  • This video describes the three best techniques to rule in piriformis syndrome.
piriformis syndrome is generally suspected when suffering from pain in the buttocks into the leg with no correlating signs of disc herniation or sciatica.
these three tests are useful in ruling in this condition.
first test is called the fair test.
The name of the test implies the positioning of the examined. the Patience is placed in a sideline posture with the involved side up. the extremity is flexed adducted an internally rotated. a reproduction of a symptomatic pain is a positive finding.
next test is called the pace test. This test is performed with the patient in the seated position The patient is instructed to externally rotate the hips or push out the knees against resistance. This test induces a contracture of the piriformis muscle and other hip external rotators. if this reproduces the similar symptoms This is a positive test.
The last test is called the freeburg test. This test is performed with patience and a supine position. The involved leg is internally rotated with the knee and full extension. This test will elicit a stretch response to the piriformis. if this reproduces similar pain in the buttocks and lower distal extremity this is another positive sign.
