    Excuse my ignorance describing the Portuguese new story as Spanish during recording 🤦‍♂ They are just not languages I am exposed to frequently in Australia, but now that I've worked with both Portuguese and Spanish SLPs in translating my website, the difference is *extremely* obvious.

    How do you correct it?

  • @LindaDulcinea

    Would it be accurate to say that if someone has movement of the upper face but weakness of lower face they must have UUMN dysarthria or can this pattern on an OME still be present with flaccid, spastic, or mixed dysarthria?
    For example, if a person has this finding on OME (UUMN dysarthria?), but also presents with drooling (flaccid dysarthria?), strained vocal quality (spastic dysarthria?), and emotional lability (spastic?), how might you classify this?

    • @johnepierce
      @johnepierce  3 роки тому +2

      Hi Linda, I assume you are talking about lower weakness on one side only which does suggest UMN damage (most commonly stroke). However, this doesn't automatically result in UUMN *dysarthria* and can be present in other dysarthrias due to pre-existing damage to the contralateral side (e.g. an old infarct) causing spasticity; or a premorbid, unrelated neurological condition like MND, Parkinson's, an ataxia, etc. That's why it's important to use all the features you can identify as puzzle pieces:
      * Drooling is caused by impairment to oral motor control and/or swallowing. It's common in flaccid dysarthria but definitely present in others. If you can't gather and swallow saliva, you may drool.
      * Strained vocal quality is most common in spastic dysarthria, particularly if severe, but can be present in others.
      * Emotional lability is unique to spastic dysarthria as far as I know.
      Overall it makes a spastic dysarthria most likely AND a new stroke - it would be worth checking for hyperactive gag and asking about the onset (gradual or sudden).

    Good evening my son had a motorcycle accident and now he has severe dysarthria. She did a speech therapy course but with poor results, her condition is more complex and I would like to contact her if she has an email address. You will be very grateful to explain her situation. Thanks so much

      Unfortunately I can't get involved with treatment advice online. Spastic dysarthria is difficult to treat though so the speech pathologist may not have done anything wrong!

    Hello! Could you please explain why a slow rate would impact prosody??

    • @johnepierce
      @johnepierce  4 роки тому +3

      Yes I didn't explain that well - the strain/tension in the larynx means that the pitch and loudness are resistive to change, or at least rapid change, so the normal up and down within and across words isn't possible.

    If you need a fresh Spastic Dysarthria sample from a person about to be diagnosed with ALS I can hook you up. My most recent addition to my diagnosis process is SD w' tongue Fasciculations

    • @johnepierce
      @johnepierce  7 місяців тому

      Mate that's terrible to hear - hopefully you're wrong about the potential diagnosis. Are you seeing a professional?
      Tongue fasciculations suggest you'd have flaccid dysarthria (but if it was ALS, it's common to have *both* spastic and flaccid, which is called mixed dysarthria).

    • @HonorableHarbinger
      @HonorableHarbinger 7 місяців тому

      @@johnepierceoh that's..great, well I can definitely give you an example of mixed dysarthria in that case. I'm finding out in about 16 hours whether or not I have ALS. I definitely look like I do unfortunately, I was hoping it was something less bad. Hope your well mate

    so it looks like you are not familiar with the language spoken in brazil. it's not spanish. it's portugese--another related european language. just fyi

    • @johnepierce
      @johnepierce  5 років тому +5

      Yes sorry about that. I did realise but only after the audio was recorded, so I inserted a correction at 15:24. I don't hear either language often in Australia so I have to concentrate to distinguish between them!

    • @johnepierce
      @johnepierce  4 роки тому +6

      Sorry! Unlike the USA and even UK, Spanish is spoken by 0.8% of the population in Australia, and Portuguese 0.2%, so I just don't hear them enough to effortlessly distinguish. I did notice while editing and put a correction at 15:24 😭
