I don't know whether my pain is sciatica or this one? Whenever I work pain comes And this year I load and lift some heavy woods and now the pain has come again what should I do.
Paresthesia neuralgia is usually compression of the lateral femoral cutaneous nerve and presents with pain/numbness/tingling over the front part of the thigh. Often made worse with clothes that are compressive around the waist. Exercises may help, but d/t a non-specific nature. Glad the exercise helped!

After I get out of the car, few minits I am not able to move my right leg forvard, cuz of SI joint pain, is this due to piriformis maybe cuz its always painful there?
mine happened during a prolonged bike ride with a really uncomfortable saddle I was testing, kept adjusting it along the way but I was a long way from home by the time it started getting almost unbearable to sit.
I’m still not sure, I have a history of Sciatica, that flares up on a occasions, like years apart, but now getting to a standing position is difficult, it gets better as I walk and it appears to stay over my left buttocks and lower back.
Just the Left, it’s an acute reaction to something. I am not like this all the time. My legs aren’t affected. But the very first time I had sciatica I could barely walk without excruciating pain. Now it takes much effort to stand and getting up from a lying position. Walking is okay with a little discomfort.
So you recommend not side sleeping? I assume back sleeping is best then? Never thought about it but I always lay on my side in bed, I bet thats a huge factor for me also.
Yes, sleeping on your back with a pillow under your knees is often best. If you do sleep on your side, make sure it is the none painful side and place a pillow between your legs
I wish he would have elaborated on the sleep position, I do not understand. I have performis issue, at times worse than others, my daughter is a pilates expert, tried to help me, I have not disciplined myself, but massage helps some, I find sitting, driving, in car alot, what is best sleeping position.. my right side is the issue.
For the positions, find a position that decreases compression to the right side. Driving is tough, but for sitting a donut or soft cushion can help. For sleeping, sleep on your left side, with a pillow between your legs or potentially on your back with a pillow under your knees. This can be a very frustrating and persistent condition.
Hip snapping is typically caused by the gluteal tendon (laterally) or psoas tendon (anteriorly), so possible could contribute to the lateral hip snapping. Unlikely to cause pelvis floor issues

It's now two weeks since I started practicing these exercises some improvement but when am doing some exercises I feel pain now what should I do with those exercises that I do and feel pain otherwise thank you and hope to get better
Keep it up. Focus on the exercises that help reduce the pain, and stop the ones that are aggravating it. It will take time, and there will be good and bad days. The goal is that there is an overall positive trend.
The gluteal/cluneal nerves can get irritated (however the diagnostic criteria for this is poor) and potentially irritated with EXCESSIVE back extensions. As for anterior pelvic tilt, thus is “normal” as is prevalent in the population with no pain (symptoms). And due to bone shape not “tight” or “weak” muscles.
It can, it also increases the likelihood that is could be coming from your lumbar spine (i.e. sciatica) This video would be helpful: ua-cam.com/video/YjDBMjoaAcE/v-deo.html

Thank you for the video and the info. I've suffered bilateral buttocks pain around sitting bone area, and left leg numbness on sitting posture for 2-3 years. MRI and X ray reports are showing hamstring tendinitis and L2, L3 & L5 buldging discs mildly contacting nerve roots. I'm good on standing, walking and lying posture. I went through all of treatments, the left leg numbness is better but buttocks pain is still going on whilst sitting. Do you think it's piriformis issue or something else please? Thank you very much!
Thank you, sorry to hear you have been dealing with this for so long. Those are definitely classic symptoms of piriformis syndrome. Have you tried the exercises and nerve mobilizations (or something similar) yet? It could also be proximal hamstring tendinopathy, have you done any rehab for that?
@@PerformanceSportSpine thank you for your return, I feel so grateful to hear from you. Also, buttocks pain comes up shortly after sitting, I can't say it's over use of glutea muscle, etc. Then... Where does this pain come from? @@PerformanceSportSpine Thanks for the quick reply. The nerve mobilizations have been part of my daily stretching routine for 2 years now. The self release techniques are also making the pain much worse, together with sitting for longer periods of time. I’m still not sure, I have a history of Sciatica, that flares up on a occasions, like years apart, but now getting to a standing position is difficult, it gets better as I walk and it appears to stay over my left buttocks and lower back.
Sciatica is definitely far more common. Does your pain just stay in the buttocks, or does it radiate down the leg? Any imaging of your low back?

I do have a disc problem in the lumbar but I do not want to get the major fusion surgery only to find out later it was just periformis syndrome

Can it post cause snapping hip or pelvic floor muscles issues?
Hip snapping is typically caused by the gluteal tendon (laterally) or psoas tendon (anteriorly), so possible could contribute to the lateral hip snapping. Unlikely to cause pelvis floor issues

I've been having pain in both sides of hips and buttocks. Feels like a muscular pain. MRI shows I have spinal stenosis ins L3/4. The neurosurgeons recommend surgery, even neurologist I see. But it feels like something muscular. I walk with a walker and I use all my might to endure the intense pain. Is there a test to rule out muscular. I had surgery two years ago in L/4/5 and pain was and still is gone. I do have periphial neuropathy. in my left calf and top of foot. What are your thoughts on glute nerve pain that is triggered by back extensions or the even just by an anterior tilt of the hips?
The gluteal/cluneal nerves can get irritated (however the diagnostic criteria for this is poor) and potentially irritated with EXCESSIVE back extensions. As for anterior pelvic tilt, thus is “normal” as is prevalent in the population with no pain (symptoms). And due to bone shape not “tight” or “weak” muscles.

Sorry to hear that, some cases can definitely be aggravated by stretching. Have you tried the nerve mobilizations (neurodynmaics), making sure the glutes aren't over contacting and limiting any self release techniques (using a tennis ball)? Thanks for the comment!
@@PerformanceSportSpine Thanks for the quick reply. The nerve mobilizations have been part of my daily stretching routine for 2 years now. Together with the glute bridges, side laying leg raises and clamshells I have been able to get the pain to a tolerable level. But I just seem to can‘t get rid of the pain completely. The self release techniques are also making the pain much worse, together with sitting for longer periods of time.
