  • There are many important questions around SI joint pain as a cause of low back pain, tailbone and pelvic region pain, and even pain into our hips and legs, like sciatica. In this video we dive deep on UA-cam viewer questions about sacroiliac joint pain that we see frequently.
Here are the SI joint pain questions and answers covered in this video indexed for easy navigation:
0:00 - Introduction to SI joint pain
1:10 - Can an SI joint sprain cause pain for years?
3:01 - Is my pain coming from the sacroiliac joint or a lumbar disc?
6:06 - Are SI joint and groin symptoms related?
7:12 - What is "shearing" of the sacroiliac joint?
9:02 - Leg length difference and SI joint pain? And heel lifts...
14:10 - Would running cause SI joint pain to flare?
15:52 - Can you have sacroiliac joint dysfunction on both sides?
16:56 - Will any SI belt work to help instability?
19:31 - Can pregnancy and natural childbirth cause SI joint instability?
21:09 - I've used the SI belt for 2 weeks, now what?
21:53 - Lying on my back & bringing leg down I hear a loud clunk - is that my SI joint?
A big thanks to all the viewers who have commented on our sacroiliac joint dysfunction and SI joint pain videos. It is because of people engaging and commenting that this video is possible. Here are the SI joint pain videos and SI joint exercise videos where we obtained viewer questions:

There are a lot of questions around the sacroiliac joint (aka SI joint) that can keep people from getting the care that they need. I frequently see patients in my chiropractic practice who have struggled for years with pain in various areas like their low back, sacrum and tailbone region, and hips - with the common underlying cause being SI joint dysfunction.
SI joint dysfunction can present as SI joint instability or SI joint fixation. Many times chiropractors tend to see every problem as a joint fixation and therefore perform chiropractic adjustments, like a side-posture manual chiropractic treatment, to help the problem. But when the pain is being caused by instability in the SI joint then creating stability around that sacroiliac joint is key.
In this video we talk about activities that can increase SI joint pain. We also go into things that many times can bring SI joint pain relief, including SI belts, SI joint pain exercises, and avoiding activities that create stress around the sacroiliac joint itself.
We also go over a simple test that we use in my chiropractic office called the SI belt test that allows us to better determine if the patients pain is coming more from their lumbar spine (low back) or their SI joints.

Since my MRI showed severe athropathy in the sacroilliac joints (bilateral) 3 years ago, I have learnt so much!! Your practical advice has been incredibly helpful. SIJ is just like "Goldilocks", can't sit for too long, can't stand for too long, can't sleep for too long. If you do sit, the seat can't be too hard or too soft. Can't lean to the side, have to stay straight. Everything has to be "just right" lol!!! Wish you were in Brisbane Australia!!

I wish I could give this video atleast a thousand likes. I m 8 mnths postpartum and started having what I initially thought was lower back pain and sciatica since 2 mnths ago. Now when I pinpoint, it actually looks like my SI joint. I don't have problem bending but extreme pain while sitting and standing from lying down postion. Also looks like one of my legs has functional height difference and feet pronation. Severe pain from SI joint to the ankle. Painkillers don't help much. This video helped me understand the pattern of my pain so much. I hope the SI belt helps. Had no pain or problems during pregnancy but postpartum has been a struggle especially since I have another little human to care for more than my own self.

I sit a lot for work and I play a lot of golf. However those 2 worlds collided in the worst way about 2 months ago when I was swinging a club. The pain was so severe it brought me to my knees. For the next several weeks I kept re-injuring myself and was getting very frustrating. About 8 weeks later was I feeling well enough to start doing yoga again, so I went about 50% and it went OK. A week later, I went 100% with yoga and the next morning when I sat down in my chair POW! The pain was back and I was so frustrated and even a bit scared. No one could tell me what was going on or what to do. I'm an engineer so I analyze and research everything and based on your videos and information, I bought a Serola belt (Medium of course) and I've only been wearing it a week and I can tell a big difference. For the last 2 months, if I wasn't in pain, I felt like my SI joint could give out at any moment. I wear the belt 24/7 (except when I shower) and I'm without pain and sleeping so much better. I would wake up with pain and stiffness in my SI joint, but not since I've been wearing the belt to bed. Meanwhile, I'm working on my glutes, glute meds, and TAs. Thanks for sharing your expertise with us, Dr. Pedersen!

In one video you mentioned that yoga and foreword lunges were bad for us ! I unfortunately have been doing them without realising and feel a pain/ heaviness going down the front of my thighs especially the side where my SI joint hurts from a fall Will this go away with the belt too ?
Once again thank you sooo much for giving us all some hope of a good recovery 😊

Thank you Brant. Love your highly informative, accessible and compassionate approach. Just as so many others have commented - you give me hope and for that I am so grateful. I am hopeful that I can recover again from this latest episode of chronic pain. Last time around (2006-2012) none of the orthopaedic Docs I saw gave me any hope, they all had different opinions and I didn't understand my spine/condition so found it hard to choose the correct treatment. I discovered that I have a congenital scoliosis which of course puts undue loads on different parts of my spine/pelvic area. Back then I had disc bulges and prolapses and horrible sciatica all the way down to my foot. I eventually recovered after a couple of injections. I continued with low level pain in what I now think is my R SI joint but this was never mentioned at the time. This has recently flared up badly. Straight on with the belt! And I plan to introduce the 4 stabilising exercises as soon as I can tolerate them 🤞🤞thank you xx

I’m in complete admiration of you, Doctor. One of my hips started hurting during the day, if I slept on it the night before. So now I have to sleep on my back or on my other side (hip).
I didn’t know what SI was. Just stumbled on your presentation, and am so impressed with your knowledge and demeanor. Plus you seem so trustworthy. I have been scared to see a doctor, because I’m afraid they’ll say I need a hip replacement, even if I don’t. I’m 80 years old. The hip aches all day after I slept on it, but not more if I walk or touch my toes, or exercise. NSAIDS don’t help much. The ache is deep inside left hip and is gone in 2 or 3 days if I stop sleeping on it.
The world is lucky to have you in it. I will do more research on your site. 🙏👏🏻👍🤸🏼

Hi ELIZABETH ROBINSON. Thank you for sharing your experience. I can't offer individualized medical advice through UA-cam comments, but if I had a patient complaining of something similar to what you describe (improvement but still having muscle spasms in buttocks and shooting pain down left leg) I would see if you can find a doctor in your area that is trained in Graston Technique or FDM (Fascial Distortion Model) and see if after their history and examination they think one of those treatments might help you get feeling back to 100%.
I find many times that with long standing pain there is usually a myofascial component that is lingering even after the SI joint stability is improved. I find that patients who are going to respond favorably to Graston or FDM usually see results after just one or two sessions, so it is not something that needs to take months to realize benefit.
If you are ever in the Northern California area I'd be happy to help you with those techniques (www.positivemotionhealth.com) but you can probably find someone nearer to you. Here are the websites for each and you can find a directory of providers there. I hope that helps you.

After my pt discharged me from sij dysfunction , I have been training with a personal trainer twice a week for over a year on strengthening muscles to take pressure off the lower back. I am stronger and better but still not healed. Now I discovered the belt, should I pause on heavy workout and solely focus on the exercise you recommend in the other video just for SIJ? I vaguely remember that you said try to avoid doing lunges or other steering exercises.

I fell off my horse onto my hip about twenty years ago and dislocated my hip. The sacroilliac pain lasted for years and had prolotherapy, accupuncture and radio ablation, as well as various injections. Nothing worked but it gradually got better in about ten years. When I had knee replacement four years ago, the physical therapy aggravated it again and am still suffering. I kept telling the therapist what was happening but they paid no attention. Just giving you another story, will just hope it heals with use of the belt and time. I love your videos, they are so encouraging and kind.

He sent me for an SI joint pain injection. For the first time in 10 years, I felt no pain and didn’t take a pain pill for 24 hrs. They numb the area and it wears off in hours. This confirms the pain is coming for the SI joint. I had the SI Joint IFusion method. 3 weeks post op. So far so good but I may need the other side done. Research SI joint fusion. If the SI joint is too lose, the spine becomes unstable. Be sure you go to a good Neuro Surgeon! My is Dr. Steven Dutcher in Palm Beach County, Florida.

I have suffered much of my life with pain in my low back and hip. I was told by a doctor that I needed to get my lower back fused because he seen on the X-ray that part of my vertabre was missing and it might break leaving me in a wheelchair. So I agreed to get the back fusion and the pain never left. Then he told me that he needs to fuse further up my vertabre. I didn't know what was causing me so much pain but I jumped for the surgery in hopes of less pain. That didn't work out and I began noticing that I had trouble looking up and I decided to go to a chiropractor. When I walked in the office I was walking like a 90 year old man bent over frontwards and I had to start using a cane for my balance. The chiropractor looked at my walk and then he took a few X-rays. He called me into the side room of the office and he showed me the X-rays. He said he sees all the hardware put in my back and that's not what is causing me that pain and he said that he could straighten up my upper back but he pointed it out on the X-ray that my Sacrilaliac Joint was out of place and he began putting it back in place each week. It kept popping out of place and last week he said that he wants me to go see the surgeon again and get the Sacrilaliac Joint fused. I was really surprised to hear a chiropractor say that. So today I tried to call the surgeon and he was still out of the office because of the new year . I'm going to stop by the office tomorrow and see if he can get the Sacrilaliac Joint fused very soon

    Hey Doc, VERY GOOD VIDEO!
Thank you for answer my question. It was the second one regarding how
"How do you determine the source of the pain is coming from the SI joint or disc?
Sometimes the SI joint could be irritated due to the disc. How do you differentiate this? "
The last question is GOLD. Thank you for answering.
I have another question. I believe I may have a functional leg length difference.
    But
Have you ever seen any patient that may have a combination of both SI joint and L5-S1 disc issues together?
Please do answer if you see this.

I lifted things at work that I shouldn’t have lifted & by the end of the 12 hour shift, I wasn’t able to stand up straight & the past few days, I haven’t been able to walk well at all. I was prescribed a steroid to help get the swelling down. I’ve got to work & I’m not sure what I can do to help my situation other then just not do any heavy lifting.

Great informative video! Been to numerous ortho docs who never do a hands on exam but rely solely on X-rays and MRI’s. The average office visit is less than 10 minutes! It’s been 6 months and going from an active lifestyle to this paired down one due to pain is so frustrating.
Going to try my 5th ortho doc and hope I have a better experience and some answers. Should I try a chiropractor instead? Just don’t want the issue to be exacerbated. Btw…pain started after my whole back was extremely tight, especially the thoracic area, and I bent down to pick something up. Started on right side with burning on right thigh. Initially diagnosed with bursitis. Now it has progressed to both sides with pain when bending forward/ twisting sometimes. My PT therapist did a test by me lying down and rotating my leg up and to side and felt excruciating pain. Said it may be coming from hip. Just had an MRI of hip. Wish your practice was in my area. 😢

Hi Alexa. I’m sorry to hear that you are in pain and not getting answers on why and what you can do to feel better. I think seeing a good sports chiropractor might be a good next step to see what their take is. Getting an evaluation doesn’t mean you have to have treatment unless you are comfortable with it. When I am trying to find a sports chiropractor when I am on the road or for a patient that moves. I usually look at the directory on this website: www.acbsp.com
      I hope that helps. 🙏🏼

I stared pain in mi left hip after a fall from a bicycle in 2013. Early days of pain I couldn't notice it was coming from hip joint and I thout it was due to excessive cycling and muscle soreness so I tried treating it accordingly. That treatment didn't heal it but the pain didn't increase as well.
In 2014 I got certificate as a gym trainer, so during that course I understood that anatomy and physiology of human body. I sterted strengthening my hip later on. That stopped pain for some years till I stopped my workout due to COVID.
Post COVID I started getting hip pain in left side again so I started working out again but this time pain started to increase so I got X ray done. And in that, imbalance was clearly visible in the left side. Since then I stared to look at it as left SI joint hyper mobility, and started to use SI belt for a support and it gave good confidance to my spine.
Like the test u showed in this video to diagnose Si pain, I tried it on my own by squeezing left hip muscles and bending over. It didn't feel painful at all.
If I target my left side hip strengthening, will it help me recover from my SI condition. And what should I avoid during the strengthening.?

I have one question too. I have had SI joint issues for more than a decade now. I suppose it started with Piriformis syndrome and then later developed sustained pain in SI and SI joint instability. Funny thins i was never diagnosed for any of them. All MRI, xray, specialist appointment, PT appointment just useless. Until i was diagnosed for SI joint issues by Osteopath. His treatments helped me but never lasted for more than few months.
Enough about my history, to my question now. As i have a long history with SI joint instability, the muscles in pelvic region become really tight. some feel tight even after SI instability has been rehabilitated. Does this mean there could be other issues. Is there a way to relieve these muscles? I want to stretch too much as it might flare SI instability again.

I had Si issues from just having babies and weight gain. Did land and aquatic therapy. Helped some. Then got in a MVA head on, I was passenger. My left femur jammed up and broke my pelvis, hip (discovered 3 weeks later l
L4,L5 S1 and fracture sacrum and coccyx) Also my mid navicular and mid tarsals were broken.
That was 11 yes ago. New hip in 2014, foot rejoined with plate and larger screws and bone graft. As for SI joint, well please do not touch a certain area in my back because I will jump and yell. I have had 3 RVA , lasted the longest was 4 months. I get a " cocktail " of injections about 2/3 times a year. When I get them my pain level goes from a constant 7/8 to 2/3. Think a belt would help in-between before it gets so high. I cannot get a fusion because of the placement of my pelvic plate.

I've had my si joint pop in and out for 15 years now, with sometimes tightness, but never this extreme stabbing pain right in the joint. It seems to be severely inflamed this time. It got triggered by walking a lot for a week.
As a teenager I trained to do splits but usually more vigorously on one side. I feel like my pelvis is still out of balance because of that, so that I get pain from just walking. I do ballet now (for more than 10 years) and it's gotten better, since I feel more stable in my legs and hips now. But this attack now is just making me want to cry. I really hope that this might help me get rid of this imbalance and undo my stupid teenage years of ambition. Is that even possible?

I squated fast Saturday and acute pulling in low back I let go of weight lol it was not heavy I was lifting my butt/hips faster than the back. So since then It hurts in the low back or si join when I gate/walk "left leg forward motion" hurts! Also laying on my stomach makes that area settle or separate and it hurts so bad and when I moved out of that position to the floor I could not stand so I crawled to bathroom on all fours and it set back in place I guess and I forced myself up to my feet and that makes it feel better still short steps with left leg full stride with right leg. Siting in car is painful also lifting legs up and extended in a sitting postion hurts agrovates that si low back area like something out of place. I've had an si issue a year or so before said the chiropractor. He said to bend down try to touch toes I did though it was stiff and then he said lean back I did it, it did not hurt he said good its not a disk its probably that si joint he rubbed the crap out of it then swist popped it and it felt better I'm going back if this doent improve.

I love your videos. I have an SI belt it has a plastic buckle on it and I hate it. Also, do you have to wear this thing forever?? I just can’t imagine wearing any type of belt for a year or more! Ugh.
QUESTION IS: when I walk, EVERY step I take makes this clunking noise. I really don’t know where it is coming from though. It doesn’t really hurt but seems to wear me down by the end of the day. I have had spinal surgery (with rods SI L5 L5 L4). I do recently have an MRI,XRys (bending and standing) and a CT scan. My hardware looked ok, and the surgeon (who ordered all this) says it’s possibly my SI joint. It definitely hurts in my butt area and down my leg to my knee. I just want it gone!! What will EVENTUALLY correct. The clunking and pain issue? Please don’t say wearing the belt forever!
I swim once a week, walk a mile (used to do 3 till this issue) and also do some milder exercises in the mid Morning.

Hi Dr! It's not necessary to use the belt for a long time, just when it's inflammation and pain, right? Then ,after recovering we use exercises for gluteus medius etc and for pelvic stability and the problem could be completely gone, right? Thank you!

    • @PositiveMotionChiro
      @PositiveMotionChiro  10 місяців тому

The belt helps the ligaments to heal and get back to their normal stretch and length. It is not necessary to wear it forever. But depending on how long a patient has had the problem and how stretchy their ligaments are in general, the patient may wear the belt for weeks and in more difficult cases a few months. The exercises do help the body to maintain balance and put less stress on the SI ligaments and joints. The goal is always to get the pain to go completely away and for the SI joint to heal completely (no inflammation, normal motion, etc).

I feel that the weight of the backpack making contact on my SI region is the large part of the culprit. Adding some kind of padding seems to help. I've yet to be able to do anything that will stay in place. If the padding is too thick, then that changes the weight distribution of the backpack on me, causing back pain. Any suggestions? Thanks.

My doc says I have arthritis in 4 out of 5 of my SI joints? I haven't heard you bring arthritis up in a video yet.

Can the SI joint cause leg weakness? I'm having growing problems and hurting on both sides into my right knee.

Hi Dr, I believe I have a SI joint issue. It happened about 9yrs ago in a Zumba class. It’s never been 100% since. I’d love to be healed and fully recovered once and for all. However, I’m in Modesto. Los Gatos is not that far. However, do you have a Dr recommendation perhaps for me that is closer?

I have had tailbone pain for over a year. Had a MRI and it showed nothing wrong with my tailbone but showed I have moderate arthritis in my SI joints. Could the arthritis in my SI joints be causing my tailbone pain??

When I go up the stairs I noticed that the pain is reduced? Is there an exercise that is similar to this action?

I have had SI joint problems for 8 years with frequent chiropractic treatments some which help for a limited time and some treatments make it very much worse. My question is why do I feel at times, little pain during the day, but are woken after 3,4 hours sleep with terrible aching pain that no position gives relief. I have tried sleeping on different mattresses with no difference. Would appreciate any help.
