    I am 53 and I still don't have any deep wrinkles. I have rosacea so my skin is very, very sensitive. I came up with a routine that works so well that my GP didn't know I had rosacea until I told her. I wash my face at night with Vanicream's soap bar. It is a brand that has no scents or artificial colours so it doesn't hurt my skin. While my face is still wet I use three drops of jojoba oil. Jojoba oil has slight antibiotic qualities so it calms it down. Then I finish with Vanicream's moisturizing cream for sensitive skin. It has no scent and no colours and it contains petroleum jelly which works well against wrinkles. All of this is less than £30. I will be doing this until I die because it is cheap and it works on my skin.

      Vanicream sunscreen for sensitive skin is my favorite sunscreen. And I tried many. I also use jojoba oil daily. 52. No deep wrinkles also

    From a 53yo with rosacea, fair and oily skin, who is allergic to sun and to most artificial fabrics: Don't give up on retinoids! I noticed that azelaic acid (since I have rosacea) helps the skin to be ready for retinoids. Have been using sunscreen as well daily since 18yo, due to my prone to acne skin and also my mother had melanoma at the time. Cotton or linen light fabrics with UVA treatment are a must to my sun allergy, in order to dessensibilize it slowly every spring and be able to enjoy outdoor life (hiking, beach, you name it-always with sunscreen). I have been avoiding corticoids for at least the last decade, despite all my skin sensitivities.

      I have PMLE - sun allergy has completely destroyed all my collagen on face/hands & feet
      No matter what I do it doesn’t help other than a little Botox & filler x

    That's true about Retinol. I'm a natural blonde. The first few applications of pure retinol acid were quite uncomfortable. But I stuck it out and now I have smooth glowing skin one year later. Use it every night.

    Guys! I’ve got to share this. I am 60 years old. I started retina a when I was 24 years old. I was given retina A CREAM and tried that for 30 years. i was constantly sensitive RED skin did not peel much but I could only do it once or twice a week recently I got a tube of retinol a GEL! Gel changed the game. I was allergic to that darn cream. I do Trentinoin 1% GEL every night a bunch of it and don’t even need moisturizer but do a small amount in the morning with sunscreen. I wish I had known sooner. No provider ever filled me in on this, dang it, but I’m so grateful now this gel is amazing. My daughter who just turned 30 has been on it for a few years as well. It’s the golden ticket and I still look forward to my deep plane in a few years. Sorry for dictation here Siri kind of sucks.🙄😆

    Great info for people with these issues Doc. It must be mentioned though that if you live in a dry climate or the winter weather is drying your skin out that hyaluronic acid will make this worse as it will draw moisture from your skin if there’s none in the atmosphere to draw in. I struggle with dry skin in the winter and have to avoid HA for this reason.

      Thank you for watching! Hyaluronic acid is generally effective at hydrating the skin and doesn't typically draw moisture from the skin in a harmful way. In extremely dry environments, it may not perform as optimally due to a lack of moisture in the air, but this can be managed by using it in combination with moisturizers and applying it to damp skin. If you are in a particularly dry environment, layering hyaluronic acid with a good moisturizer can help ensure that your skin remains hydrated and healthy.     For those who think retinol is not tolerated by their skin, 1. Keep applying it but reduce amount and how many days in a week plus sandwich method. 2. Skin needs to get used to it so keep going, forget about how your skin may look like. Prioritize the getting used to the retinol over looking great next morning. Use cream and hydrating sprays during the day if you hate the pilling from retinol. But get your skin used to it. 2 or 3 month sometimes.

      Good recommendation! I started a retinol a month ago and notice a few red blotches on one cheek. My first thought was to avoid the retinol if that is what is causing the dermatitis but will continue for a while longer.

      This! Also consider applying a moisturizer 30 minutes *before* applying retin a. The retin a will still work, but your skin will stay hydrated so you'll better avoid the flaking & irritation.

      I tried tretinoin consistent for 6 months. I really tried everything. I wanted so bad to make it work but nothing worked. My skin was terrible. As soon as I stoped using it my skin was so beautiful. I just returned to retinol and I’ve been good. I know it’s not as good but for my skin it is 🤷🏻‍♀️

    Dr Karam - this is so helpful & I will share with my daughter! One question on RF, I’m getting to confused & you seem to be on top of this. I’m almost 60 and had Halo done on knees & thighs for skin wrinkles & rejuvenating. Great on rejuvenating but nothing on wrinkles. Now my dr wants to try Secret RF because it doesn’t go deep like Morpheus and won’t destroy fat but she said will tighten skin which is what I need. From what I can tell you’re saying RF really does nothing for this.

      Thank you for the question, Babs! RF Microneedling will not tighten up loose or sagging skin (jowls, neck laxity, upper/lower eyelid sagging etc). When properly administered this procedure can be great for crepey skin, fine lines and wrinkles and general skin aging that needs to be addressed.     I am 68 and after years of using it successfully (and uneventfully) just last year Tretinoin started making my skin peel continuously. It also changed the texture of my skin, giving it a thick, almost poultice-like appearance. In addition, the color was a patchy red. I tried using it just a few times a week, but even once a week I would peel almost every day between applications. Finally I stopped using it and started using topical estrogen cream instead and simply exfoliate / dermaplane a bit more often. Mt skin looks and feels much better, but lines are deepening and I am scheduling a facelift within the year.

      Try The Ordinary Ascorbyl Tetraisopalmitate Solution 20% in Vitamin F. I can use it on my neck without any issues, but I get irritation with active form of vitamin c serums.

      I actually created Quench for just this reason. It's a combination of 3 different formulations of Vitamin C and other supporting ingredients. It is designed to increase the effectiveness of the product over standard Vitamin C formulations yet be well tolerated by all skin types, even sensitive skin. This particular compound out performs the typical Ascorbic acid that is used in many Vitamin C serums which can be irritating. I believe that microdosing Vitamin C twice daily in comparison to using a standard 10-12% Vitamin C serum once daily is much more effective and also avoids reactions like breakouts and irritation so you might consider giving it a try karammdskin.com/products/quench-vitamin-c-serum

    Can sunless tanner make melasma worse? I stay out of the sun and use sunscreen religiously. I use sunless tanner for a nice glow, but wonder if the mechanism of action is sending me backwards in my efforts to reduce my melasma. Thank you!

      @@dramirkaram Thanks so much for your reply doc, I really appreciate. In my experience, and they are many (age 52 here, tried a lot), niacinamide absolutely dries the skin. Just one example, by mistake I bought an eye cream with niacinamide (I knew I shouldn’t, for the reason mentioned), in few days, my upper eyelids were so dry, cracked as a desert. It took me 2 weeks (!) to get them back to normal. Also my chin, nose, under eye area - they get extremely dry, even flakey, with niacinamide products. Maybe 2% max doesn’t do that, but I have no idea why they say it’s keeping moisture. Sorry, I’ll keep saying - avoid if you have dry skin, or on areas with less oily skin - like around the eyes or neck, as what it does, it regulates the amount of oil produced, and this is a problem if your skin is dry.

    As a skincare expert myself (and by ‘expert’ I mean I’ve watched all your videos) I think this one was spot on, as someone who has ALLERGIC skin but can handle tretinoin and all that good stuff. But put an ingredient im secretly allergic to in a face moisturizer and my eyes and neck will swell by morning. It’s insane.

    Ugh! This won't play for me! I really need to hear what you are saying. My skin burns when I use some "actives" like vitamin c, it stings. I'm not sure if I should keep using or what to do????

    Perfumes, fragrance and certain ingredients in skin care give me burning, red eyes and or red or itchy skin in the area. Dry weather like in California, Nevada and Arizona make my skin super dry meanwhile vacationing in Florida my skin was soft and plumped.

    I am on trifecta and have chin irritation so bad that my skin is oozing and my eyelids very dry. I have been on it for several months with no irritation and now it has started. I don't want to stop and have started using aquophor in irritated areas. I have stopped for a week and skin is getting better. Should i start back?

      If your are experiencing oozing and your eyelids are very dry, it's essential to take immediate steps to address the issue and protect your skin. I suggest you wait until your skin has completely healed and then re-introduce the products one at a time to see how your skin reacts.     could you talk about what people can do who have gummy smile? is botox the only option if the country you live in, doesn't have dentistry that helps with that? if botox is used for gummy smile for years, does it ruin your face and make you look older/like your face is melting with time?

If specialized dental procedures like gum contouring or orthognathic surgery aren't available in your country, Botox can be a viable option but prolonged use of Botox can lead to slight muscle weakening or atrophy in the treated area.     Is tretinoic acid better than retinol?

      Tretinoin (tretinoic acid) and retinol are both forms of vitamin A but they differ in strength, effectiveness, and how they work on the skin. Whether tretinoin is "better" than retinol depends on your skin type, concerns, and tolerance for these ingredients. Because of its potency, tretinoin can cause irritation, dryness, redness, and peeling, especially when you first start using it. Retinol is available over the counter and is a milder form of vitamin A. It needs to be converted by the skin into retinoic acid to become active, which makes it less potent than tretinoin and it delivers more gradual results compared to tretinoin. It’s effective for improving skin texture, reducing fine lines, and treating mild acne, but it takes longer to see visible results. The retinol in KaramMD Illuminate is a cutting-edge and produces results but is well tolerated for all skin types and textures even for sensitive skin; therefore the concentration of the retinol has had to be carefully modulated to meet these specifications.     I have questions if you are really answer me I would appreciate it. I do have visible 11 line between my eyebrow( i use a lot expression when i talk) botox not really works i still see the lines. Two doctor I went they suggested to me i should get my eyelid done. Is it possible to smooth those lines without eye surgery? Why they suggest eyelid surgery to smooth 11 lines?

      Some mineral sunscreens use nanoparticles to reduce the white cast, and while studies suggest they do not penetrate healthy skin, there is ongoing research about their safety. However, current evidence supports that they are safe for use. Thanks for the suggestion regarding Estriol. I will look into it and consider it for a future video!

      The idea that retinoids cause skin thinning likely stems from misunderstandings about the early side effects, which can make the skin appear more fragile. However, these effects are temporary and Clinical studies have consistently shown that long-term use of retinoids enhances skin structure by increasing epidermal thickness and improving the density of collagen and elastin fibers in the dermis.

    doctor, is there any connection of collagen supplements and heavy sweating? for whatever reason about a month ago i started to heavily sweat on my face, especially my nose.. i have not made any changes in my lifestyle, i‘ve never seen sweat on my nose before. collagen powder is the only thing new in my life 😭

    Dr Karam, Here's a surgical concern I haven't heard any content creator speak about: Future resetting of the genetic programing to it's more youthful state. If this becomes available in the next 5 - 10 years for humans, either by editing of 3 of the 4 Yamanaka Factors or something simpler, how do you predict this would impact a face that has been surgically altered?

      This technology would likely require a rethinking of how we approach cosmetic procedures, with an emphasis on adaptability and long-term outcomes. While there has been promising progress, we aren't close to making this a practical and safe option for humans so I haven't given it much thought at this point.     It's weird, my skin tolerates in office deep microneedling, but if I get the most gental of chemical peels my skin freaks out, I'm told to expect a bit of slight flaking after a pca peel but I shed and get swollen eyes! I think the dry red skin is normal, but I get a swollen face. I don't mind if my skinny face gets a bit swollen, but I don't think it's normal, very itchy. I've stuck to only microneedling this year. I like it it works for me. I do wonder if the last girl that did my peel added something that I may be allergic to, but I was afraid to tell her what she did. She doesn't work there anymore, I wonder if I should try again. Don't know. I'm kind of afraid of it.

      PCA is a great peel if applied correctly. There are many different protocols. Perhaps your Aesthetician may have either been too aggressive or maybe not using the proper peel for your skin type.

      @dramirkaram Thank you for responding. I use my plastic surgeons office. They have women in charge of the med spa services and always did a good job. She stopped working at the office o go to, so to 1 of her other girls. I asked her if she followed what was in my chart. She said yes, but I think she was just covering her mistake. I heard the women in charge is coming back. I might try again with her and tell her about what happened. The girl that hurt me is no longer there. Maybe I'll try 1 and not buy a package. Just in case.
      I did my last skinpen treatment yesterday. It's time to figure out what to do next.😉

    Hi. Dermatologist here. It concerns me so much that a facial plastic surgeon throws around a VERY unregulated term such as "Clean" in his video. It is not backed up with scientific clinical research or is even evidence-based at all.

      Thanks for that feedback! I realize "clean" is an unregulated term. In this case I'm referring to products that avoid ingredients that are considered harmful such as parabens, sulfates, phthalates, formaldehyde, and certain synthetic fragrances and dyes etc.
