Hi Fatma, thanks for the question. Androgen excess (too much testosterone, DHEA, DHT, etc) underlies both PCOS and androgenetic alopecia, thus many women with PCOS have androgenetic alopecia and vice versa. PCOS really is just a collection of symptoms and if you have at least 2 of the 3 rotterdam criteria (1. androgen excess, 2. polycystic ovaries on ultrasound, 3. oligo/anovulation), then you are considered to have PCOS. www.racgp.org.au/afp/2012/october/polycystic-ovary-syndrome/ It sounds like you have hirsutism: excessive hair on the face, chest and back. So hirsutism and androgenetic alopecia points to androgen excess-->fulfilling 1 of the 3 criteria. When it comes to the menstrual cycle, there are many women with PCOS who menstruate regularly, but they are not ovulating or not ovulating regularly. So you can ask your doctor for an ultrasound of your ovaries, and track your body for signs of ovulation. Just took my son to the dermatologist she looked at a scalp and said he had antigenic alopecia and he should use minoxidil. No talk of hormones no talk of nutrient profiles nothing. Thank you for normalizing the experience of losing hair at a young age! I have lost about 1/3 of my hair in the last year. Still trying to identify the root cause. Birth control may have triggered it (was only on it for 5 months), but 10 months later, I’m still shedding excessive amounts. Now seeing doctors to discern between thyroid and PCOS disorders. I have diffuse thinning, which more around my temples. It’s been devastating. My question is: can you shed more hair due to excessive androgens, but NOT have androgenic alopecia? I have high DHEA and total testosterone levels. But I’m wondering if this is consistent or just a product of periodic hormonal imbalances. Do you have any light to shed? Hehe. Unsure if this is me. My blood test came back fine. Recently I started lifting weights and gained a lot of muscle. Out of nowhere my hair started falling out in clumps and thinning profusely. I developed cystic acne as well. I had assumed it was tellogen effluvium and would go away but what you are saying about, testosterone makes me think it was the weight lifting? Hi Peter, thanks for the question! From my research, it seems only testosterone is converted into DHT. However, checking DHEA-S is still a good idea because it can reveal whether you have adrenal androgen excess. As the adrenals also produce testosterone and androstenedione (which can convert to testosterone), DHEA-S excess will likely also translate to high testosterone produced by the adrenals. As far as I know, the only DHT test is a serum one, and it may not pick up high levels in the scalp. Please let me know if you find out anything new, always looking to learn :)

Hi, my reports came. My Free testosterone is 1.62pg/ml and Total testosterone is 51.25ng/dl which are within the range. Also my DHEAs is 496.80 ug/dl which is on the higher end of reference. Is is normal? Because i am experiencing more than 300+ harifall a day and I have lost 50% of my hair in just 2 months.

I am so desperate after the last doctor visit when he told me I have androgenic alopecia 💔, i had hair loss since forever and my hair texture has changed also i have oily and brittle hair, i don't want to believe I have AA bcz i already have hypothyroidism (I take medication for that) also I have low ferritin lever and low vitamin d and zinc which I took supplements for but didn't help (still taking them anyway) also I have acne even tho I took two accutane cures and hirsutism , today I'm going to do an ultrasound to check if I have PCOS(I do have regular period tho) , well it can be better that having AA also I don't remember any of my family having hair loss but me, i may sound negative but I'm so sad for losing my hair and my skin I look sick all the time i wish to find another reason for my problems

I have alopecia. it started in my mid 20s now in my early 30s I have tried alot of different things to promise to stop hair loss. I take finastride just as long as I do not plan on getting pregnant while on it and I use the bosley hair loss shampoo n conditioner. those 2 things an trying to manage my stress are the best things to help control it. the bosley shampoo is on groupon and the price isnt to bad. the finastride was prescribed by a dermatologist that diagnosed me with the alopecia

My doctor said there was no point in getting DHT levels tested because it's not necessarily about how much DHT you have but more about how sensitive your follicles are to it. I've paid for so many blood tests and they have all come back normal. Usually that would be a good thing but with hair loss you almost want something to be off because then you might be able to fix it. Instead, all I've basically been told is that it's genetic and apart from trying rogaine, there's nothing I can do. Have you had your thyroid tested, as that can cause hair loss too?
its EXACTLY the same for me, and thyroid is also normal... I have done so many blood tests everything seems to be fine, the only thing they recommended me was minoxidil (rogaine) and finasteride topically but it irritates me too much and also my scalp hurt so i stopped... Im thinking on buying a topper because I spend so much time fixing my hair and never feel good about it...
I haven't started minoxidil yet. I'm afraid to start because it's a lifetime commitment and I'm also afraid of losing more hair initially. Did you lose a lot of hair when you started using it? They never mentioned topical finasteride to me. My doc said he wouldn't prescribe it orally because of my age ( 32 then and 35 now) My thinning is mainly at the front top of my head. I'm not wearing a topper or anything yet because I can sort of hide it with a side parting and sometimes use fibres but it's the constant fear of what's to come and how bad it will get. I know people say "it's only hair" but this really is soul destroying.
Do you have any suggestions on what type of doctors to see? I saw my PCP and then a dermatologist and both did not really seemed to concerned. The only thing my derm did was write me a prescription of the spironolactone. He "did not think anything was wrong since my general blood work was fine". Its all so frustrating..

Even if general bloodwork is fine, losing hair means something is wrong: whether it is hormonal (PCOS, androgen excess, low estrogen, not ovulating, thyroid issues) and/or caused by certain medications (BCP), nutrient deficiencies (iron, zinc, protein). Also important to look at different psychological stressors. It really takes a holistic view of all the body systems to pinpoint what the problem is. And it also takes looking at optimal ranges to interpret labs instead of just reference ranges. Therefore I recommend finding a functional medicine practitioner or naturopathic doctor who focuses on women's hormonal health and can put all the pieces of the puzzle together. I’ve started noticing hair loss since I was like 12yrs old. Now I’m 32 and I went to see a Dermatologist he stated it’s female pattern hair loss. Of course they prescribed Spironolactone & Birth control. I also saw a Gynecologist bc I present all the symptoms of PCOS every single one you mentioned in this video and did not diagnose me with it. All my labs have came back normal for several years. I’ve lost hope, as now my hair loss is so noticeable. It makes me feel so insecure on top of the cystic acne, dark patches around my neck and underarms, back, even in my buttocks, i have excessive hair everywhere.

@@speakyourself7920 It wasn’t pleasant and Spironolactone is a blood pressure medication and I already have normal to low blood pressure so it definitely would bring my blood pressure too low. So I stopped taking it. When you don’t take it for it’s intended purpose it can also mess with your electrolytes from what I was informed. But that’s why dermatologist order labs every couple of months when prescribing it for hair loss to make sure there are not imbalances in your blood levels.

I do recommend seeing a naturopathic doctor who focuses on women's hormonal health to help you through diet, lifestyle and natural supplements! This can definitely be done and if you can lower your androgens via reducing insulin resistance/stress, increase your estrogen by improving the signaling in your hormone axis, and supporting your thyroid, you can grow your hair back :)

Thank you for normalizing the experience of losing hair at a young age! I have lost about 1/3 of my hair in the last year. Still trying to identify the root cause. Birth control may have triggered it (was only on it for 5 months), but 10 months later, I’m still shedding excessive amounts. Now seeing doctors to discern between thyroid and PCOS disorders. I have diffuse thinning, which more around my temples. It’s been devastating. My question is: can you shed more hair due to excessive androgens, but NOT have androgenic alopecia? I have high DHEA and total testosterone levels. But I’m wondering if this is consistent or just a product of periodic hormonal imbalances. Do you have any light to shed? Hehe. You also talk nice and slowly for people to understand, which helps when talking about scientific details. Unsure if this is me. My blood test came back fine. Recently I started lifting weights and gained a lot of muscle. Out of nowhere my hair started falling out in clumps and thinning profusely. I developed cystic acne as well. I had assumed it was tellogen effluvium and would go away but what you are saying about, testosterone makes me think it was the weight lifting? Hi Peter, thanks for the question! From my research, it seems only testosterone is converted into DHT. However, checking DHEA-S is still a good idea because it can reveal whether you have adrenal androgen excess. As the adrenals also produce testosterone and androstenedione (which can convert to testosterone), DHEA-S excess will likely also translate to high testosterone produced by the adrenals.
As far as I know, the only DHT test is a serum one, and it may not pick up high levels in the scalp. Please let me know if you find out anything new, always looking to learn :)

Hi, my reports came. My Free testosterone is 1.62pg/ml and Total testosterone is 51.25ng/dl which are within the range. Also my DHEAs is 496.80 ug/dl which is on the higher end of reference. Is is normal? Because i am experiencing more than 300+ harifall a day and I have lost 50% of my hair in just 2 months.
Does that mean there are different forms of anderogenetic alopecia? Like it’s not always thinning at the crown and at the front?

Same with me. Apparently it isn't always caused by excess testosterone but by a person's sensitivity to DHT. It is so depressing!

My doctor said there was no point in getting DHT levels tested because it's not necessarily about how much DHT you have but more about how sensitive your follicles are to it. I've paid for so many blood tests and they have all come back normal. Usually that would be a good thing but with hair loss you almost want something to be off because then you might be able to fix it. Instead, all I've basically been told is that it's genetic and apart from trying rogaine, there's nothing I can do. Have you had your thyroid tested, as that can cause hair loss too?
its EXACTLY the same for me, and thyroid is also normal... I have done so many blood tests everything seems to be fine, the only thing they recommended me was minoxidil (rogaine) and finasteride topically but it irritates me too much and also my scalp hurt so i stopped... Im thinking on buying a topper because I spend so much time fixing my hair and never feel good about it...
I haven't started minoxidil yet. I'm afraid to start because it's a lifetime commitment and I'm also afraid of losing more hair initially. Did you lose a lot of hair when you started using it? They never mentioned topical finasteride to me. My doc said he wouldn't prescribe it orally because of my age ( 32 then and 35 now) My thinning is mainly at the front top of my head. I'm not wearing a topper or anything yet because I can sort of hide it with a side parting and sometimes use fibres but it's the constant fear of what's to come and how bad it will get. I know people say "it's only hair" but this really is soul destroying.

Do you have any suggestions on what type of doctors to see? I saw my PCP and then a dermatologist and both did not really seemed to concerned. The only thing my derm did was write me a prescription of the spironolactone. He "did not think anything was wrong since my general blood work was fine". Its all so frustrating..

Even if general bloodwork is fine, losing hair means something is wrong: whether it is hormonal (PCOS, androgen excess, low estrogen, not ovulating, thyroid issues) and/or caused by certain medications (BCP), nutrient deficiencies (iron, zinc, protein). Also important to look at different psychological stressors.
It really takes a holistic view of all the body systems to pinpoint what the problem is. And it also takes looking at optimal ranges to interpret labs instead of just reference ranges. Therefore I recommend finding a functional medicine practitioner or naturopathic doctor who focuses on women's hormonal health and can put all the pieces of the puzzle together.

I’ve started noticing hair loss since I was like 12yrs old. Now I’m 32 and I went to see a Dermatologist he stated it’s female pattern hair loss. Of course they prescribed Spironolactone & Birth control. I also saw a Gynecologist bc I present all the symptoms of PCOS every single one you mentioned in this video and did not diagnose me with it. All my labs have came back normal for several years. I’ve lost hope, as now my hair loss is so noticeable. It makes me feel so insecure on top of the cystic acne, dark patches around my neck and underarms, back, even in my buttocks, i have excessive hair everywhere.

@@speakyourself7920 It wasn’t pleasant and Spironolactone is a blood pressure medication and I already have normal to low blood pressure so it definitely would bring my blood pressure too low. So I stopped taking it. When you don’t take it for it’s intended purpose it can also mess with your electrolytes from what I was informed. But that’s why dermatologist order labs every couple of months when prescribing it for hair loss to make sure there are not imbalances in your blood levels.
