream and his heart started racing out of control!!! I new other people that did the same thang!!!!!!!! That's what caused his heart attack!!!!!!!!!!!!!

      Lou ferignou took roids and was 330lbs and now he is about 70 years old but he was not sniffing caffeine powder!!!!!!! To much caffeine will speed up your heart and cause heart attack!!!!

    Rich was too smart to overdose? Are u kidding me? Addicts never MEAN to overdose! He was an addict and most likely kept upping his dose just like addicts do until they OD

      VeggieGiant he was an addict thru and thru- I liked his videos and honesty but his inability to see thru his " bigorexia" brought on the multitude of health issues you discussed and caused his eventual passing...thinking we are invincible and utilizing anything and everything (and rationalizing this unsafe behaviour) is the CORNERSTONE of addictive behaviour... his death is very sad but I could see it coming long ago, especially during his "bigger by the day" challenge...he has admitted his own dysmorphic thinking and stopped at nothing to achieve the impossible ... truly unfortunate.

      Upping your dose does not trigger an overdose. The body builds a tolerance so quick to most recreational drugs. I had a friend who was taking 500mg of oxy IR in a single dose which would kill most people out there. But for him and his tolerance that was his normal to be withdrawal free. Benzos like Xanax, and amphetamines like adderal are the deal with rapidly increasing tolerances.
      The people who are dying from overdoses usually involve Fentanyl which is hard to mix homogeneously due to the strength per milligram/microgram. They don’t know the strength of what they are getting from their “herioin”.
      Pharmaceuticals like adderal/Xanax/oxy don’t have this issue so users will have a very hard time unintentionally OD’ing. They know the strength of each pill and can increase accordingly. The bodies tolerance will also provide a wide margin of safety. Like someone who regularly takes 40mg of oxy IR per dose will have no danger taking 80mg. They might throw up but that’s about it. With fent or fent contaminated heroin it’s much more dangerous, they just pass out and choke on puke, or pass out and stop breathing.

    Thanks for taking your time to explain Rich's unfortunate situation. You have opened my eyes immensely and have helped me turn the mirror on my own lifestyle so I can re-evaluate .
    Sadly, our dear friend Rich has passed and this video is definitely about his tragedy BUT you might have saved my life by educating me as a result of watching this video honoring Rich.
    I cant help but wonder......Perhaps Rich's high profile/popular life was taken from him so we, his fans could learn from it and make the choice to wisen up and turn our lives around.

    I'm a Real Doctor....yes an MD.....you are on point with most of your observations, and completely off point with others. I think you nailed it when you concluded that Rich died of a heart attack, what us doctors call a Myocardial Infarction (MI). Sadly, I predicted this. I told one of my buddies a few months ago that Rich (and others) were risking their lives being so big and doing so much Gear....years ago in my bodybuilding life I did big doses and what I saw in my blood work Shocked Me....doing high doses of testosterone (800-1000 mg/week) dramatically altered my Lipid Profile, seeing Low HDL (HDL=Good Cholesterol) numbers and High LDL (LDL=Bad Cholesterol) numbers, even tho I was eating clean...back then I was delusional and wanted to be a professional bodybuilder, despite my education and great job! I trained relentlessly and ate right and took a ton of drugs: Testosterone Cypionate/Propionate/Enanthate (mostly cypionate), Nandrolone Decanoate (Deca), Stanozolol (Winstrol-V.....my workout buddy's brother was a horse trainer), Boldenone (Equipoise...came in a huge bottle that never seemed to run out lol), Methenolone (Primobolan...did both the oral and injectable), .....and Yes...I did cycles of Anadrol-50 (oxymetholone)! Whaaaaat! That's The Most TOXIC and Effective oral anabolic on the planet!!! Yes Sir! And Growth Hormone and Insulin Too...I Did It ALL and at my best I was 6'0" 249lbs with abs, which after all that work and pharmacy didn't even compare to the professionals of that day (1990s) Dorian Yates, Paul Dillet, Flex Wheeler, Kevin Levrone, Nasser El Sonbaty....I gave my Delusional Dream a run and I didn't even come close to those guys lol! MY LAST INJECTION OF GEAR was in 2002...I had an Epiphany...a patient in the ICU needed a critical care MD (me). I was on the first floor. The ICU was on the second floor so I ran up ONE FLIGHT OF STAIRS AND I WAS WINDED! Out of breath, despite being near 250lbs with low body fat which I Thought was good lol! Turns out Everything I Was Doing Was Wrong. I have No Answer for why I thought I could be a Professional Bodybuilder, but I gave it my best shot! GENETICS MATTER More Than Anything in my humble opinion and after trying to climb the ladder of Professional Bodybuilder and Failing despite my best efforts, I can tell you those dudes At The Top aren't just there because of anabolics, because if it were that simple, I'd Be There Too.       Honestly no one needs that much gear. That was the major mistake you made, especially at such a young age. The key is to stay natural until you meet your genetic potential, then take a standard pharmacy grade(TRT) amount of test and continue to eat healthy and workout. If the genetics are there, you should be able to look like the greats. That level of gear becomes counterproductive. Just a 250mg shot of test each week will make you grow like weeds. And you have a medical professional monitoring your levels and health. If you’re scared though, you shouldn’t do it anyway. With everything we do comes risks, and it’s your responsibility to decide whether you can accept those risks and if they’re worth it for your goal.

    He lived a lot in his life. Nobody is going to get out of this alive. What difference does it make how he passed? He was loved and is in gods hands now. He paid a price for his career like many athletes and he knew there would be a cost.

    I followed Rich for a few years before he passed and it's ironic how his motto "Whatever it takes" was the reason why he had so much success AND simultaneously the reason why he had so many health problems.

    I completely agree with you about Mr. Piana being a time bomb. The guy as of the past few years really looked bad. His body looked so stressed and taxed especially after bigger by the day. I actually remember saying to myself "I would not be surprised if one day in the near future we here about him passing away".

    If an experienced and well informed drug user suddenly drops dead at 46, how long do you think inexperienced users are likely to last? Bottom line, it's not worth it. I guarantee you that Rich would give up all his Riches, partying, drugs etc. for a few more healthy years on this earth. Smh. Sad.

    Great talk. I am former world champion highlands games athlete competing well into my late 50's. I had heart attack, or what I thought heart attack at 57. Turns out it was ventricular tachycardia. Then later found out I had Wolf, Parkinson's, White. A congenital heart condition caused by faulty electrical nodes surrounding heart. It usually kills males in their late 20's and 30's because hard to diagnose. My first tachycardia, when I was taken to emergency room, cardiologist said my heart was beating almost 300 beats per minute. Would kill most people, but because of my specific training, Olympic lifts and such, my heart wall was quite thick and could withstand this trauma. They call this the silent killer because what happens is your blood pressure drops very low, you pass out and your heart beats out of control to failure. These faulty electrical nodes tell your brain your heart is doing crazy things and then your heart valves open at all the same time causing your blood to circulate in super fast cycle around all 4chambers. My competitive body weight at time was @ 265 on 6' frame. I was diagnosed at 30 with hypogonadism and had to take test, under my doctor watchful eye. I took 1cc every week for years. Now I am mid 60's and they have take smaller dose. This is first I hear about PMMA. So horrible and ridiculous to take for vanity. I don't really consider bodybuilding a sport but more entertainment. You are judged how you look like beauty pageants. I question when bodybuilders say they are athletes. But nothing they do requires athleticism. They lift weights but are not judged by how many sets or reps or time spent in gym. But I am sad for this mans loved ones.

      Oncology, Hematology and years of informal study along with many professional seminars etc. My education is a combo platter lol. I am still involved in the medical profession via non-professional means. I was going to medical school years ago to be a doctor but my life took a different turn, but medicine and the human condition are still a passion of study for me.

    The key point is the following. I'll preface by saying, that from watching his videos, Rich seemed like a wonderful bloke, highly talented, a leader, intelligent, and of course anyone who gets the amount of tributes this guy gets is doing something right. However, his pursuit of the lifestyle he lived, the steroids, the drugs and whatever the fuck else he was doing is a sign in my opinion of a deeply troubled and insecure person. Before people hate on me, this is not so surprising. Great people, great leaders are often depressives, are often insecure. Rich could've saved himself if he had have got to a psychotherapist, a behaviour therapist, and looked carefully at why he was doing what he was doing and trying to understanding how, as he got older, he could start to transition into a new healthy lifestyle. This is the tragedy from my perspective. This is a story about pharmacology, but at its roots, this is a story of mental illness. What made this guy so great was also his biggest weakness. The message to come out of this for all you gym junkies out there should be balance, and where possible aiming for healthy options as opposed to synthetic ones.

      No source about Sara, just from personal experience with "foreign" women and seeing what ended up happening with Rich's marriage to her and her subsequent behavior. That was not meant to be of importance, just my personal, sensational opinion. Maybe i should have left that out to be more thorough, Good point.

    Dude you don't know the exact cause of death and you can't know unless you talk to and immediate family member, the only thing you have is speculation based on perceived symptoms and prior reports released by Rich himself. You should change the title of your video to My opinion of Rich Piana's passing. Your title comes of sounding like you know exactly what caused Rich's death when you don't know there for I can only come to the conclusion that this video is titled so that you can attract more views to your channel.

    What happened was this, he either snorted cocaine or Adderall, and had cardiac arrest. You lose blood immediately to your brain, thus causing you to pass out(also shortness of breath as well). He either snorted before he got his hair cut or during. I'm sure abuse of pre work outs (which he used tons of), recreational drug use, and health concerns he may not of been aware about was a huge factor in his death. He could of easily been a "functioning" drug addict. Which can be common. I'm starting to believe TMZ that "drugs" was the white substance found in his home. I mean honestly, who the fuck snorts pre work outs? For real, who the fuck does this?

    She said he was gasping for air. Any of those things you listed would be instant death. I am positive that he died of liver failure.     Any man that is 270 to 300 pounds is a high risk. Just because it is muscle instead of fat don't make a difference. Wens the last time u seen a large old man very very rare.

    By law they have to perform an autopsy if the cause of death is inconclusive.
    He death could be the cause of multiple damage. I think the main factor was the development of arterial sclerosis that eventually starved his body of oxygen. It's very common is steroid abuse which make the internal organs look like those of an old person's.
    It's sad and shocking because Rich worked very hard to become the person he became and people gravitated towards his charisma.

    It's not rocket science.. when you drop dead without warning it's typically cardiac etiology. Likely went directly into ventricular fibrillation, which causes instant loss of consciousness. If it was a "heart attack" which is a myocardial infarction meaning part of the heart dies due to a clot in a coronary artery blocking oxygenated blood from feeding the heart, typically there's chest pain, shortness of breath, nausea, etc.
    All this shit is pure speculation so it's not really worth talking about

    Sir, your sympathetic explanation is quite heartwarming. He, by his own admission, was taking risks like everyone else in that game. He wasn't an idiot, but I think like everyone who isn't a scientist and has 'lay' knowledge of drug interactions, and who at some point crosses the line between wanting and needing drugs, loses control. I knew Rich for years - many years. He was a truly nice guy. I think your overlooking the amount of pain he dealt with on a daily basis - joint pain in specific - led to use of medications to alleviate that pain. So, any speculation regarding the make-up of what was part or not part of hs drug cocktail is specious at best. I am not really hear to pick that apart. Bodybuilding at this level and the image that Rich portrayed as 'killing everything' and ultra-super human, comes with such large risks that his death should be a warning that this industry is getting out of control. We were all concerned about Rich for years, and several of us spoke candidly with him about it. It all contributed.

    Another interesting thing was in his videos he'd be driving around at midnight fully awake with Sara getting ice cream etc..... I'm 50 and train and after a 10-12 hour work day I'm wiped out-when I was 25 running around with girls from the gentlemens club I worked at we were stimulated back in early 90,s and I recall having ton of energy and grabbing breakfast at 3-4:00 am after I full day's work-too bad one of his friends couldn't of intervened😣

If it so requires go to college...but many make more than nurses like electricians and troopers...believe it or not after you put in about 8 years make more than nurses...no college debt. The whole point is go out there and you can make a living ( if you don't already ), make an impact and build your own family....have kids...and meet the girl or guy...of not of just dreams ( this isn't fantasy ), but that is right for you.

    Sleep apnea is a gigantic bitch. Going through that now. Had five heart attacks and three strokes. I had congestive cardiac failure.
    Had a big decision or death from large heart. Been clean eighteen months now. Growth was my love. But I. Feel better now

      VeggeMeister No thanks I am perfectly healthy. For your information I am 6ft 82kg, fit and never touched anabolics in my life. Also going vegan will not change the fact that each year billions more animals will be slaughtered for food. Last year the count was 56 billion and let me tell you why this number will continue to go up contrary to what you believe. There are over 7.5 billion people on this planet and by 2050 Africa's population alone currently at 2 billion is projected to hit 5 billion. By this time the annual killing of animals for just food alone will be in the hundreds of billions, not including sea life. This will be fact by 2050. Technology and health is improving which means more people which means more animals will die not just food related but the fact that we are a major imbalance amongst the rest of the species. If you drive, use technology, travel etc you are still contributing to killing. Even being vegan you contribute to killing small animals that get killed by the machines that grow the vegetation you eat so whatever you think there is no escaping it. Technology is the main reason why we went from 2 billion to 7.5+ so you can blame it on the very same device you used to replied to my message. The only way this number goes down is if the population plummets but we both know that aint going to happen.

      VeggeMeister First of all I know nothing about this guy. This is the first video I have seen from him so you are wrong about everything you said about me once again. On the topic of cancer the truth about is it isn't from one cause or another. It is triggered by a mutation in our DNA and there are literally thousands of ways we can get it m What we do know is that it is mainly seen in the elderly which tells us something as to the cause. As we get older our immune systems weaken so my priority is to keep my immune system healthy and strong and I can't even remember the last time I got severely sick. There is also genetic factors that contribute so you firstly need to know how your own body reacts to certain foods and adjust from there through trial and error. Also good to know a history of your relatives and if they have had any problems.
      Here is my list of things to do in order to function optimally.
      1. Fasting (anywhere between 16-72 hours)
      2. Control your sleep/wake cycle (keep it consistent and avoid blue light such as tv's, mobile phones, computers etc during the night as these will decrease melatonin - low levels of melatonin have been linked to certain cancers - Download a program called f.lux if you want to use your phone or computer during the night and dim tv and if you are getting shitty sleep take melatonin in low doses)
      3. Exercise / Weight train consistently (lots of reasons why this is important but main one is for bone density. My bone density at age 33 is super high - also in combination with fasting and weight training this will deplete glycogen levels and your body will burn fat when glycogen is depleted. This is how I am able to stay lean all year around. Of course you need to be familiar with macronutrient breakdown and calories in vs out etc)

      VeggeMeister Also I have accepted reality in that even if I was to be vegan and stop eating meat it will not change the fact that 56+ billion animals will die each year because sadly it is the truth and as I said knowing the population will continue to rise this number will only get higher. At the same time if a pipe dream of say the government was to ban killing animals for food and that eating meat is illegal I would have no problem with that. All this is out of yours and my control because these are billion dollar industries and are not going anywhere. I help and save animals that I can and have the power to do so.
      I know you are angry about it all and that your experience of having witnessed a slaughterhouse first hand has led you to spread the word of veganism which tells me you have an awesome heart but I also know you know there is nothing you can do to prevent it from happening and it kills inside knowing this reality.

    Glad somebody put it in honest & understandable terms.
    I've always applauded Richs honesty, he also admitted synthol use for a while but as u said he was very knowledgeable in his use of substances & he himself discouraged the use of anabolics & other substances unless u were competitive. Not saying that's great advice but again I applaud his honesty. If you want to honor his memory for being so genuine & letting the public into his life just stay natural, as he said it's more about how u eat & how hard you work. Unfortunately he became addicted to size but even he said if you're deciding weather to try anabolics or not do yourself a favor and don't even start.

    Rich also took HGH extensively for a long period of time when he was trying to compete. He even stated that his hands and feet were enlarged because of the abuse of the drug. Now, would that cause his heart to be enlarged as well?

      hGH abuse DOES cause the hands, feet and facial bones to grow. It will cause the heart to grow but at higher doses and for much longer than what Rich was using. For example, Andre the Giant died from an enlarged heart but it took decades of mega doses of hGH to enlarge the heart.

      All tissues are affected It would seem all parts change in the presence of Anabolics from fat cells to your T cells to your red blood cells muscle fibers in soft muscle tissue as opposed to like hard muscle cells found in say your biceps or forearms but soft muscle is found in the heart and is not supposed to be exposed to exogenous amounts of steroids be it Anadrol or Stanolozol to Teen to whatever its for Race Horses n Super Soilders like Comic book shit nobody is supposed to be using these Enhanchments for anything other than Aids or Cancer or Anorexia but Guys are fools they die 30 years early. Yes it was the Anabolics his heart had CHF and his ejection fracture was low below 50 due to the roids it stretched out the elasticity and overall it blew out the valves in his heart. It could have been coke as well speed too that was the deciding factor or elephant that broke the camel's back if you will... If you juice you aint supposed to ever indulge not even at a party once or twice especially once your a mass monster like that His cardiologist should have explained absolutely no stimulants other than a couple cups of coffee for men over 300 pounds over 40 on juice too

    I trained with a guy who in his late 40s was diagnosed with an enlarged heard. He did heavier cycles in his late 20s early 30s. Then beyond that he did 500 mg test and possibly a lower dose of deca though infrequently. So his last ten years of use was very low doses. He had an enlarged heart. So the guess Rich's heart was not enlarged is a very I'll informed assumption

    In some of Rich's videos I would notice that he would sweat a lot and breath heavy, I know that is not normal unless you are working out, and when he did work out he would get tired fast and he looked as if he could not lift.

    Thanks for that very informative video. As a natural bodybuilder I like to be informed properly. From what you describe its a wonder he was even able to train. He must have had an incredibly strong body in the first place. Mental strength is a massive asset if used correctly, but it can also lead to this sort of thing as well. Learning to live with slow, small gains by being natural (rarely standing out) also requires great mental strength. It's a quiet, personal strength that serves you throughout your life.
