Boom Lift Catastrophic Accident, being unloaded the WRONG way.
JLG 1500SJ, 150 foot boom lift, cherry picker, self propelled boom lift, MEWP, Mobile Elevating Work Platform, AWP, Aerial Work Platform, Bucket Truck, being unloaded the wrong way and direction off of a lowboy tractor trailer truck. Fortunately the worker was not injured severally due to the fact he was wearing his full body fall arrest harness and it appears to be fitted correctly. Workers have died in incidents and accidents like this when they do not fit their harness correctly and the shoulder straps come off of their shoulders and they fall out of the harness. Mostly due to a harness that is not fitted correctly. A full body harness that is too loose or the chest strap and/or the rear d-ring is too low or loose can cause the worker to literally fall out of the harness completely with not protection from hitting the ground below.

This is why we didn't wear harnesses under 2 stories height. And usually not on the third if it was summer and dry. I cannot tell you how many times we heard about broken ribs and groin area injuries far worse then a 15- 20 ft fall, FAR worse. Safety is only safe if it doesnt make your job overly complicated and cause injuries itself over time..Common sense and a basic plan of operation goes a really long way towards fast work with no injuries. P.S when young I was a blind crane guide I couldnt strap in much as i was all over the place watching the load set, running back along wall tops to guide crane operator, all day long. @x4 wall in fall were the worst, theyd get frost in a spot and youd surf on your boots for three feet along a 2x4 wall top 5 stories up at 7:AM, drop like a possum onto crotch and your safe, fall any other way and your screwed. You learn to get a hand under your crotch before racking yourself when falling too after awhile...1970's - 90's construction everywhere. I survived! Sorta, im all busted up now...creak creak creak.

I would say that if you operated this equipment in accordance to manufacture recommendations this would have never happened and you wouldn't be worried about wearing fall protection which is mandatory.
