Hey Scarlett, I seriously doubt they are lipomas... my guess is they are areas where the wavy collagen matrix is bunched due to an inability to extend, or because of additional cross-bridging and collagen fiber formation. Great Question! Dr. B2
Great video. I may have missed it, but with the trigger point- are you actually manipulating that with the instrument or are you using a static manual release? Thanks!

I've never gotten graston on my back in child's pose. Usually I'm laying flat. But this was cool to see semi what gets done to me. I get my low back done. Is it normal to get red after it? It doesn't hurt

I do have a hypertrophy on my left side and I've always been able to notice it, but had no idea what muscle that was until today when I looked it up. How can I fix this unevenness in my lower back?
In the lumbar area on both sides i have a pt that is so lumpy/ bumpy. With years of lower back issues could that be scar tissue and could this technique potentially break up the adhesions?
Hey Gary, The bumps may change a little, but it is unlikely you will make radical changes to scar tissue years after it has formed. What may happen is you will improve extensibility of the tissue and aid in remodeling of new tissue. You need to find a couple of assessment you can use to re-assess after you have done the technique to determine if you are making objective changes to motion. Dr. B2
Gary Bruchman I have this issue and it has come with increasing pain. Mine all started with a significant injury which worsened over time. The knots are a herniation of the muscle on both sides. Fat, tissue, and nerves can protrude through the herniation causing significant pain depending on the amount of nerves. Mine were a part of a larger problem. When the doctors finally diagnosed and recently did a diagnostic numbing/steroid injection, I was pain free for the first time in five years. Side note: a spinal block had taken away about 60% of pain weeks prior. Please do not rub or agitate these areas at it can signifiicantly worsen the problem.
what if lumbar flexion causes increased pain in the low back? I had a patient yesterday who had major low back pain and hyperextending the back eased symptoms...
Hey Jaay, I refer to this as a release technique, but the response of fascia to stress is complex. Strain hardening may increase tissue stiffness, where as increased glide between layers may increase extensibility. In either case, stretching and pinching structures can both cause pain. Long story short, assess, try the technique and reassess... if it helps keep doing it, if not stop ;-) Dr. B2
Hey there. I'm a licensed massage therapist. I became a Graston provider recently and it's such an awesome advancement in my bodywork. I incorporate it in most of my massage and bodywork sessions.
Hey Shawn, My suggestion is always to use them as little as possible. Try to use other fingers, pisiform/hamate grip, elbows or fists, etc... it also helps to learn some IASTM and maybe the vibration massage techniques as additional tools to reduce the strain on your hands. Dr. B2

Hey man, to be more effective for people to understand your point as you’re working with patients, I suggest you simplify the vocabulary and less repeating yourself. It’s clear you know your textbooks of this field, but how you can deliver this information in a way multiple types of people can get it is a whole other level. A lot of the things you said in terms of vocabulary of certain body parts can honestly be simplified down. Too much information can make the teaching monotonous. I’ve seen other videos like this with instructors who’ve been doing this a lot longer than you keep the audience’s attention more because of their simplicity in explaining and demonstration. It’s not always about what you know, but sometimes how you can deliver it in a way that’s effective to people on all levels. Do that, and you’ll be golden
I disagree respectfully, Our only intention is to optimize education for professionals. I appreciate what you are saying, but this video is not intended for the general consumer or patient. I think there are individuals who are trying to "play" to both audiences, resulting in less than ideal messaging for both. Dr. B2

Love the information but a bit distracted by your communication with the patient. Especially as a woman, it would make me uncomfortable if a male doctor lifted my shirt and band of sports bra without first gaining explicit consent.
Would have been nice to see a ROM test before and after the treatment to measure the effect.
What if the lumps you are feeling are lipomas? Are you able to tell the difference?

It doesn't cause me any discomfort whatsoever, but it just looks uneven when I bend over. It's visibly larger on the left side.
