This ep introduced new story threads while interweaving old story threads from sea 1 absolute masterpiece and it's also great at show don't tell. The sewerage system, the cat, the smell from the north, the sickly person, the new girl, the blonde twins, make up poison, poison herbs/tea/spices & the clothes, Shin & Hongniang, poisoned death at the end and 2 pregnancies, such much to unpack and very excited for the future.
Well, In the last episode, the first time that Shishui appeared was just out of the grenat palace (for a few seconds). She was bowing in front of Jinshi with other maids when he came to visit Loulan. So you might have something there. Let’s not forget that the concubines are political tools for their clan. If the girl can give a child to the emperor, this will be a huge asset. And likewise, if a concubine sent by a rival clan/ political faction gets pregnant, this would be troublesome. I wouldn’t be surprised that some clans try to get rid of rivals, especially since the emperor has neither a male heir asside of his brother nor an emperess.

I'm anime only so I do not know if it true or not, but back in ss1 there's a line that said "new concubine change her outfit every time emperor visit, make him not recognize her", also support the theory
I think the dress they sell this type is to find which concubine is pregnant by the type of the cloths they buy That's why maomao said to buy other type of dress too I think

That said she definitely still was the female role model for maomao for good or ill and that matters more then if it's actually a blood relation or not.
