  • Ankle ligament injuries are extremely common. The usual mechanism is rolling over the ankle, or what is described as an inversion injury. The most common ligament to be injured are the ones on the outside (lateral) part of the joint.
    In this video, Mr Hisham Shalaby, a highly specialised Foot & Ankle expert in Edinburgh - Scotland, describes the causes of ligament injuries around the ankle and their acute management, including the rehabilitation.
    Ankle soft tissue injuries can be as simple as a strain to the ligament, that usually heals well and leaves no long-term problems. Equally a bad injury, that results in significant swelling and bruising, can signify a more substantial damage to the ligaments. If this is not treated adequately the patient can be in pain for a long period and might feel like they cannot trust their ankle again.
    Significant soft tissue ligament injuries can leave an ankle unstable. An unstable ankle can cumulatively cause ankle arthritis and can lead to further injuries and more damage.
    To avoid instability the initial Ankle soft tissue injury treatment must be aggressive. This generally entail a period of immobilisation in a walking boot. This avoids excessive calf muscle tightness and allow the ankle to rest and the swelling to subside. It is important to rule out fractures and an x-ray is usually needed even if the patient can weight bear.
    After the initial period physiotherapy can start and there is a national rehabilitation protocol of ankle ligament injuries that has been developed in Scotland, that any physiotherapist can access.

    The acute management is a period of immobilisation & rest followed by physio. If the ankle is still painful or feels unstable after 6-12 weeks of rehabilitation you need to see a specialist and may need further imaging. Hope you feel better soon

    I've done atfl ligament reconstruction on 01 March 2023, but still I'm experiencing severe pains, and I can bear weight and I've been complaining about the pain to my orthopedic who have done operation , he then refer me to psychology, but I then decided to see another Orthopedic, yesterday I went to do Xrays and MRI scan, and the report says, periarticular soft tissue swelling and ATFL torn off. I'm really angry for my doctor for not listening to me for more than a year.
