    I'm in Manitoba. I'm on a 3yr waiting list just to see a surgeon. It's been 1yr now. So painful in both hands. Not able to work anymore and have little quality of life. My heart goes out to anyone struggling.

      I'm so sorry. I think I ruined my left hand due to my bookselling business. It's the way I would hold the books with that hand and now I might have to give that up and am not sure what I will do. I didn't expect hand pain to be the intense. I hope you get this all figured out and find ways to feel better.

    Thank you! I'm a guitar player who's been unable to play due to this basal arthritis. To hear you confirm the ability to recover enough to play guitar again has given me immense relief! 😊

      Even though it's a year later, consider very carefully this surgery. I am also a guitar player and had this surgery in 2010. I regret it. It took three years until my left hand became somewhat normal and still, 11 years later, the muscles in my left hand (that had the surgery) feel significantly tighter than those in my right. Additionally, my thumb joint cannot bend backward to do proper barre chords now. Also, I found that eventually the pain in the joint somewhat "burns out" and nowhere as painful. Good luck.

    I got CMC tendon transfer surgery done last year right before Christmas. The surgeon downplayed the recovery time (or maybe I misunderstood him) however, it's been 6 months since the surgery and finally I see a light at the end of the tunnel. I still experience pain from time to time when I overdo it but it's coming along. For those of you who are considering it get done, you should feel better between 6-12 months after surgery. Be patient with it, it takes time to heal but it's worth it.

      @@TR5T I did some exercises after the surgery, but it was not done on a regular basis. It's much better now about 8 months later.

      That's good to know. My left hand hurts so bad, I can hardly stand it. Injection didn't do much and. don't want another one due to now fat atrophy in my hand. I hope you are doing even better now.

    I am a craftsman (English medieval carpentry) and a musician. The basal joint arthritis I suffer with is so debilitating and takes away my purpose in life..

    Thank you for this video. I’ve had basal arthritis for about 10 years. Lately, I cannot grip without experiencing sharp burning pain up my arm. I got the cortisone shot and only helped for a day. Started taking aleve and it wears off in 5 hours. I use CBD so I can sleep; by morning I’m in pain again. Glad to know I have options. I sew handbags for my daughter, love to make bread/bagels and cook. I am not just a gardener but a grounds keeper for my yard. I would love to do these things with less pain.

      Noooo! That would leave you with no hands to take care of your personal needs (wiping your butt, hair combing, eating etc) for at least 6 weeks. Getting one hand done and getting through the therapy will take time. Do one hand until it is pretty usable (3 months) before you do the other.

    Thank you so much. Know I know what's been hurting my hand . I will be seeing my doctor asap. Anti inflammatory and cortisone hopefully will help me recover. Yes I notice it goes away and comes back too. Stretch of the neck help but pain and swelling of the basal joint is bad.

    I chose the tendon, 6 days on only waves of pain not like day 1-2. I did my research and this was a tried and true method. Waiting on the main bandage to come off and the cast to go on. Do stretches lightly from day two, it helps.

    Thank you for explaining the basal arthritis, I did the x-ray last month and it shows that I have it in both hands, sever pain at times and little pain all the time it's been over a year now, I have an appointment with my family doctor next month to discuss the issue, I think an operation in both hands is needed !! is it best to do both hands at the same time or one at a time if needed? Mind you I never try any treatment yet but I'll be getting the brace soon hopefully it works!! thank you

    I had this done to my right hand, I was to the point of dropping object suddenly from the pain.
    I went straight for the operation instead of the shots.
    Best thing I could have done. I ride offroad motorcycles (enduro) and was worried about doing that and work, It’s true you gain back at least 80% of your ability to grip which was at least 40% more than I had before the operation.
    As soon as I feel comfortable with the medical community again I will have my other hand done.

    Why can’t you remove the trapezium, clean it up maybe whittle it down to a smaller size and reinstall it?

    The simple removal of the trapezium bone seems to be the least invasive, but what about thumb stability, I am a piano player and would never want anyone to touch my tendons in any way. I would go simple removal of the small bone (trapezium) and see how that goes, I guess it could be revised later if it fails. I will need this surgery soon so I will request the simplest and least invasive procedure first. I have read that the recovery from the LRTI tendon transfer procedure is horrific and long, there must be another way! Thanks for your informative video Doctor :-)

      Thanks for the reply, that's great, what was the surgery post pain and recovery like? Are you now pain free in both thumbs? At the moment I am having lots of luck using splints and hand therapy, but can't wear splints forever lol. What time frame was there between procedures on both thumbs? Some people do one thumb then wait a few months until that has healed then do the other thumb. Did you have regional arm nerve block or general anesthetic?

    For me, this is not a lifestyle issue, it’s a working issue. In the spring and summer, I operate equipment that uses a joystick to control most aspects, and the right thumb does most of it. In the off season, winter, I do mechanical work, and simple things like threading a nut onto a bolt is very painful. My surgery is scheduled for a few weeks from my writing of this comment.

      @@RacingEuharleeI had my surgery 27 weeks ago. I was out of work for 8 weeks on short term disability, not by my choice, but because the surgeon didn’t want anything to happen to my thumb and derail the recovery.
      Now I’m very functional with nearly no pain. But strength and full range of motion are still lagging. Might be as much as a year before full recovery.

      Are you for sure getting the surgery, or is this the consultation to see if this is the best option for you?
      The first part of the recovery was harder than I expected. Couldn’t operate buttons, zippers, or shoe laces for weeks. My wife got a pair of dress sweat pants (who knew?) because we had a couple of functions to attend.
      For where I am now, I’m glad I did it. I’m 62 now, and another possible 30 years of dealing with the pain doesn’t seem to be a good time. Let me know how it goes!

    Really liked your video--and usually I am not a big fan of "talking head theatre"! I was also impressed by the emphasis on conservative watchful waiting and on patient choice. Informative! Thank you. (In the interests of full disclosure, my daughter is a Dartmouth alum and worked at DHMC during her undergrad years, so I already respected the work you do there. Still--it was the best video I've seen on this subject, and I've seen a dozen or so. (Go, Big Green! Go home, Keggy!)

    Surgery on my right hand was done June 11 2014. This time I was ready with extra help at home. Pain meds from a pain specialist. This surgery was nothing like my last. As long as I took my pain meds for the first three days I did fine. After three days I cut myself back on the pain meds and I was going 10 to 12 hours between. I have now completed therapy, which was somewhat painful but nothing like I expected. Close to 3 months now and I will get some pain if I use the hand too much. My strength is coming back and I am so glad I have all this behind me. The pain I get now isn't even 25% of what it was before I had the surgery. I hope this helps those of you that are on the fence of getting this done.

    I think that I have definitely developed Basal Joint Arthritis. I broke my scaphoid 17 years ago playing hockey in college, and over the past few years I've had serious issues with pain in my thumb, right where it meets my wrist.
    It's funny she said "common complaints are things like opening jars" whenever I speak about this problem I always reference how I can't open a Gatorade bottle. Anyway, I also have major pain surrounding my previous scafoid fracture, I have a huge cyst that's painful and have lost a massive amount of mobility with my wrist and hand. I think I'm a candidate for surgery due to the years of pain I've had as well as the cyst...

    After seeing my local orthopedic doctor for the basal joint arthritis I was totally discussed with his approach to my problem. My right hand is the worse. I had the injection but only had relief for a few days. His solution was to grind off the two bones and apply a rod to the thumb and allow them to fuse together. Then he wanted to remove the rod or splint. (another surgery) I did not like his solution. I have been researching other methods, including hand exercises, etc. I wear a compression gloves at night and try my best to avoid NSAID's. When I do take them they do not work, I usually take 800mg IB when it gets bad. Nothing has really worked.
    Where I live, there are no hand specialists, most doctors here are orthopedics and treat the entire body. Each day my thumb gets worse.
    I am retired and downtime isn't a problem.

    rheumatoid arthritis in my wrist's one day my wrist got so swollen i made a fist my thumb made a pop shound had a sharp pain for few mins after i lost the abilaty to move it i lost use of it what should i do happen a year ago now i all ways have a pain in that wrist

    Need help, I can't figure out if this is my issue or not but I'm an artist and about 4 months ago I got into the habit of drawing everyday and have been since November, now my wrist hurts every time I bend it up and down and sometimes it hurts to even bend my thumb up and down, on top of that i notice that if try to open my hand wide my left thumb doesn't naturally bend back as far as my right thumb anymore and it hurts every time I try to, i wear a splint everyday and when i go to sleep and since i started wearing it, its not as bad but the pain still comes and goes, it's not to the point where I can't draw, I'm just looking for advice to figure out what it is in case it is because of drawing or something else so I can treat it before it gets worst, not really in a place to afford to go see a doctor so i would really appreciate it if anybody knew what this is and maybe had a remedy

    Thanks for this. My Dr. keeps saying this is a carpal tunnel problem, when there is simply no pain in the carpal area; this nails it. There has been some issue in this joint since this joint was damaged in a fall while in the army, 45 years ago; I guess this is another issue for VA. It has worsened in the past year, and causes me a lot of problems now that I am retired and have time for a lot of hand woodworking, fly tying, etc.

    Enjoyed the video. I was just dx and tx with a steroidal injection today for Lt 1st CMC OA. The sxs appeared only 9 days ago along w/another flair up of my R thumb DQ tendonitis so I immediately went to my PCP and got a referral to the hand doc for the shots. NSAIDs don't help much and my work gets annoyed when take medical leave. Still I won't consider surgery yet since it only started in Nov 2010. Yikes! I just turned 41.

    I don't know if this is what I have, but my right hand hurts at the base of my thumb and down into my wrist. I feel it every now and then when it seems to flare up. The problem is, I'm in my twenties. Isn't that a little young for a problem like this?

      No, arthritis can hit at any age. Your primary doctor will probably have the radiologist do several x-rays, at different positions. Women get this thumb problem....way more often than men.

    Great video! I am scheduled to have this surgery (with tendon graft) in three weeks. My surgeon has more or less downplayed the recovery, but I am reading and hearing a lot to the contrary. I'm 59 and live alone. I also have to travel 125 miles for the surgery. Will I be able to drive and function alone after surgery? Can I hold my new granddaughter or keyboard? I'm getting nervous!

      NO, YOU CANNOT BE ALONE OR DRIVE AFTER THE SURGERY THE MEDICAL PERSONNEL WILL NOT ALLOW YOU TO DO THAT. GO WITH SOMEONE.

    I've tried all non-invasive treatments - didn't work. Finally I had the surgery and it was a nightmare. No so much for my thumb, but I developed several issues in my shoulder from the "nerve block" given to me right before the surgery. I now have Nerve Damage from that as well as Frozen Shoulder and I developed Bursitis in my shoulder on top of all that. This has really impeded my recovery for my thumb. I'm 2 1/2 months after surgery, in terrible pain, and I Vlog about it all on my channel. Heat and cold packs are my best friends right now...thanks for your video.

    I’ve had this surgery about 3 years ago. My hand surgeon warned me that it would be a very painful recovery for the first 6 months. However, it was a very easy, painless recovery - a most welcome and unexpected surprise! I started off getting injections in my thumb joint...ouch! After they stopped working is when I consented to having surgery and I was happy I did.
    Now, my other thumb joint has the same condition. I’m taking the same route as I did the first thumb and I’m planning having the same surgery with the same surgeon in the fall of 2018.
    My advice is to talk with your hand surgeon about the expected level of pain for the first few months. My doctor told me I would be in terrible pain for 6 months and then it would be much better. Also, expect a 15-20% reduction of strength in the hand you have surgery in.
    All in all, I had a good experience with hand surgery.

    Follow up: I had the surgery done three weeks ago. I actually had minor pain for the first three days and was off meds totally after that. The worst part is taking showers and trying to keep the cast dry. It's amazing what you can learn to do with your other fingers (such as tying your shoes) when you can't use your thumb. I am able to cook meals, drive, shop, and do laundry with not much problem.

      Shannon Warren Yes, it has now been almost three months since the surgery. Remarkably, I have significantly improved function and strength in that hand. I have only a little pain (from overuse) now and then. The only issue is the scar (a burning sensation), which I am dealing with through over the counter items. I might add, that nothing was added in place of the removed joint. Soft tissue was allowed to grow back. It looks like a normal hand, except for a tiny scar.

    My doctor is going to do this surgery soon on me. I have been trying to avoid it for several years. I've tried injections, splints, rest, meds, etc. The pain is so intense now, that I have given up and am going with the surgery. I drop things and have great difficulty opening jars, etc. I am looking forward to getting it over with, as I will eventually have to have the same surgery on my other hand, which is not as bad right now.

    I had my trapezium removed and a mini tightrope installed in my hand. I am experiencing worse pain with less function. The cortisone makes it so much worse. I fear I am never getting my hand back. The pain just won't stop. I wake up throughout the night with an ache that feels like I have a screwdriver piercing my wrist. I should never have gotten the surgery.

      Cortisone seriously impedes healing. I can't understand why doctors use it for OA therapy when they know the patient is going to need surgery.

    I have the same concerns (living alone) - and it's my understanding that one only gets 50% strength - that's not good enough - I need to be able to do my own yard, scoop my own walk, etc. Why do they wait until the situation is hopeless? Why won't the materials they use on hip and knee replacement work for resurfacing?

    Wow, so you are saying that to just remove the bone without going through the tendon graph is just as effective? I play the piano and I am dreading surgery on my left hand. I just had rotator cuff surgery on my right shoulder and it's absolute hell, so not looking forward to more surgery, but I'm afraid there is not much choice.

    i had this surgery on the 25th of july. now remember...everyone is different, but i will tell the truth in my case. i am in a cast, my thumb completely immobile. the first week was very painful and couldn't get comfortable at all. my cast was very tight from the swelling. i got my stitches out 8 days later. since then..i have been pain-free. cast off aug 25th, then a removable splint for 4 weeks, then a thumb splint after that .the cast is bulky and my thumb gets a bit achy being immobile.

      REST DO NOT USE YOUR HAND TOO MUCH GO SLOWLY AND THE RECOVERY TIME WLL BE GREAT.

    hi karen. i haven't had a problem driving at all. it will be hard for you to open jars, take showers, dress, do your hair and makeup ,which is my #1 problem. (by the way..I am Mike's girlfriend) Is the surgery going to be done on your writing hand? mine was..so doing everything left handed has been a challenge. your cast and bandage will be very bulky and awkward. you will realize just how much you use your thumb when u can't use it. be prepared for a difficult few weeks.
