Municipal cemeteries can be different, I know of one where one row of graves have headstones whilst the row backing onto them either are buried opposite facing or they have foot stones rather than headstones. But allows more space across the acreage.
Why do are they in such disrepair? Don't get me wrong; there is something beautiful about it but it seems treacherous with the stones leaning all over the place. It would seem that they would be made to remove them or reset them if the public is free to walk around them.

Before magnetism was understood and compasses were a thing, people valued the rising sun with a higher importance, therefore placing this at the top of the map. They related this to the orientation of graves also, people should be buried facing the sun!
