How To Make Your SI Joint Adjustments Last

  • Ever wonder why sometimes you get a chiropractic adjustment and you feel great and stay feeling that way? And other times the relief is only temporary, causing you to have to go back over and over to get relief. We are going to cover one reason that can happen with the SI (sacroiliac) joint. A fixation of the SI joint can cause lower back pain, SI joint pain, hip pain, and sometimes sciatica type symptoms. But maybe not on the side you would imagine.
    Many chiropractors will do generalized adjustments where the entire visit takes 2-3 minutes. There is nothing wrong with that for generalized mobility in the spine and pelvis, but with specific pain, especially chronic conditions that don't seem to be responding to care, it is imperative that joints that are fixated are adjusted AND (possibly more importantly) joints that are moving properly OR are hypermobile NOT be adjusted. Yes, you just had a chiropractor tell you that sometimes joints shouldn't be manipulated or adjusted.

    Absolutely the best and most comprehensive channel regarding SI joint related issues. I've been suffering for about six weeks with intense pain in my right S I joint.. The pain is especially intense when I get up and start walking, the pain radiates throughout my gluteus muscles. Is that something you hear from other patients?

    I see Dr Pederson and he amazing! I drive 2 hours and 15 minutes and have had 2 appointments. My problems were hip and lower back pain. I have no pain anymore! Definitely recommend Dr Brant P.

    I love your videos and use your exercises to strengthen the areas that you talk about. However, I cannot do the ones that are on my knees. Because of knee replacements I need issues. Is there a way you can show us an alternative way to do those?

    Hi Dr. Pederson. Thank you for explaining the SI joint and symptoms we get when there's a problem. Right now I feel pain over the right sacroiliac bone and I started to get numb feet day before yesterday. It feels more protruded and my left is flat even my left front groin is mildly tight. I feel my walk is not even and both groin are trying t start problems. I've been undergoing accu spina lumbar stretch treatments and then chiropractic adjustments. MRI revealed small cysts in my lumbar area also. I messaged th e area but did I cause the SI joit to protrude. Would a trochanter belt really work to stabilize it after and what can we do to prevent the SI and hips to become uneven between chiropractic trreatments? How should we sleep? Is it good to use a lumbar pillow for sleep? I don't know if the uneven hips or SI are the cause of my symptoms. Thank you for any insight.

    I'm fairly constantly bothered by SI joint pain ... and frankly got fed up with going for treatment. My SI joint was damaged with pulled ligaments some thirty years ago, whilst swinging a 28lb. sledge hammer. It never really recovered and over the years chiropractic helped a lot. One day I asked the chiropractor whether there was anything I could do myself to keep it in line so to speak. He told me to sit on a chair with good upright posture, and taking it in turns, pull each knee up into the chest whilst retaining relaxation in the sacrum area. I do this when I have a bad event, and it seems to centralise it, until the next time. I'm not sure what it's doing ... but works for me. Great video explanation. Thank you.

    Actually, I have encountered this situation before. When I did it for some patients using the conventional adjustment method, it worked well. But seeing some patients like the above situation, the doctor said, maybe I have to change to get better results. Thank you doctor. I come from Vietnam

    I'd love to know your thoughts on why my upper body - back, chest (ribs and collar bone), and neck - can't seem to hold an adjustment for more than a few minutes.
    I seem to be an easy to adjust in and easy to slip back out kind of patient and my chiro and I are trying to figure out what to do to help me hold the adjustments better and longer.

    Hi Dr. I appreciate your clear explanations. I too have wondered why I have to keep going back to my chiropractor every 2-4 weeks. I have a rib (sometimes 2 or 3) that are “out” frequently, causing me irritating pain in my right side. When I ask him what I can do to prevent/heal this problem, he says there is nothing. I did fall on that side 2 years ago, and wondered if I broke a rib, but never had it xrayed. I haven’t seen you address this issue. I’d appreciate anything you have to say about rib problems.

    Great video thank you. I constantly have pain in my left sij! sore to touch and in conjunction with left psoas pain. I am a therapist but cant treat myself.     Hi. I'm so glad that I found this video. I've been diagnosed with the exact same problem today. My lower left back is killing me due to this pelvis stuck in that position as you described. It's causing all sorts of pain and discomfort including sciatic pain, pins and needles in foot, stiffness in back leg and sometimes cold and warm feeling. My chiropractor said it's common and we'll be having a few sessions the following days to treat it. Hope it'll be fixed eventually. Thank you.

      I've been seeing a chiropractor with this problem for a while. Work in progress. We've had a dozen sessions now I still have not really feeling any major improvement. It takes time according to the specialist. I just really wish one day I can get back to normal life without pain and my pelvis will gain back it's normal motions.
      Could you please share some of your advice that could help me with the healing? Would you have any effective home exercise that I can do to speed things up perhaps?

      Great question! I think for a worldwide perspective on all things chiropractic I would look to the World Federation of Chiropractic (www.wfc.org). That should be able to give you information on schooling, and the practice of chiropractic in different countries. The profession is constantly improving and growing with many exciting things.     Enjoyed the video and am searching for a solution. I had surgery to remove a benign cyst in my roght illium five years ago. There was a hole drilled in my illium to remove the cyst and, in a separaste procedure, had a bone graft inside my illium. All of this was adjacent to the SI joint.
    I have been having worsening pain and difficult remaining erect, particularly when walking. I am very protective of my back and have an embarrassing forward lean. I have no numbness or pain resulting from my sciatic nerve.
    Based on the pain, I have had an injection, ablation, and PT an am still dibillitated at 63. Right SI joint was DX to be dysfunctional.
    I am looking for a path forward.

    Hi Dr. Pederson, I have had lower back, hip. SI joint pain for over 10 years and have done so many different programs to try to live life without Pain. I did have L5-S1 lower back surgery in May of this year and I am still continuing with mostly the same type of pain. I really think it is coming from my SI Joint because it has been so long can it be fixed without surgery?

    Wow, I wish I lived closer! Adjustments do not help my SI pain. I have recently started wearing a SI belt. PT gave me stretches and exercises as well as my sports med ortho MD along with celebrex (had side effects)so I continue to struggle with this. Do you have a link for specific exercises or stretches for SI instability or is walking the best? Also does Graston techniques help with SI instability? This may be something I can ask for locally. Your videos are awsome!

      Thanks for your inquiry. Finding a really good doc can be challenging. I generally have good success with recommending sports chiropractors for people looking for a good chiropractor in their area. Sports chiropractors (CCSP or DACBSP) have done a significant amount of additional training after their 4 years of chiropractic college, passed an extra national board exam, and have to take more continuing education each year than state licensure requires.     Now I'm confused. I had about 10 sessions with a chiropractor who identified SI joint and each session adjusted the sore side of the SI and left the other side alone. This sounds opposite to your treatment?

    I wonder. I have back pain in my upper back. Between shoulders. I sometimes crack my back in that aerea myself. But pain and thightness always come back an hour after. Perhaps I need to get another area adjusted..

      When I have a patient who has chronic upper back pain that feels like even after getting adjusted they need another adjustment in a few hours that it is usually caused by a postural issue.       Sometimes if you are cracking your back yourself and the pain comes back then it can be that the area you are "cracking" is not the real problem and maybe even becoming hyper mobile. Sometimes there is an area right around there that you can't crack and it is really fixated and if you get that area adjusted it might make your back feel relaxed and wonderful again.

    Pretty much everyday I come home from work and my sacrum area feels locked up and achy. I had a chiropractic adjustment about a month ago that helped and I felt better for about a week or so but now its slowly starting to go back to where it was before. I'm not sure what it is thats causing this but I'd love to hear some stretches or things I can do myself to help prevent or lessen the severity of my symptoms. Thank you

      Hi Justin,
      What a great question!
      Mostly what I teach my patients is to try and find symmetry as much as possible in daily life to avoid fixation and imbalance at their SI joints.
      Examples of things that through the SI joints out of balance:
      1) Sitting cross legged. Many times we end up putting one leg over the top of the other and favoring that 90% of the time because it feels "more comfortable this way." Try to balance that out or don't sit cross legged.
      2) Men sitting on a wallet in their back pocket. Creates an unlevel surface to sit on. Move it to the front pocket or elsewhere when seated.
      3) Driving long hours. Driving is a very right legged activity and as such people usually sit asymetrically in their seat when they are driving. Try to balance this out by bringing both buttocks back against the seat back equally and sliding your seat slightly forward might help this as well.
      4) Sleeping on your stomach with one leg pulled up is really hard on your SI joints. Try to sleep on your side or back.
      And getting regular exercise, like walking, is really important to maintaining good sacroiliac joint mobility and stability.
      I hope this gave you some actionable items you can put into practice to stay feeling great.

    Hi, I was just wondering what I can do to help relieve the pain in my hip/pelvic joint too! (I believe this is the same area) but say for instance if I’m laying flat on my stomach, it’s as if my hip on the right side is higher up ⬆️ in the direction of the ceiling. And this pain is constant, like it always needs a good pop. I’ve gone to a chiropractor in the past and they would relieve the pressure/pain by making an adjustment but this only lasted 1-2 weeks before I’d have to go back again. I committed to A LOT of sessions and figured nothing they’re doing seems to be working plus it was quite pricey at the same time! Which is why I had to stop. To this day I’m still feeling a lot of tension and pain here, what can I do now?! please help.

    Why does my fysio therapeut working on my upper back: shoulder and between shoulder blades when I have sacroiliac joint pain. He explained that it is connected. Is that true? Does that help with sacroiliac joint pain?

    I'm in Australia and have had BILATERAL Total Hip replacements. They were done in the first step of alleviating my back and groin pain, and assist with weight reduction as I was 118kg and 170cm tall (5' 7"). 7 years on all I can report is no groin pain back pain getting worse plus at the scar sites down my outer thigh I have nerve pain and cannot sleep on my sides. So now I sleep in blocks of an hour, roll over wake up in pain. They cant fix it either. If thats not enough my back pain is in three segments best described as follows :-
    Draw a straight line from L Hip to R Hip at L5 S1, then bring the outer edges together at L1, plus right at the hip joints as per this video
    Draw a straight line at the shoulder blades, bring that back to L1
    From the same line as above take that to the base of the skull
    Pain starts from the straight lines and is intense and unbearable at the points of each triangle.
    I have severe pain in those areas on movement. It goes from nothing to painful in the lower back from 20 to 50 M. Then pain in the thoracic area to neck area then pain in the mid back region. At that point I get a hot burning sensation down my left leg to above the knee its not like nerve pain or an electric shock that I get its like a spilt hot coffee and both legs begin to shake (the burning sensation is only on the left. That would be at 200-250M. Eased by sitting down and resting.
    Doctors over here say I'm not a surgical candidate but it will get better. Well 7 years later I can tell you it is not the case. 1.5 years ago the govt cracked down on opiate medications and I had reduced weight from 118 to 82.5kg. My weight now is 111kg. Almost where I started but now I cant sleep. I can not wear the brace you demonstrated due to the pain at scar site, it runs from top to bottom of each buttock.
    Apart from a bullet to the head :) do you have any suggestions? I feel like the man in the monty python movie, the meaning of life; just waiting to explode!! Thanks for reading.

      Im sorry to hear about how much you are suffering.
      I had 3.5 years of very nasty chronic pain in my back that disabled me and made me suicidal at times. Tbh its only because i have a wife and kids that I'm still here.
      I had 3 surgeries and none of them worked. They all left me in severe pain which would not go away. I never took as many meds as you. I just took to my bed for weeks on end getting more and more depressed.
      I was eventually recommended a physio to me who helped me understand how my whole nervous system and brain has learned to create pain even though my tissues were mostly fine. He recommended a book to me called "Explain Pain" which does exactly that, citing lots of scientific sources.
      Understanding where a lot of the pain was coming from and learning to fear it less and manage it better, along with an excellent physio who gave me the confidence to started moving again restored me to pretty much full function.
      If you haven't been down this path, it is something I would recommend trying.
      Edit: pain is so weird. I am here because i have a separate problem with my SI joint that is not chronic in the same way. But just thinking about your situation has made my SI joint pain worse. Mirror neurons replay your description of pain into my nervous system and raises my sensitive to pain. Crazy.

    I have a fused Sacrum up till Lumber 4, with partially herniated L1and 3. I have swelling on the top of my left SI joint. It is hard to the touch and feels like bone but is not mirrored on the right SI joint. I suspect ithe SI has popped or is stuck and would like to know how to reset it with self adjustment. In South Africa.....

    I was blessed when I found my chiropractor. He does adjust both sides. He took it easy on the painful side at first ( the pain was unbearable). 2 weeks later I’m just about back to normal!! Chiropractors for too long have been getting a bad Rep. I will always go to the chiropractor first!!!! ❤️

      I’ve been going to the chiropractor for 25 years. The good ones are absolutely worth every penny and bring great relief and healing. Unfortunately there are plenty of not so good ones out there as well. I’ve been to both and unfortunately the bad ones at times give the entire profession a bad name.

    This helped answer the question of why one side hurts because the other is locked up. But I thought there would be a way to make the relief last for longer after an adjustment. My pain and tightness comes back after a few days or a week. Can you help explain this?

    I have one side that seemingly doesn't move. I could always lean to left to get my right si to click/pop, and same to right. It started with a slight pinch in left si when stretching hamstrings forward.
    I was leaning right trying to release my left side, when it basically locked. When I attempted to straighten up i got the knife in my si. Adjustments are giving me pain relief, but that side still refuses to move!
    Still working on it!

    I have been going to a chiroprator for almost 6 months now. and I continue to spradicoly suffer with right sided SI and Piriformis pain. I get an adjustment, feel pretty good mostly for a week sometimes two and then it's back. any suggestions.

    Hello doc, i go to a chiropractor every 4 weeks. Im normally stiff around my left hip. I get both hips rotated and sometimes my leg pulled up depending if i need it. I get upper back and neck adjustment aswell. My back spasmed 5 yrs ago when i hurt it and its been a battle ever since. My job is sedentary. I.e. driving. Is it all necessary? Thanks.

      It sounds like you have a chronic injury that started five years ago. And it sounds like your 1x/month chiropractic adjustment helps to keep you balanced and feeling better at your hips and back. Also, because your job is sedentary it is important to keep your joints moving well. If I was in your shoes I would be doing just what you are doing. Great question!

    What about leg length disparity. I’m higher on one side by about 3/4 inch. Or more. My lower back / SI has been killing me son and off since I became a nurse.(35 yrs ago) One ortho told me nothing worse than a nurse with a bad back. I feel like I need chiropractic adjustment every day when I’m flared up. Plus I have terrible fibromyalgia. So would that little zap pen adjustor help. I’m desperate. I can only walk for about 10 in a store. 0:29

    I developed SI joint pain in my right SI joint after a fall on stairs where I landed on my left buttock. It happened a couple of years ago and at the time, I had pain for several months, including radiating pain. It eventually settled down but it is still vulnerable. There are movements it doesn't like such as a bird-dog pose which puts torsion through my lower spine. It also eventually starts complaining if I do leg raises lying on my back because I push my lower back to the mat and it doesn't like that motion.
    I have S1-L5 fused btw, since 2019. That is generally ok but i got a flare up recently so I have been much less active recently, in fact i have sone days mostly in bed waiting for the inflammation in my S1 nerve to calm down.
    I properly triggered the SI joint a few days ago doing some gentle forward bends to demobilise my lower back.
    I would really like to "fix" the SI joint if it is possible.
    I know a local chiropractor who I trust as he was the earlier the physio who helped me deal with many years of chronic pain after my spine surgeries. Do you think chiropractic adjustment can help with my SI joint?
    Does it seem plausible that the fall nudged my SI joint out of position and it can be reset into a healthy position?
    Should i hold off chiropractic until my S1 nerve has settled down (the flared up of my fusion op that I mentioned'?

    I have pain at right side of just above the hip while long sitting and raising my legs, also when bending forward. My physiotherapist has told me as it's SI joint pain. They did sij mobilization and made me do he kinds of exercises but the pain isn't going.

      I'm sorry to hear you are still in pain. It could be that you have hyper mobility in your SI joints and then stabilization with something like a trochanteric belt (SI belt - I like the one manufactured by Serola) might give you relief. You might ask your medical professionals about that and see if it is indicated in your case.

      However, If I was in your situation I would probably check out another health professional (another DC, MD, or PT) to see how they would approach your pain and getting you back to feeling 100%. When I am seeing a patient if they aren't getting better I am ordering imaging, labs, and/or giving them a referral to someone I think can help them.

    I recently started seeing a chiropractor for this with severe pain in left piriformis. My doc does not do xrays. he also is only adjusting the painful side. Do I question him or just find someone new? I've been 4 times and don't feel any better.

      In our office if we see a patient three times and don't see dramatic, lasting improvement we are looking for something else going on. We might order x-rays or an MRI, change up how we are treating them, etc.
      We have good success finding good chiropractors here: acbsp.com/directory/ These are chiropractors that have gone on after chiropractic school to do more studies in sports chiropractic and then passed board exams in that speciality. They also have to do additional continuing education each year to maintain their certification.
      Athletes don't put up with not getting results from their medical professionals. Sports chiropractors, in general, get results fast and if they don't are looking for what else could be going on.

    What if you’re experiencing a lot of popping and cracking from the SI joint region and it relieves some of the pressure and shooting pains? Does that still mean it’s the opposite hip?

    I suffer exactly what you describe. All the time since my last fusion surgery in 2018. Currently doing chiropractor, PT, and massage therapy with about 40% relief. I have voiced my concern about SI Joint pain with my chiropractor and P Therapist. At 60 years young they both say my SI Joint is probably fused and won't address it. So where are you located and if you are to far away how do I convince these knuckleheads to take me serious?

    Hi Doc I have been to ( Orthopedic epidural back shot " ouch" did not help, they tried pain meds I said no way, been to different Chiropractors and just what you said temp pain relief and if I move fast ot wrong I am in pain. Now seeing part I osteopathic and helps more but then I still have where if I move wrong back in pain, helps more then the Chiro due to manipulation techniques. Its been 3years sir and still having to deal with this. They say its my right leg longer and my hip is twisted inward therefore making my right leg longer. Any advise or techniques I can do at help to get that back in place for good. And a Chiro here in Tucson AZ??? Help 😯 thanks

      Sounds like you have tried a lot of things looking for pain relief. I'm sorry you are still in pain.
      A couple things you might try:
      Sometimes the problem is that the SI joints are hyper mobile versus being fixated. When someone has a hyper mobile SI joint I am usually going to stabilize the SI joints using a Serola SI belt ((www.serola.net/)). It is my favorite trochanteric belt and it helps stabilize the SI joints when the ligaments aren't doing their job fully. I would work with your medical professional to see if that is indicated and then give it a trial. Patients who need and benefit from an SI belt usually feel tons better immediately after putting it on.
      Also, I find that sports chiropractors, chiropractors that have gone on to do their specialization in sports chiropractic (like an MD doing a residency in sports medicine or orthopedics) is a good place to start when looking for a great chiropractor.       There’s a manual physical therapist named Tim Fearon in Phoenix. He is supposed to be the best. He does grade 5 lumbosacral adjustments. He might be able to help you. Good luck.

    I’ve had pain in my lower right back, in the area of my SI joint, for several years. If I turn to my right, I feel burning pain that is in the right quad, not rear. I’ve been to a couple chiropractors, and 1 could not get any release on the right side, and another got slight release. I have what I assume is referred muscle pain from my hip area, up to mid back on the right side. I’m 63, played sports all my life, and would love to find someone who can treat this without SI joint fusion.

      That is a good question that I get asked a lot. It is common for SI joint pain to be made worse by sitting for long periods of time. Sitting posture is so important and with long hours of poor posture when sitting if can get worse.     Any advice for when the pain is stemming from hyper mobility in the first place? Pain instantly resolved once I can get things back into place. According to my Ortho I have more like 9° of SI flexibility.

      Great question! If there is hyper mobility in the SI joints then manual manipulation is not something I would do with the patient.       I'm in the same boat! I have Ehlers-Danlos Syndrome type 3 (hypermobile type), so all my ligaments are permanently overstretched. They're more like a piece of wiggly rope, rather than a rubber band that snaps back to normal size or length. SI dysfunction is my biggest problem, second worst problem is constant dislocations and subluxations throughout my entire body without doing anything...a lot of times it happens in my sleep. Whose toes and middle of their feet sublux while they are asleep?!

    Hi Dr, I have two questions.. 1)Please advise how would it normally take for the SI joint to heal permanently, if wear the belt, do moderate walking and take the supplement? I've had chronic pain for years and OK for the moment, the chiropractor helped but I can't afford to go any longer(1 went for 18mnths), I still feel the pain after long walk, sitting, lunges, just as you said, or even if I have a break from exercising for a few days. 2) Would it be safe to run on a treadmill at a slow to moderate pace? Or will that exacerbate it?

      I’m sorry you are having chronic pain. I’m glad you found my video and that parts of it resonated with you.
      Unfortunately, I cannot provide individualized treatment recommendations through UA-cam comments, so make sure that you are working directly with your medical professionals to determine the cause and best treatments to help you feel your best. I know that can be frustrating when you have worked with doctors and not gotten the results you desire, but do not give up and possibly seek out other medical professionals to help you.
      So, in general here are the things that I find I need to consider when giving a patient with SI joint instability a prognosis for return to full function and pain-free status. These variables all come in to play. How long they have had the instability? How severe is the instability? That many times is answered by what was the mechanism of injury? What is there ligamentous laxity state in general? Are they someone who has looser ligaments or more stiff ligaments? The age of the patient? And their adherence to treatment recommendations, meaning how much are they wearing the SI belt, are they avoiding things that stress the SI joint, etc. in general, my patience with SI joint instability will start to notice a decrease in their pain almost immediately upon wearing the SI belt and avoiding activities that sheer the SI joint. Based on the questions that I mentioned previously full return to all activities with no pain can take between a few weeks and a few months. But that is very case dependent. Also, there are some patients that do not respond to conservative care measures. Those patients I refer her out four imaging, labs, or a consult for possible prolotherapy injections and/or SI joint fusion procedure.
      Regarding your second question, I can’t give you specific individualized direction, but in general with a patient with SI joint instability I just have them walk (not run) at a slow/moderate pace and I make sure that they are not doing their walking on much of an incline. If their walking is done on a treadmill that is fine, but I would caution them against taking too long of strides which can sometimes irritate a sacroiliac joint that is either fixated or unstable.
      I hope that information helps you better understand and I hope that you will find pain relief soon!

      I'm sorry you are in pain. When a patient has SI joint issues (fixation or instability) it can cause pain when weight is applied through the SI joint like with walking and moving. If the problem is a stuck or fixated SI joint then many times a chiropractic adjustments can provide good relief.     I’m 21 weeks pregnant, was in pain on right side and was told my sacroiliac was misaligned. The chiro adjusted me to get my sacrum aligned. It felt good. Question is, how long/ often will I need to go to make sure I am aligned? Will I keep getting out of alignment while pregnant or can I stop going once aligned? Thank you!

      What a great question! I like to teach my patients that it is less about the actual alignment of the joints and more about the joints moving properly. When a joint needs to get adjusted it is usually because it is "stuck" and not moving properly. That causes the muscles around the joint to tighten up, the joint or associated joints to become inflamed and painful, etc.
      When we think about joints as "in place" and properly "aligned" but not moving properly, then we think about adjustments more to restore proper motion vs. putting something back in place.
      I can't give you specific individualized health care recommendations through UA-cam comments, but I hope the above explanation helps you understand adjustments better. I would continue working with your medical professionals (including your chiropractor) and follow their recommendations.

      Hi Bruce,
      What a fabulous question! I actually sat down and wrote you a response for 20 minutes during my lunch break about 2 1/2 weeks ago and when I hit Reply my browser froze and I lost the answer. I was livid that my response was lost because I feel your question is so core to the health care issues in the US and that is why I spent so much time answering. But alas. I now copy my long responses to the clipboard prior to hitting "Reply" just in case.
      So how to find a provider like you describe... I have set my practice up to 100% treat the person vs. treating them like a list of CPT codes. Let me describe how I have my practice set up and maybe that will help you to find something similar in your area.
      1. My first visit with a patient is 1 hour. During that time we are taking a detailed history, doing a thorough exam (orthopedic, neurologic, functional, etc), delivering treatment, and then going back and checking those positive exam findings to see if we made a difference. If we are not making a difference then why continue to treat in that fashion. It may take a handful of visits to see lasting progress, but not months of care.
      2. On the second visit I present the patient with my care plan recommendations. That starts off with my listing what I heard their goals of care in my office to be. I think it is vital for the doctor to reiterate to the patient that they understand the exact reason the patient came to see them and what they are considering a win.
      3. We use those written out goals as the goal post at the end of the care plan (usually after 4 weeks) where the patient grades how we have done. I show up accountable at the re-exam for the progress of the patient. If the patient under my care is not seeing substantial and lasting improvement with their goals then I am going to help them dig deeper, like getting imaging or labs, recommending consult with another specialist, etc.
      4. I am out of network with all insurance companies. Insurance companies have decreased the payment on codes so low and limited what they pay for that I cannot even cover my overhead with their reimbursement. That is because being a problem solver takes time and great knowledge/skill. With their reimbursement they are incentivizing doctors to spend less and less time with each patient and bill as many codes as possible to make money. Also, they many times require reports and other admin paperwork which takes away from time with the patient and reaching the patients goals.
      Things to beware of:
      1. A provider that wants you to pre-pay for months or a year of care. How can they know how much care you will need when they just met you?
      2. A provider who recommends high numbers of short visits - like 3-5 minutes.
      3. A provider who hooks you up to a whole bunch of unattended therapies like motor nerve stimulation, hot pack, cold pack, massage tables, etc. The medical literature in most cases doesn't support these modalities as curative, mostly they are just palliative and the pain relief doesn't last.
      4. A provider who doesn't do re-exams and check in with you on a regular basis to see if treatment in their office is helping to reach your goals.
      Lastly, I find that sports chiropractors as a whole are a different bunch vs. non-sports chiropractors. It doesn't mean that non-sports chiropractors are no good, just that in general I find that sports chiropractors usually are more interested in getting you back to 100% faster (athletes don't have time to come in 3x/wk for months on end), look for and expect results, treat a wider variety of MSK issues, and give more active vs. passive care (like individualized and targeted rehab programs). Also, sports chiropractors obtain a substantial amount of additional training after chiropractic school, have to pass a rigorous board exam, and recertify every year.       Thank You for taking the time to draft this thoughtful reply. I have definitely run into a lot of warning item #3 . (Hey go pedal this old stationary bike. ) As well as 3x per week for indeterminate periods of time. No time for that.
      A sports chiropractor sounds right. I am really just trying to keep up with the aches pains and tweaks from working out regularly. I will check the list you provided. I do have insurance coverage but more focused on the right approach.

      It depends on if the muscles that support and stabilize the SI joint are weak in your case. If the stabilizing muscles are weak then SI joint exercises like we describe usually get people some good pain relief. But make sure you are working with your doctor to determine the exact cause your pain.

    I have a great Chiropractor and get great results, but I need to see my chiropractor monthly, sometimes more sometimes less, as the alignment that I get undoes itself overtime…
    I can never figure out why the adjustment does not last longer?
    (the reason why I watched your video)
    I’d like hear specific reasons why this is the case?
    Am I doing something wrong with how I sit, sleep, my nutrition, my activities, is it genetics’, etc?

      Thank you for your comment and the questions you pose. I love it so much that I will do another video in the future on "How To Make My Chiropractic Adjustments Last Longer"
      In a short preview to that video here are some things that can cause adjustments not to last:
      1. Postural stresses - bad ergonomics at your workstation, poor seated posture throughout the day, poor sleep posture, etc. Also, the body hates static postures. We were meant to move and in varied ways.
      2. Past injuries and resultant fascial disruption. When we get injured the big structures like bones and muscles heal fairly quickly, but we can have myofascial adhesions that create imbalance that throw our joints off. Those myofascial adhesions can be worked though with things like Graston, IASTM, Rolfing, etc.
      3. Genetics - ligamentous laxity will cause a person to not "hold" their adjustments as long and I advise my patient with looser ligaments to make sure they are working on strengthening their stabilizing muscles vs. flexibility training (loose ligament patients usually want to do yoga type flexibility exercise because they will naturally just be "good" at it).
      4. Trauma / injuries - but this is an obvious one.
      5. Repetitive motion injuries. If we are moving inappropriately (like bending from our lumbar spine into flexion repeated vs. moving from our hips) that will throw stuff out and hasten our need to get into the chiropractor more regularly.

PLEASE PLEASE include remedies and recommendations for things like Item #1 Postural Stresses, etc. Its hard to filter out which products actually work and which don't. I enjoy my Lazy Boy recliner but I suspect it contributes to Postural Stress, but I can't determine what to replace it with. That's just on e example. Again Thank you for your response. It is the best I've seen. =)

    I just saw a chiropractor. She said I was stuck on the left and mobile on the right. I was surprised because my pain is only on the left. However, she demonstrated to me, and it seemed correct. However, I feel worse and more off since the treatment? Why is that?!
    This is my 3rd failed chiropractor. I saw 2 for 2 different sets, and they never worked, but I thought I would give one more try...

      Thanks for your comment. I obviously can't diagnose or provide medical recommendations through UA-cam comments.
      Generally though getting chiropractic care should make you feel better. Sometimes things might be sore for a day or two after an initial chiropractic adjustment, but not lasting pain. Sometimes when someone with a hyper mobile SI joint get adjusted it will make them worse.       What a great question. No it should not hurt. If it does hurt you need to find another chiropractor. Also if you’re afraid of being adjusted manually where you hear a pop there are other options like drop table adjustments or activator adjustments that don’t involve any popping or cracking of joints but still get you the benefits of chiropractic care. I hope you find a great chiropractor and enjoy the awesome benefit!

      Getting adjusted feels very good. If u are worried about anything just skip the neck adjustment and only do back. It can tend to be a little more riskier.

      Hi Dilbir,
      What a great question! An SI joint MRI will show evidence of gross SI joint pathology like ligament avulsion fractions or rupture of sacroiliac joint ligaments. But SI joint ligamentous laxity will not be well visualized on an MRI. Inflammation could be picked up as "increased signal intensity" which means there is more "fluid/water" in the area of the ligaments, but that is an indirect finding.
      The best way to determine SI joint instability is through functional and physical exam tests paired with patients symptoms. You might find additional information that is useful to you in this video: ua-cam.com/video/-lAIOBjgYkE/v-deo.html

    Where are you located?
    I currently see a chiropractor, but am very unhappy with the care I am receiving. I used to see a chiropractor in a different state who was very good. He would manually felt each vertebra before making an adjustment. This is something that my current chiropractor does not do. This person relies on brute force in my opinion. I get results but it does not last.
    However I have seen other chiropractors in my area who have the same type of treatment mode. Please can you tell me how to find someone who does chiropractic right.

      What a great question: "Tell me how to find someone who does chiropractic right?"
      I think that is a very common question and there are many different types of chiropractors with many different styles of practice. I am trained as a sports chiropractor which means I did another year of study after chiropractic college, sat a board exam specific to sports chiropractic, and then take specific continuing education each year (in addition to what is required for my California State chiropractic license) to stay current on diagnosis and treatment. I find that when I am looking for a chiropractor for a patient who has moved out of the area that sports chiropractors are a good (not foolproof) place to look for a good chiropractor.       Also, I think with the right chiropractor you should see results within a few visits that are lasting. If you are seeing someone who says "This pain didn't come on over night and I need to see you 2-3x/week for months" that is the wrong doctor. In my experience the right care given to a patient gets lasting results very quickly (not always fixing everything in a few visits - but noticeable LASTING improvement).

    My lower back pain relief doesn't even last me walking out the door. Went 3 times last week still exactly the same pain in the lower back. They said it was the L4 L5 as they were out. But does nothing after they adjust I've tried 3 different chiros

      I'm sorry you are still in pain and the chiropractic adjustments you have been getting don't seem to be providing lasting relief.
      I find that sports chiropractors, chiropractors that have gone on to do their specialization in sports chiropractic (like an MD doing a residency in sports medicine or orthopedics) is a good place to start when looking for a great chiropractor.       You might also go online here (www.serola.net/) and purchase an SI belt and give it a try. Also, it is very important to make sure if your pain is not resolving to see an MD to make sure there is not another cause of your back pain that is unrelated to your lumbar spine and SI joints. You should be checking in with your MD each year for a general physical exam and blood work. That is always recommended.

    My story is that I have alower back pain and go to doctor who made manipulation of sacroiliac joint , before that I don't feel any pain in my si joint ,it was my sacrum only and lower back , after that manipulation my left sacroiliac start to hurt and it is about six month now , I feel that the doctor cause me that pain , and I feel like that my left sacroiliac is unstable or mobile and the ligament become lax , i do MRI of the joint and it came normal , is there is any exercise that aim to fix this laxity of joint .. exercise that aim to strength muscle without Streatching because stretching cause the joint more move and cause more pain ... Iam far away from Egypt so I can't communicate with u in personal ...thanks in advance ❤️

      @@PositiveMotionChiro thnx Dr ..the video was amazing ..but in my country unfortunately I don't have sacroiliac belt , all I have is lumbosacral belt and iam afraid it will cause muscle weakness but I wear it once or twice a week , and the tablet also not there , if there is another type u recommend or what vitamins I search for in the drug that help healing of ligament, one more thing I hope u describe in avodio how the dancer girl treated and what exactly the steps u follow with her and exercise for hypermobile joint inorder to stablize the joint , I hope I could be near to your clinic 🥺💔but Iam glad I come in touch and hope you help despite of destince

      @@esraasoso6180 Good idea about not wearing the lumbosacral belt all the time due to causing atrophy of your core muscles. Sometimes we will have a person wrap tight an Ace bandage around their hips (same area as the trochateric belt) with sufficient tension to mimic a trochanteric / SI belt. That can be an option if no SI belts are available.
      Regarding supplements, I don't have a work around on that front except that good quality protein and collagen peptides (think bone broth) are important in tissue healing.
      And with the dancer woman I did drop table adjustments to her pelvis, had her use an SI belt, and gave recommendations against shearing of her SI joint (she was doing lunge type stretches thinking that flexibility is always good and so was doing these stretches a lot and that was actually taking her farther away from being pain-free).
      Great questions and I hope my answers will help you in your recovery.

      Honestly waking is the best exercise to help the SI joint to stabilize. Not on crazy mountains, but just on fairly flat ground.

    I am hypermobile and have seen a chiropractor for over 25 years. Whether I strengthen the area or not, my double-jointedness creates the redundancy. My ligaments are shot. Right now the pain has traveled up to the side of my face. It can never be fixed.

      I'm sorry to hear that you continue in pain. You might want to ask your doctor about stabilizing your SI joint using a trochanteric belt / SI belt (the one I like is made by Serola - you can find it online). Sometimes a patient needs stabilization versus mobilization. Many chiropractors are great at getting stuck joints moving through chiropractic adjustments, but sometimes the best thing to do is stabilize a joint. You will need to work with your medical professionals to determine what is the most appropriate for you in your case. I wish you the best.

    Actually where does SIJ pain come from? Isn't SIJ very stable being covered by so many ligaments like you have said?

      The SI joint is usually very stable. It should have just a few degrees of motion as we bring our hips into flexion/extension, like when walking or doing the splits. However, the ligaments can become sprained and have instability and some people are hyper mobile in their ligaments due to genetic or other reasons. Then the sacroiliac joint instability can really become a problem, one that is often overlooked by many medical professionals. This is such a problem in such patient populations that there are even surgeries to fixate the SI joint. SI Bone is an example of this type of surgery: si-bone.com

      Hmmm…. Hard to say what is going on there without the ability to do a physical exam. There are many different patterns the body can take on based on past injuries, compensation patterns, muscle inhibition, motor patterning, etc. I hope you get both sides feeling great soon.

      If the underlying cause of why the SI (sacroiliac) joint is not being addressed then the SI joint fixation will return. If the SI joint, which is held together almost entirely by ligaments, has some of the ligaments overstretched or sprained, then the SI joint can be painful and unstable. In the case of instability a chiropractic adjustment will not stay and/or a chiropractic adjustment done manually might actually increase your pain.
      That is why diagnosis of what is causing the SI joint pain is so vital. Without proper diagnosis the proper treatment cannot be given.

      For sprained and overstretched SI joint ligaments I am usually going to stabilize the SI joints using a Serola SI belt (you can find them online). It is my favorite trochanteric belt and it helps stabilize the SI joints when the ligaments aren't doing their job fully. I would work with your medical professional to see if wearing an SI belt like a Serola trochanetic belt is indicated and then give it a trial. Patients who need and benefit from an SI belt usually feel tons better immediately after putting it on!

    I'm 6 months postpartum and ever since I had my baby I haven't been able to walk they told me I have SI joint Dysfunction and osteitis condensed ilium I was on Physical therapy for 4 months and it was just very very painful and no success. I'm going to a Chiropractor for 3 weeks now and I only from good after he adjustments me but is only for couple of hours then I'm in pain again and if I try to walk is so painful I can't even carry my baby. I live in Sacramento. The doctor offered steroid injections which I'm not sure I want them. But I want to walk again and have no pain. I been so miserable

      SI joint dysfunction comes in different varieties. The most basic delineation is fixated versus hyper mobile SI joints. Chiropractic adjustments are wonderful for fixated SI joints, but are not lasting with hypermobile SI joints. If someone has hypermobile SI joints then I usually recommend that they stabilize the SI joints using a Serola SI belt (you can find them online). It is my favorite trochanteric belt and it helps stabilize the SI joints when the ligaments aren't doing their job fully. I would work with your medical professional to see if that is indicated and then give it a trial. Patients who need and benefit from an SI belt usually feel tons better immediately after putting it on.
      Since I haven't seen you in my office I can't offer advice on if steroid injections are indicated in your case.
