  • In this video, I reviewed the recent CDC research that was presented on COVID masks. I also review randomized clinical trial data on COVID masks from Denmark and Bangladesh. Do different types of masks work and what kind of reduction of risk can you expect?

    I am a consultant in Internal Medicine with special medical interests in gastroenterology, cardiology, and echocardiography. I am a a full-time practising physician in these areas. I was an undergraduate at the University of Toronto, Trinity College where I received a number of scholarships including one for top student at Trinity College. I attended medical school at the University of Toronto graduating with a gold medal. My internship was completed at McMaster University in Hamilton followed by a residency in Internal Medicine at the University of Western Ontario in London. I then completed a fellowship in General Internal Medicine at the University of Western Ontario. I am an active echocardiographer who has been certified and recertified by the National Board of Echocardiography. I am certified in cardiac sonography and have trained and completely educated a number of cardiac sonographers. I am the medical director of my cardiology laboratory which was established in 2001. My laboratory performs echocardiography and stress echocardiography amongst other tests. I maintain my certification in the American Board of Internal Medicine. I have over 27 years of experience as a hospital-based consultant in internal medicine and intensive care unit attending physician.

    Excellent talk. As a non-medical scientist, I am becoming increasingly disappointed with the application of statistics by the CDC in the context of this pandemic. It’s even more concerning when a purportedly scientific organization is shall we say, over interpreting a weak statistical study.

      As someone that is not a scientist, but comes a from family of doctors and scientists I am amazed at how much basic math is glossed over. They have so much trust in the system that they don't believe the cdc and Fauci can be so off the mark. Anyone with a high school level math education should be able to interpret this data and see things don't add up to justify the mandates, restrictions, lockdowns. The pandemic of the unvaccinated was the biggest lie and easily disproved with simple math a 6th grader should be able to do.

      The CDC is not a scientific organization, they are a government agency, and subject to politics like any other government agency.

    This is concerning. As a public health employee at the state level, our job is public trust over everything else. We have a hard enough time dealing with fake- or mis-information. Now we have to deal with poor analysis by the CDC and ASPR. What becomes more challenging is the political agenda that might motivate such questionable conclusions. The CDC has been wrong about natural immunity. They have politicized vaccine mandates to the point that well intentioned public health employees can't talk about the benefits. There is no conceivable way that we can restore public trust.

      Just as all politics are local, all trust in institutions is local. My local health employees are trusted so far and know we have their backs as do many localities but when it reaches state and federal levels, there is little or no trust. All you can do is follow your conscience and hope for common sense epidemic.

      I haven’t seen a medical professional in 2.5 years due to all this Covid mess/lies/back-and-forth. I don’t plan on seeing one anytime soon. It will be natural self-care, a naturopathic doctor or just death for me. And I’m totally okay with that. My trust in doctors is next to zero and who can blame me?

    My disgust with the CDC just ever deepens. I used to assume they were careful, thoughtful, analysing the data carefully and sensibly like Dr Moran. That's been thoroughly disabused, again and again, by the CDC. We need to disband government agencies that don't serve their purpose, and replace them with something effective.

      The CDC has greatly served its purpose and has been massively effective. Just depends on the point of view you are looking from...You are 100% right in disbanding all government agencies that aren't about the people and for the people. I'm not sure if we can ever get that done without all-out
      war unfortunately.

    An N95 must fit correctly to work. That's why we have to do fit tests annually. If there is a slight leak it does not protect. If it fits correctly breathing becomes difficult and have seen ppl pass out

      I've worn gas masks and those were tougher to breath in than N95. N95s were considerably easier to breath in than a gas mask.

      You're correct. It has to be fit tested on a machine for leakage. It's done individually. Critical care RN with TB patients

      @@shootingbricks8554 it is nearly impossible for anyone with heart or lung disease to wear N95 for longer than a few minutes. I passed out in home Depot when wearing one. I have heart disease. There are Tudors showing considerable negative impact on people with those underlying conditions. Kids also CANNOT wear them. They were never designed or intended for long term wear

      Depends on the mask. I can breath ok with 3M Aura. I fit tested that with vape (made a video on my channel).

      @@milenalm5288 That is true. In that case a PAPR device (a powered respirator) is the best. I own JSP Jetstream (made a video about it) and JSP Powercap (havent made a video yet on it but there are plenty in youtube).

    My informal study of the CDC concluded that they are 98% effective at supporting the narratives preferred by pharmaceutical companies and associated businesses. The range for statistical error of the study was around 2%.

      As an EXPERT, I will give a peer review...I believe the statistical error will be closer to less than 0.1%. Otherwise, an excellent EXPERT analysis. Keep up the excellent work. May truth prevail. Oh, yes, I will add there is no conflict of interest. Of course, please consult you PCP and stockbroker.

      @@lionheartmerrill1069 probably bc telling someone to wear a mask has caused quite the uproar so anything else is probably a waste to recommend. Infection through the ear is likely much less common than the eyes, nose, mouth though.

      They are in CYA mode. For two years they made unscientific recommendations that were turned into mandates, and now that public opinion is changing, they are trying to justify it. There is no other reason to do a study like this as a pandemic is winding down.

    Such a brilliant video. I also like watching md mph Vinay Prasad who also made a video on this study. How is it possible that CDC published such a poor study. Only thing they are achieving with that is losing public trust. Such actions are very harmful because in the future many people will remember what CDC was publishing and saying back in the days and will discredit them because of that.

      The CDC is nothing but the arm of the government that funds it. They do what they are told to do by the politicians so they keep getting funded. They are not trying to protect us they are trying to control us. They published the story because it fits the narrative the government wants us to follow.

      I was in the grocery store a couple of weeks ago and someone was shopping was wearing a neck ring given how cold it was outside and he brought it up over his nose to make it a “mask“.

    Some of the studies resemble work we used to encounter in electrophysiology. We classified them as from the journal of non reproducible results.

    I love people asking for the risk/benefit analysis of everything related to this pandemic. For a long time, asking for such analysis was enough to get one labelled as "antivax" and "anti-science". And as noted here, much of the data provided by the various scientific, medical and government sources was contradictory (or made no sense).

      People are horrible at risk benefit analysis. During the Omicron wave every time I went into a store I had to be the ONLY triple jabbed person not wearing a mask which is absolutely ridiculous. Oh and guess what? No COVID still our Omicron wave is over in NY and I have magically dodged it again (didn't get Wuhan, Alpha, or Delta either). I honestly can't believe it and I kind of want to get an N protein antibody test just to see if I have had an asymptomatic infection and just never realized it

      @@AdamSmith-gs2dv Answering to your first statement: individually, yes, because most of them are trained to follow arguments of authority. This is in a sense normal, because all people cannot spend their time reading and understanding. But as a group, it tends to work out, because the ones that get informed tend to make the good analysis. The problem now is that the governments, media and big data companies allied to suppress the opinion of many of these people that think for themselves.

      Risk/benefit analysis is what should have gone into policy decisions. Both Trump and Biden completely abdicated their roles in ensuring that policy weighed all that, and instead turned whatever NIH and CDC recommended into mandates with no regard to cost/benefit analysis. A 5 or 10% reduction in the rate of spread for a disease that has a rate of spread of 10, will not reduce total illness, so why would we resort to draconian approaches if in doing so we can not reduce deaths nor bring the pandemic under control?

    Great info Dr Moran! I have known most of this info since early last year after watching the Japanese Health Dept video testing each mask in a controlled laboratory conditions. The N95 was really the only mask to actually minimize Covid transmission. The virus blew right through the cloth mask like it wasn't even there! I wonder if lawyers were involved adding the # sign next to the cloth mask picture on the CDC site! Lol! I hope your cats are doing great!

    As regards your comments on the couple on the motorbike, science has shown that it's not necessary to wear a helmet on all your rides: they're only helpful on those rides where you're going to crash.

      When I see mothers pushing their babies in prams regularly alongside busy roads where deisel powered cars and trucks bath them in deisel particulate matter I wonder to myself what risk level does this present to the newborn over the rest of their lives? DPM nanoparticles cross the BBB....

      John Crowley, there is a very striking difference on your comparison- You are most likely to die by head injury if you don't wear a helmet and crash but wearing a mask or no mask the chances of getting infection is not much of a difference and the chances of dying is also very less below 2% and that also mostly those with co-morbidities and obese.
      Science is not simple.

    The fact that they are still mandated in Canada is absurd to me.
    The young children are going to be damaged from all of this. We need to normalize quickly for their sake. Wasting time shouldn't be an option.

    What's next? I am expecting the CDC to release a study showing how ice cream causes drownings, because the most drownings happen right after a spike in ice cream sales. They will say eating only half your ice cream will save 5 lives a day. You want to save lives don't you? Some people have never heard of 3rd factor correlation.

    Good morning, Dr. Moran. I’ve been waiting for this news. I’ve had Covid twice & have never felt the masks have done very well. I’ve had the infection 3 times & im extremely careful when around other people. Thanks for keeping us safe & informed. It’s sad that a large scale study hasn’t been done as of yet.

    There will be people say masking is usefull & vice versa. Since there are so many factors whether you will get covid or not, how about do whatever you think will help you without forcing what you believe. It is the fact that WHO & CDC made mistakes before in their “suggestions” & they are not pure health agencies. There are some political things in it.

      The CDC and NIH should not be making policy. Policy needs to be balanced, taking into consideration costs and benefits and psychology. When government mandates masks, most people think that means that masks are highly effective, and because they think masks are highly effective, many will go out in public and do things they wouldn't have done without that false sense of protection. It may well turn out that mask mandates actually increased the rate of spread as a result.

    I was a mechanical engineer (now retired) who did research on aerosols and filtration early in my career. I also served on numerous safety committees at the national scientific laboratory I worked at throughout my career. I my rule-of-thumb about mask wearing is to look at where I am and ask the question, whose air (i.e. exhalation) am I breathing? It is not hard to figure out that a crowed enclosed room or even a shoulder-to-shoulder crowd outdoors is probably a high risk environment. I first started wearing my N95 mask at a Chinese New Year event outdoors in Bangkok at the very beginning of the pandemic, January 2020. I agree with Dr. Moran that an N95 mask is the way to go if you want to protect yourself from others but you also need to realize that the best protection is to isolate from others. A mask reduces your risk. It doesn’t eliminate your risk of exposure. My safety training experience also tells me that greater doses matter so limiting time of exposure will lower risk as well. And BTW here in Asia everyone wears masks all the time and I seldom see cloth masks anymore.

      @@suhubu1 As far as comfort is concerned I am not bothered by wearing the mask. The standards of use in a medical or laboratory setting don’t have to apply in a public health setting. You are just trying to do something reasonably effective in the circumstances of being around a lot of people. To avoid infection most effectively you should isolate. If you have to go out then wear the best mask you can.

      There was a study done of some army people and masks made no difference because they were in an enclosed space for a long period. Basically, living with somebody or being in the same room all day (school, work) then masks will be overwhelmed.

      @@AlexSmith-gr4hp Any situation where you are in close proximity to a lot of people, in a confined space, for long periods of time is a high risk event. If those people are talking loudly the the risk increases. If you were the only person to be wearing a mask (N95) then your risk would be lower but not zero. This way of thinking would work outdoors in some circumstances. Say if you were in a crowd of raucous people shoulder to shoulder in front of a stage during an outside concert. Always ask yourself whose air am I breathing and for how long?

      I am an ordinary citizen and I started wearing p100 mask first week of January 2020. Now I wear British military n99 mask with carbon and silver layers. I don't really trust N95 because you still have 5% unfiltered. The silver inside my military mask kills the virus and they are reusable. A bit expensive but worth every dollar.

    There is a good reason why there aren't randomized studies on masks. Its because the answer which is rather obvious would not be politically palatable
    Youre being very generous to the cdc. That graphic was not made accidentally, its practically made to be shared on social media and put up on TV news. It also means that there were a bunch of review meetings where this was approved for posting.

      Actually the 3M company did these tests years ago. I have been wearing N100, P100, British military N99 etc masks since the first week of January 2020 along with safety glasses. I knew from the very beginning this was Airborne I don't understand how it took the medical community so long to understand this.

      @@James-ke5sx transmission is predominantly via respiratory droplets but can also show airborne transmission is certain settings such as in poorly ventilated areas. Airborne and droplet are often used interchangeably but aren’t the same.

I also knew that unless you cover your eyes you can get this virus in your tears which go into the tear duct in the inside corner of your eye which leads to your nasal passages this is why I started wearing safety glasses and a very high quality mask way before the medical profession knew what was going on

      @@dcottt95 It's not a mucous membrane infection of the eye I'm talking about because the virus is not entering the eye it's being Swept Away into the tear duct. I think in the eye you can get pink eye, congevitus. Even though the medical field knows about this how come they are just wearing a face shield when the virus can easily go underneath the face shield and onto the surface of the eye. you need full protection not just a face shield for an aerosol virus. The Alberta Health Services here in Canada heard about this virus in early December 2019 they had plenty of time to figure this stuff out but they all sat on their butts and did nothing . I am not a health professional but it took me only three days of research at an easy pace to figure out this was a aerosol virus it wasn't rocket science.

    I did a similar study to the CDC mask study. I walked around and asked 520 people if they had had a covid test, what the results were, and if they used masks. 23 of them responded to my questions, the rest walked away, and from those 23 I was able to determine conclusively that masks reduce transmission by 30,000% but ONLY in public restrooms at hardware stores. ;-P

    When I worked in the hospital in Canada we were tested once a year which size of N95 mask would fit properly around your face as every ones face is different. Once we had the N95 mask on, they put a device on our entire head and squirted in some very strong odor and if we could smell that odor the mask would not fit properly and therefore had to continue trying on different sizes until you reach you could not smell the odor.

    This study was done with random calls to people whom had tested positive. The vast majority of people did what most people do when they receive a call like this they hang up. I couldn’t think of a less scientific way to run a study,

    Thank you so much for a rational, evidence base discussion absent the politics. I think one of the biggest shortfalls I see in these discussions, and you alluded to it a little, is that cloth and surgical masks are inadequate to handle the SARS CoV2 particulates of 1 micron. The K95 can if they are properly fit tested and worn correctly. Unfortunately, I doubt people are following the NIOSH protocols for fit testing with hoods and saccharin spray. So anyway, thank you again sir--well done!!

    Dr. Moran I live in California and we have had really bad fire season since 2017. Some seasons we could see ash on our cars. My question is do we have any peer reviewed scientific/medical journal articles on fire smoke and ash inhalation and how that effect one’s ability to fight off SARS-COV-2? I do not smoke but I do worry about wildfire smoke inhalation due to the California fires.

      Thanks for commenting. Masks of course are not meant to be worn at high speeds, the fibers will not hold up and I do wonder how many of the wearers end up by inhaling some of the fibers.

      Rion, I'm not sure that there is any benefit to a lot of the cloth masks that people wear. I think people touch their face more often when they wear a mask to adjust it. Many people touch the front of the mask when they take it off. I think when people do randomized trials with masks they need to ensure that people are taught how to wear them and put them on and off properly. I made a video on this at the beginning of the pandemic because nearly every patient I saw coming into my office early in the pandemic were making mistakes with their masks.

    According to a statistical study on effectiveness based on mandates in every state, actually use, and other factors it showed mask decreased infection rates by an average of 13%. The study showed in the way it was presented that confidence in it working was more responsible than mandates and oppressive means. At 13% it could actually be in the range of a placebo effect for those thinking that.

    I tested masks with my laser particle counter. I can't talk about viruses but cloth masks do nothing to stop fine airborne particles at .5 microns in normal air. You will also end up inhaling fibers coated with chemicals from laundry detergents and fabric softeners if you wash them with your usual laundry. N95 masks did a great job at .5 microns but only fully sealed.

    Great video! I was surprised to hear masking 2-6 year olds is beneficial. I guess it's because it seems like they would be more likely not to keep them on correctly and because they aren't very cognizant of their germ spreading behavior outside of mask wearing making the mask less effective.

    My favorite mask story is the young couple I saw going full speed up a steep hill in a remote area while wearing N95 masks! Needless to say they were struggling to breath! As to the motorcyclists wearing masks but no helmets my guess is that they may not have anything in their heads worth protecting anyway?

    Fine. BUT, ***there are Instruments that directly measure the effectiveness of masks*** Particle Counters are ALWAYS used to test filtration practices, membranes, and the finished device. Effectiveness is reported as a Beta Ratio as function of particle size. Thus the Quality and Effectiveness are VERY well known, from HIGHLY CONTROLLED laboratory tests. Tests that are run daily, for the last 20+ years. This inform should be the baseline before virus transmission rates are done in low control environments. Yes, I'm a Design Engineer VERY familiar with these Instruments . You can compare the "Beta Ratio" directly between the various devices.

Ultimately, you can have the best mask but if you touch it and there is a significant amount of virus on the outside and you then touch your mouth and nose and get infected it was of no value to you.

    The reason for the N95 failure of 85% would include not properly sealed, and fake or Chinese-rip-off masks which abound.
    Just the people I've seen wearing N95 masks without proper seal would explain the discrepancy.

    I went to a tech school for hvac and from what I understand of filtration there is just no way any of these masks could physically block a virus at all and still let you breath. To be able to filter something with a barrier the holes in the barrier need to be smaller than what your trying to filter, with a virus is just to small to filter. Those are just my thoughts though, if you want a larger scale of this get one of them dirt sifters and try to get the rocks through without any of the dirt.

    Money is a factor. Those using N95 masks are those who could afford them, especially at the start of the pandemic. Those using paper masks would be buying boxes of them and changing them every use. Cloth masks are the cheapest, but those using them have to properly wash the masks between use. And many had allergic reactions to paper masks, possibly from the chemicals used in them or their manufacture.
    And concerning diabetes, it's very important to distinguish between type 1 and type 2. Type 1 is far more serious for those who get sick. Properly managed type 2 patients many not show blood sugars much different than those who are considered normal. These patents can benefit from limited eating. Type 1 patents who are unable to properly eat can see spiking blood sugars.

    I recently observed a cyclist wearing no helmet while dodging cars on a busy road in Toronto, ON - Canada. The cyclist was however protected from covid because they were wearing a blue mask. Still confused about this individual's prioritization of personal safety.

    As someone with a non-medical PhD (economics), this sounds like good science. His statistical concerns are the same kinds of concerns that I would have in an economics paper. I would like to see a public debate between Dr. Morgan and one of the scientists who wrote the CDC study. We can only get to the truth with such a public debate. I would need a public debate to be convinced that we should be wearing masks and that we should be masking our children. The fact that no one from the left seems willing to debate someone like Dr. Morgan is telling.

    I remember you talking about the Bangladesh study and I think you mentioned they estimated only around 40% actually wore the masks. When masks use was being publicised in the UK, a lot was made of the necessity of both people wearing masks rather than just one. IE. One reduces the amount of virus given out while the other reduces the amount of virus taken in. Would a low mask uptake therefore reduce overall mask effectiveness?

      Masks help a little without any risk. No one will be able to perfectly wear their mask at all times. At some point you gotta pull it down and touch it or touch your face. Masks are effective if you pop into the gas station and go back home but human error likely diminishes all effectiveness if around a group of people for any extended period. This study is trash, but masks decrease the Amount of droplets someone puts out and protects from someone sneezing around you for short periods so in that regard masks are better than nothing, but no one can wear a mask perfectly at all times and anyone expecting them to significantly reduce infection will not see it. Should be up to the individual if they want to wear it or not. But on the other hand, people trying to argue masks negatively impact health in whatever way they make up should just say they don’t want to wear one and that’s that. The nonsense about breathing your own CO2 Or getting less oxygen is completely false. No justification needed.. if they don’t want to wear it that is cool. Both sides of this issue have had their fair share of nonsense, but masks being dangerous is hilarious.

      Thanks for commenting. Well done remembering that in the masked villages only about 42% consistently wore masks. The pro-mask person would suggest that if more people wore masks you would have more benefit overall but we don't know that. You have to work with the data that you're presented with. The anti-mask person would say that you couldn't find a significant reduction with cloth masks so masking more people would not likely benefit. The 11% reduction with the surgical masks, as I mentioned in the video, Is biased because on further analysis by other researchers not involved in the study they found that more people were randomized to the masking group than the control group, It's supposed to be 1 to 1. So there were more villages that were masking and one might argue that those villages were keen to mask because they thought it was going to be helpful and that they were more conscientious and skewed the data.

    Is there significant data on P95 masks (commonly used by industry [OSHA] for quite a long time) with respect to virus transmission? It seems logical that those masks that are N95 but also capture VOCs would be significantly more "sticky" to the droplets and virions passing through the activated carbon layers. Of course, the P95's are significantly more expensive because the carbon filter portion has a limited lifespan and needs to be replaced after 8-40 hours of use depending on the style and brand of the filter media. But I'm still wondering why nobody is really talking about those. Also, aren't P95 filter material similar to the type that virus scientists are using when they wear a full hazmat suit?

    I have been wearing a 5 ply cloth mask with extra filter if wanted to add and it had kept me safe thru all phases of Covid so far and I am around hundreds and on weekends thousands of ppl some mask on some not and if not we give them a mask or have to leave, so at least my cloth mask 5 ply works extremely well.

    My partner and I both were at a social get together last month . We both wore our KN95 masks , the only ones who wore one the entire time. We never took them off. Out of 20 people , we were the only two to NOT get it. That was enough for me, masks work very well. At least the N95 masks do. I think the reason why they don’t work for some people is because they often take them off .

    My question is this: since viral particles are so tiny and pass through the mask fabric, attaches to saliva and mucus, is then exhaled whereby the larger infected droplet are caught by mask and then re-inhaled into the airways thereby possibly increasing viral replication?
