The secret to lives is in Butane Pain! There is a secret area in the map that has like 10 lives. You just take 10, die, get 10 gain, die, keep repeating.
It's possible to get that melon without being hit, but very, very difficult: you have to jump to it from the near-side of the steam jet (away from the direction of the galaxian) and as far away from the steam as you can manage while still being able to land on the ledge.
That chest has an extra live for Pac-Man, so don't worry, Atrocity3010 did get 100% on this level :). I even counted the Pac-Dots, Galaxian, and Fruits X).

The crate he missed only contained an Extra Life. I've played through this level and I know what's inside. Extra lives aren't required for 100%.
