In this lecture Professor Zach Murphy will be presenting on Chronic Kidney Disease (CKD). During this lecture we will be starting with a discussion on the most common causes of CKD. We will then transition into the pathophysiology and most common clinical features associated in patients with CKD. We will then conclude this lecture with the diagnosis, treatment, and complications of CKD. We hope you enjoy this lecture and be sure to support us below!

Hi ! I really felt the need to come in here and say I HAVE PASSED MY PATHOPHISIOLOGY EXAM WITH A SOLID 100 !!!! And you have helped me a lot to understand the mechanisms and more than that, helped me understand my teacher's book better. It was an oral exam (basically I had to speak on those subjects and explain them to my teacher) and one of my subjects was about compensation mechanisms of CKD. Can't thank you enough for all the hard work and great videos!!!

I listen to this once a day. Diabetes was too hard to understand. When proteins started leaking 2012, I tried to get my endocrinologist to talk. He sidestepped.
My friend died recently-her kidneys.
She died not knowing why-she blamed diabetic meds.
I'm having a surgery exam in 5 days and I don't know why I'm watching an internal medicine topic 😂.

Love your vids! It’s a good pathophysiology refresher for those MDs who have done USMLE Step1 years ago. This is stuff is taught in the first 2 years of medical school, but it always come back in all the licensing exams. Good job! Oh, and I see the passion you have for teaching!

WOW! I stumbled upon this video because from a nursing standpoint I wanted to understand more about kidney failure. Your lecture with the visual illustrations really helped me understand the pathophysiology of kidney failure, causes, and treatments. Now I understand why renal patients receive certain medications and the s/s associated with kidney failure. This was the BEST educational video I've ever watched. Thank you so much for sharing your knowledge with us. Now, I hope I can find a lecture related to all things cardiovascular! :)

I have ckd and have been trying to get the medical community in canada to help me / support me and slow down the progress. Also have been asking questions. What is ckd? How does it happen? Why does it happen? How do we slow it down? Can't seem to get any answers, or a clear description. This is the first description/ analysis that has made any sense (after 15 years of frustration). I have CKD stage 3A. My lab work, imaging, signs and symptoms show this. Told it's "not a big deal". I'm "not on dialysis yet". I'm not "on the transplant list yet". I "have lots of time". GFR of 45 and creatinine 118. Told I'm not "bad enough" for urology or nephrology" yet. I'd like to show the medical community this. Have a feeling they wouldn't be happy to hear from me.
I got a blood workup in Japan with 49 GFR and 123 creatine. US doctor never said anything about anything. Now I'm all worried. Some more urination at night over the last year when I started taking some ADHD meds... now I'm worried if I have CKD or if I would have to stop taking the meds which are so helpful. Good luck getting help... in the meantime, follow the steps to slow progression I guess...
The nephrologist in Regina Saskatchewan Canada called this week. Wants me to come in for a checkup ASAP. Monday January 30. Blood work. Why call me in for a 6 month follow up after you told me there was nothing wrong and I needed to quit using his appointments time? My nephrologist needs to see this and learn something.

Loving your videos. You are a super teacher. Just wanted to add my 2 cents. Some of us with CKD worked in the nuclear weapons industry, and had consistent exposures to high radiation materials and processes. Don't know how you'd classify this one, but would love to hear you teach about that. Some developed cancers and lost kidneys. Some were just exposed enough to give them problems. THanks.

One note, in 1:06:56, the mentioned antidiabetic agents, metformin and SGLT2 inhibitors should be avoided/used with caution in patients with CKD. With these patients, metformin may result in Lactic acidosis and SGLT2 inhibitors wouldn’t be that effective with the low GFR that these patients have.
Also ACEI and ARB contraindicated in renal impairment..
@@SamiulWasifShovon ACE inhibitors and ARBs inhibit the angiotensin-induced vasoconstriction of the efferent arterioles of the glomerular microcirculation. This inhibition leads to a reduction in both intraglomerular filtration pressure and proteinuria. Several controlled studies have shown that these drugs are effective in slowing the progression of renal failure in patients with advanced stages of both diabetic and nondiabetic CKD---Harrisons Principles of internal medicine
i-SGLT-2 are actually beneficial at lower doses (i.e invonkana 100 mg die) for preventing the progress of CKD even if they don't reduce the HBA1C below 45 ml/min (see CREDENCE study)
But KDIGO guidelines recommend the use of SGLT2i and Metformin in patients with GFR >30 for T2D in CKD. IF these don't work or have any CI (like GFR

Brilliant! You just described what has been going on with me since I was in a car accident 5 years ago. AND this is pretty common, for genes to activate because of a car accident. Trauma, especially traumatic brain injury can have a multitude of gene expressions.
