Usually, a simple liver cyst does not cause any issues and does not require any treatment unless there are symptoms or if the cyst grows in size.

If the liver is bright, the diaphragm can be seen weakly and the hepatic veins can be seen weakly, but the wall of the portal vein cannot be seen and the length of the liver is 18 cm is this fatty liver grade three ?

Yes that is correct although the liver length may not always be 18 cm.
