    PCOS & Skin | Dermatologist Review
    PCOS is a multisystem disorder seen in women. Up to 20% of the population will suffer from PCOS. PCOS is essentially a metabolic syndrome, however, the first presentations are often in the skin.
    2. How do we diagnose PCOS?
    Essentially, two out of three characteristic findings would a patient with PCOS.
    1. Irregular menses.
    2. Ultrasound scan which shows multiple cysts in the ovaries.
    3. Signs or symptoms of elevated androgen levels
    3. What are the skin signs of PCOS?
    Patients may present with some if not all of the following features. Acne which is usually located around the jawline area. Unlike normal acne, PCOS acne is often hormonally based with deep-seated lesions and the relative paucity of blackheads. Characteristically, acne flares during the period cycle and patients present with painful cysts. Scarring from this type of acne is not infrequent due to the fact that patients often pick at the lesions.
    Another manifestation of PCOS is oily skin also known as seborrhoea. This may be isolated or can occur with acne.
    Hirsutism or excess hair is frequent seen in PCOS. Treatment is with laser hair removal.
    Unlike normal hair growth, patients with PCOS will often need 1-2 'Maintenance treatments
    every 12-18 months.
    Hair loss is another complaint with the patients who have PCOS. Hair loss is also known as androgenetic alopecia and infects the scalp. Conversely, there may be excessive hair in other areas such as the face and chest/back. This is known as hirsutism.
    Other skin changes include acne inversa also known as hidradenitis suppurativa. This can present as lumps and bumps together with painful sinuses located under the arms, in the groin area and often under the breast area. Medical management can alleviate symptoms of HS.
    Darkening of the skin can occur in PCOS patients. This condition is called acanthosis nigricans. Additionally, skin tags may form around the armpits, under the breast area or in the groin area. An endocrinologist or a medical dermatologist can assist with treatment of AN. Metformin and low strength topical retinoid can help.
    PCOS should be managed as a multi-team approach. The best speciality to address all these features would be an endocrinologist with input from a medical dermatologist and OBG. There are many ways to treat this condition which goes beyond the scope of this presentation. Dermatologists can also manage both hirsutism, hair loss and also acne. As this is a metabolic disease, close monitoring of patient's vitals including blood pressure, cholesterol, weight and other factors is paramount.

    PCOS can be a nightmare. The skin problems are though, but it can also cause enlarged ovaries, excess androgen, hirsutism, baldness, irregular periods, heavy periods, and you can become infertile. Therefore, if you experience any of these symptoms always contact a gynecologist.

      I do. But i think it is the pill. My ginecologa prescribed me pills for 6 months. It is extremely overwhelming. There are days that I feel sad with the tiniest things

      YES! However, I just got diagnosed and once I started taking the pill I kinda felt back to normal. Im sleeping better and my depression episodes have lessened.

    I love and appreciate how well explained all of this is. I understand that physicians may not have all the answers at the top of their head regarding what PCOS is and how if effects the body and skin, so I'm hoping more physicians can use videos like this as a resource. It's very overwhelming to just be told "make lifestyle changes, take metformin, exercise more, and/or take birth control" when a lot of the times, that's a vague solution. knowing how and why conditions affect the body can be a lot more liberating.

    Thank you so much. I am older. When my symptoms returned in my 20's I had an OBGYN who told me to go home and put my feet up, and everything would be all right when I had a baby! It's so nice to hear how much medical science has progressed and spreading the information can get women help.

    Thank you so much for the information you’ve given. No other doctor I’ve spoken with about my PCOS has given me the why’s and how’s about the treatments offered and why it’s so important to work with multiple doctors in multiple fields in regards to treating PCOS. Hopefully I’ll be able to get the doctors I need to really get my symptoms under control 20 years after my diagnosis!

    I was diagnosed infertile at 19, told was never gonna have kids never had a regular period, pretty much checked off all the symptoms of PCOS BUT last 2 months we celebrated my daughters first birthday conceived naturally with herbs remedies

    Having PCOS ups our risk of diabetes, cancers, heart disease and 30x the risk of sleep apnea. (Yes, you read that right.) Not all women will have polycystic ovaries. One only needs 2 of the 3 Rotterdam criteria for diagnosis. It is important to monitor the blood insulin levels, as well. Metformin is very important in controlling this condition, as well. (I was diagnosed finally by a functional medicine MD at age 48.)

      I had loads of side effects from the metformin. Got to a stage where I couldn't even eat green veg, pasta, rice, most sauces, eggs etc as they were making me so poorly. It was horrible. It also made me sweat loads, my hair loss increased as did my facial hair. I changed to the slow release version but there was no difference. I'm now trying Inositol to try and improve my symptoms, been taking 3.5g (powder form) for 2 months now and it's definitely helped me to feel a little better

    Thank you so much for this information. I’ve been diagnosed with PCOS for several years and have been battling the symptoms for many more. I’ve tried every skin care treatment/regime, every hair removal treatment (including at home laser/light therapy), all diet and lifestyle advice, and I’m at my wits end with it all. I have PCOS on top of two debilitating chronic illnesses (Psoriatic Spondyloarthritis and Ehlers-Danlos Syndrome), both of which have damaging effects on my skin. So to try and cope with these as well as PCOS is not only humiliating, time consuming and expensive, but can affect me mentally and socially as well. Add to the fact I’m now in my 30s and trying to get the upper hand in the aging process, I am beginning to think that there is no way to treat all these dermatological issues.
    In the UK, my GP keeps telling me that tens of thousands of women have PCOS and basically to just keep my (hairy!) chin up and get on with it, but to know that there are ways of controlling the hormonal imbalance has given me fresh hope that I’ll be able to try and tackle it in a different way, at the root cause. Thank you with arming me with this information, it’s invaluable to me. Knowledge is certainly power in this case!!

      my doc said 1/3 of women have pcos, so it's a common thing now. Have you tried IF (intermittent fasting or OMAD (one meal a day) diet/lifestyle? also definitely try cutting out visible carbs and sugar if you can or havent tried. It worked for my skin and got my period back. But I'll have to keep going with this diet to see if my period comes next month.

      I know this comment is 2yrs old but I also have EDS and PCOS! I kept getting dismissed and told to live with it until recently. There’s basically two different medication routes, the first treats symptoms like hair loss, facial hair, etc by suppressing the excess male androgens, the most common drug for this is Aldactone, but it’s VERY important not to fall pregnant on this drug as it’ll cause deformities, such as male babies not developing any genitals.
      When you ARE trying to get pregnant, you stop these meds and there’s alternate ones to take to improve pregnancy chances. I believe it’s only after breastfeeding has concluded that you can then switch back to the previous medications to suppress male hormones and improve your regular PCOS symptoms once more.
      So treatment is basically one thing for trying to get pregnant and then treatment of symptoms the rest of the time. I honestly have no idea why so many doctors tell PCOS patients to just “live with it” because there’s absolutely treatments available and they’re completely safe. That said, if you’re taking the contraception pill, as many do since it can help symptoms, double check with your doctors about how high or low the oestrogen in it is, as high oestrogen is linked to breast cancer. Studies indicate the low dose is fine and may even help prevent ovarian cancer, but that’s a whole topic on its own, just figured I’d mention in case it’s relevant.
      For anyone currently looking for help with PCOS, Google nearby doctor options to find one with a website that explicitly states PCOS as their special interest/their area of expertise. You should also be able to pick up a vibe as to whether they’re pro or anti medication, as you want to avoid the latter.
      You 100% need an endocrinologist and gynaecologist involved to treat PCOS, many people will also more than likely need a dietitian to help with insulin resistance/weight gain, of which also has medication (metaformin) to help improve. Don’t ever put up with apathetic doctors who lie and say there’s nothing they can do to help you, because it’s just not true!

    I wish I would have seen this video when I was 16. I'm 21 now and until last year I couldn't get proper care from any doctor. Finally, in 2019 in Russia I found an endocrinologist and a gynecologist. They helped me a lot. I hope everyone who has PCOS would see this video! Ps, check your thyroid, everyone. Thank you doctor!

    Thank you. I have PCOS and when diagnosed I just got given the pill. I didn't like the other side effects, and now I just live with the deep jaw acne. It's really not fun and can be a huge downer. I wish my GP would offer more help

      Try low carb, high fat diet.
      Pure spearmint tea, 2 cups daily (not peppermint) to reduce androgens (diminishes hirutism as supported by numerous research)
      Supplements to look into:
      Inositol (2000mg X2 daily, as supported by research)
      Folic acid (200mg daily)
      D3 vitamin
      Metformin - often issued by doctor. However can have gastrointestinal sideffects.

      Girl, LOW CARBS!!! or no carbs if you can do it. I try to not eat any visible carbs now, my only source of carbs come from veggies. and I don't eat sugar often or dairy and my skin cleared up and my period came back in just 5 days of this diet!! I haven't had my period in nearly 5months. and try to do IF(intermittent fasting). I did a more extreme one where it's one meal a day, and it's helped me lose weight, clear up my skin and got my period back. but i can't say for sure my pcos is cured. since I rmb my doc said that for pcos ppl having a period come once every 3months is a norm. But I don't even have that going for me.

      I've been recently diagnosed with PCOS this year and I have been on a low carb diet since. I got my period back and my skin is much more cleaner than it was before. Highly recommend cutting back bit by bit so the fatigue doesn't hurt too bad.

    I have learned that gynecologist have no idea how to treat this aside from handing out birth control like candy. PCOS is an endocrine issue so I will be asking my gyne for a referral to an endocrinologist.

      Unfortunately the real question is who is the reason of having PCOS???
      Only answer is the Insulin resistance, when you make your insulin gets higher in your body it will become diagnosed PCOS....so two things get rid of your higher insulin are
      1.low Carb
      2.move a lot(workout)

      I can’t believe there’s nothing to help, metfiemin, bcpills, etc doesn’t help at all. I feel like they don’t care to research they prefer to help people wanting to change their gender than to help a girl cure or effectively treat pcos

      They are going to tell you the same thing- birth control, metformin or spironolactone. Been there done that. I've fired 2 endocrinologists. There isn't enough research about the condition.

    Thank you for this video!!! I haven’t been diagnosed since it takes over a year to get an appointment with an obgyn, but I have my period 3-4 times per year (3 this year!!), which raised flags. On top of that, I have really good skin, no texture or anything all over my face... except on my chin and jawline where I get really painful cysts every few days, even while using Stiva A. I’m not trying to diagnose myself but it seems like many signs are pointing towards me having PCOS so while I’m waiting for an appointment, I’ll be trying to manage the symptoms with a low carbs diet. I hope to finally see a difference because this cystic acne is SO painful and makes me feel really ugly :( thank you Dr Lim for breaking down the symptoms!

    Ty for this ! Very informative. Many women are not being treated . Many Drs pass it off as premenopausal or perimenopausal etc. I have been through hell with treating this. Vitex is helpful along with a good skin care system and diet. The meds cleared skin immediately but have way too many side effects that make it hard to function well everyday.

    Thank you for making this video. I am 36 and I have been suffering from severe acne for two years. My ob said I have PCOS but there is nothing he can do about my hormones. My skin is extremely scarred. Now I know I might have other options to help.

    Thank you for the information, I take spironolactone 100 mg every day and I use adapalene gel 0.1%, benzoyl peroxide 10% and vitamin A 0.05% all of this are helping to improve my acne and scars 🥰🥰🥰🥰

    Wow! I was diagnosed with PCOS in my twenties (via exploratory surgery) when it was referred to as a disease. Doctors didn’t know it was pancreas-driven at that time. It caused my infertility. Thankfully, I have a wonderful daughter through the miracle of adoption.🙌🏻
    I’m now post-menopausal, almost age 60, and on Metformin. My two dermatologists didn’t discuss skin/hair issues related to this syndrome with me - with the exception of yeast/heat rashes for which I use Ketoconazole Cream and shampoo. I once had flawless porcelain skin, but I developed acne rosacea in my mid-50’s. I surely recall excessive oiliness, and facial hair when I was much younger. I pray hair loss isn’t in my future. This is surely a curse in many ways. It’s difficult to control my weight, unfortunately. And, yes I had those miserable skin tags around my neck removed.

    Changing your lifestyle to have way less stress- and radically changing to a healthy diet -are the biggest factors in feeling better and improving your fertility. If you stay away from medications you will not mask the symptoms. The symptoms are there to give you feedback and let you know a problem is occurring! Your body is screaming out for help. If you make the lifestyle changes the symptoms will improve. Have hope things can get better!

    Thank you for the video. I was diagnosed with pcos shortly after starting architecture school. After trying all natural remedies out there, 3 years ago i finally committed to the pill. It completely cleared my bad acne within a month.
    I recently did a break of uni for studying abroad and internship. I am back to school now and my acne is back as well. It’s not so severe now but I am still on the same pill. I’m really disappointed because I thought the pill was bulletproof regardless of the amounts of stress I’m experiencing.
    Meanwhile my hair never stopped thinning. It became visible on the front top. The men and women in my family don’t experience baldness, it is definitely not hereditary.
    I’m clueless what to do..

    Hey ladies! I’m no professional, but from personal experience dealing with PCOS for over 20 years I’ve noticed that it’s all the man-made things we use. It’s not just poor diet and lack of nutrients. We destroy our endocrine system with things we put on our skin and hair. I now only use products that have only natural ingredients without chemicals. Dairy and gluten is my biggest issue...makes the facial hair grow back faster. If you want to cure this disorder drink organic raspberry leaf tea and organic spearmint tea twice daily, (give it a few months to work) and do away with the man made foods/products.

    I was diagnosed with pcos at age 15 and am now 20. I was first put on metformin which I had all adverse reactions to, they switched me to spironolactone and again i had all adverse reactions. Then i was put on my first birth control, Yasmin. It helped with hirsutism and skin but it made me miserable and depressed for 3 years. Now i am on AlySena birth control since i believe June, and about a month ago i started getting painful papules like never before, and my skin and hair is way more oily. And i have had thinning hair on the sides of my head where the baby hairs are for forever and I've been trying different things in my skincare to help but i might have to call my OB GYN and try a new pill or something again 😭. It seems like the medications never work properly 😔

    all my life i have been so concious w/ my body hair and tried multiple hair removal done but eventually gave up not knowing i have pcos..
    i did went to my ob because of my bleeding aftr not having my period for almost 5mos and had ultrasound (that experience was traumatic 😖)
    And found out i had pcos but it was not explained so much and just told me to lose weight or take pills.. but sometimes pills (local brand) doesn't work so i stopped.. After 2yrs i was back at pills (diane35) again because i gained weight and more than 6mos i'm not having my period again.. So this is so much helpful and inspire me to help myself get better more than ever

    Can you do a video on how to treat discoloration? I have a lot, my inner thighs, knees, elbows, armpits, and butt. I’ve tried sugar scrubs but those don’t help much. Peels burn and can’t really last too long with them on my body :( 15% TCA and I’ve tried 40% glycolic acid.

    Could you please make a video about powdered sunscreens (or mineral powder foundation with natural spf)? Some say it's a great way to top up your sun protection throughout the day, but shouldn't be your primary sun protection. I'm wondering exactly how well it helps you top up? Like, if I apply a regular zinc oxide-based spf 30 sunscreen at 8am and then just use my inika mineral powder (with zinc and titanium oxide, spf 25) throughout the day, say every 3-4 hours, would I maintain my initial spf 30 protection? Thank you!

    I wish we get answers. I have a history of PCOS and have severe acne breakouts that affects my self esteem. Problem is, I'm currently breastfeeding my 10month old son. Both my OB and dermatologist say i should give up breastfeeding. Im so torn!!😭😭😭

    I had pcos almost 7 months no menses, acne and oily skin in face and i gained weight and my doctor prescribed me pills to regulate my menses and metformin to prevent me from having diabetes, while and taking those medicines I feel more fatigue so I stopped taking of those but suddenly I was shocked I was bleed for 2 days then that's start of my regular menses,I started also yoga and pilates also I manage my diet it helps more to improve my pcos

    This was so good! My Dermatologist wanted me to take Accutane or Spironolactone but I wanted to try to treat it more naturally 1st. I did lots of research and took a DIM supplement and Liver Dtox and literally within a few days the huge cysts on my jaw, chin and neck flattened. Then they completely disappeared. The DIM has completely changed my skin. I had over 30 huge painful cysts and literally hated leaving my house. What are your thoughts on DIM? Thanks so much for all your great info!!!

    My dr put me on metformin (side affect is headache ) but I’m gonna keep taking them hoping they pass after they get use to my body and eflornithine(vaniqa) he was gonna give me some pill butsaid it could give me a man voice so I said no I have learn over the years tho it’s all to do with weight when my weight is down I don’t get half of much of the symptoms as I do when I put weight on so now I stick to a low carb diet (well I try to haha ) 🙌🏻🙌🏻🙌🏻love all your videos

    Thank you doctor, may God bless you, for me I had so many times an blood analysis to find out if I have that condition of insatable hormones but no, they are OK but still have acne, over weight and a lot of hair in my legs at age of 32, but I am using curacné for 3red month the first two 20% those two months 30%, my dermatologist give that after he see that I am so dry he want to give 40% but I think he will next 2 months... I am on diet and I lost almost 10Kg, I take collegene too and i see so much differences thank God but I am wondering if I need something else

      me too. I have all these symptoms but my culture doesn't believe in dermatologists or skin problems. so its hard to convince my mom to help me. All she tells me is eat right and stop being very clean because she believes having a bit of bacteria helps my skin be normal....and I'm a germaphobe...

      @@steffanyprtll I stopped eating refined carbs (white rice, white bread, biscuits), sugar and dairy for about a month and the acne went down a lot. I’ve been trying to introduce those foods back into my diet to see if they’re really the cause for my cystic acne and yup… you can try altering your diet too. It’s pretty challenging sometimes but it’s okay to have a cheat day or two. I wish you good luck?

I cut eating rice and drinking milk 6 mos ago but now i go back to eating rice and my acne is swelling up feels like there is an inflammation

      Try seeing a different local doctor for referrals to see other doctors (specialists). Seriously a good doctor won't mind you getting second opinions. Because doctors don't know everything.

      girl, I recently tried OMAD+ no carbs, diary and sugar for around 5 days and my period unexpectedly came back (I was doing OMAD for weight loss, so it was a pleasant surprise my period came back). My skin cleared up! never had my chin so clear with no bumps. But given I never had those huge cystic acne problems, I did have hormonal acne. So I urge you to look up intermittent fasting (start with that) and then slowly ween your way in to OMAD (one meal a day). Look up Dr Berg about this. I do it for insulin levels bc most pcos ppl are insulin resistant, and everytime we consume something in our body our insulin levels rises, which is bad for us, so by doing either IF or OMAD it helps our insulin.
