Is High LDL Cholesterol Total NONSENSE? - Thomas DeLauer
There's two reasons why your cholesterol is high. One of them is because you're probably burning fat and right now you probably think I'm crazy for saying that, but I have tons of scientific literature and I have legitimate science to lay in front of you, so you know that this truly could be the case.
The second reason is that your insulin might be high, or your blood sugar might be high, and it's affecting your cholesterol. One thing that is not affecting your cholesterol in a negative way, is being on a lower carb, high fat diet, because that's definitely not where the science links. We're going to break this all down.
First things first. Cholesterol. Cholesterol is vital. It is made by the liver. Only 15% of our total cholesterol in our body comes from our diet. The rest is all made by the liver. 85% made within our bodies. Guess what? If we consume more dietary cholesterol, our liver produces less, and vice versa, because our body's always trying to keep a balance of cholesterol.
Cholesterol is not the problem. In fact, just that you know, cholesterol is responsible for so many things that keep you lean and healthy. Cholesterol builds vitamin D, cholesterol builds testosterone, estrogen, all these other hormones, and it also builds our cellular membranes. It is very, very vital.
LDL and HDL are not good or bad. HDL, which is known as the good cholesterol and LDL, which is known as the bad cholesterol, aren't even cholesterols. Say that again. They're not cholesterols.
They are carriers of cholesterol, but they also carry other things. HDL, which is the good cholesterol is said to be good because it carries cholesterol back to the liver to get processed. LDL, or bad cholesterol, is bad because it delivers cholesterol from the liver to other cells that need cholesterol.
Think of it like this, and Dr. Dave Feldman actually broke this down really well in some of his other videos. Basically we have the boat. That is the lipoprotein, whether HDL or LDL.
That's the boat. Triglycerides are the passengers. The precious cargo. The ultimate goal, what we are carrying. Cholesterol is like a life jacket. It's just there. It's important, you need that life jacket, but it's not the actual critical cargo. It's the triglycerides that are the critical cargo here. Then does that really answer the question of LDL being bad or not?
mdlingo.com/mo...
What happens is when LDL reacts sugar in the bloodstream, a process called glycation occurs. Glycation is where the cholesterol and the sugar come together and turn into a harder, denser lipoprotein.
Normally this LDL carries cholesterol and triglycerides and it floats down its little river through the ocean and it docks at the cell. Nice and easily it docks and it lets off it's simple cholesterol and it lets off its triglycerides. If that boat reacts with sugar, glycation occurs and it becomes oxidized.
That changes the form of the boat. Now the boat is kind of demented and it can't dock anymore. What happens is you end up with a bunch of LDL floating around because it has nowhere to go because no dock will accept it.
Most primary care physicians are just looking at a standard lipid profile. There's good news. If you do just get a standard lipid profile, you can do a little bit of your own investigating.
Here's what you do. If your HDL cholesterol is normal, within a decent range, that's good. Then if your triglycerides are low, you're in a good place. If your HB A1C is reasonable, like less than 5.6, A1C is your tailing measurement of your blood sugar over like 90 days.
That's the simplest way. If all your other numbers are in check, your A1C, your sugar, your HDL, and your triglycerides, if those are all in check, I wouldn't be too concerned with the LDL being high because chances are it's the nice fluffy LDL that is doing its job. If those other numbers are off, you might have some cause for concern and you need to talk to your doctor.

I eat carbs; never cut anything out except added lemon water every morning, ox bile (have no gallbladder), and cute stuff that has gums (allergic); within the past 6 months. Yet, my labs this week showed my HDL stayed in the 60s, dLDL raised to 108 from 74, triglycerides went from 98 to 60, total cholesterol rose from 146 to 170, VLDL is 12, glucose 89, A1C 5.2, and I reversed Hashimoto’s naturally. Good info. I’ve always had a high metabolism. I got on local fire department 40 years ago. Shortly after getting on the department started doing annual physicals. From the very start about 38 years ago I was told I had high ldl and I needed to start taking statins. I refused! I’m not one to run to pharmaceuticals to correct a problem. More exercise more dieting. The next year my ldl shot up over100 more points. Doctors said I needed statins more than ever. Told me I would die soon if I didn’t get my cholesterol down. I refused. About 18 years ago my department doctors sent me to get a heart scan, said they saw something on heart graph while I was on treadmill. They were going to get me to take statins one way or another. After my heart scan I waited to find results and to get paper work for the department doctors. The doctor came out with the findings. First she asked me why I was there. I told her why and she laughed. She said I had the heart and arteries of a 20 year old. I was in my late 50’s. I thank her and delivered the paper work. To this day I am still being told that I need t be on statins. The doctors just don’t get it! Oh, and I still refuse. This video just says what I’ve known all along.

It is a pretty good overview of the subject, and I didn't notice any glaring technical inaccuracies. @@sadielong3021 You're welcome. I particularly recommend listening to Dave Feldman. He's an engineer/programmer rather than a doctor, so I find that he presents the information in a very clear way without a lot of unnecessary technical jargon that can be confusing to non-doctors (of course there is still plenty of _necessary_ technical jargon!). He's also not biased by the sort of preconceptions and dogmas on the subject of cholesterol that get drilled into doctors in med school which often prevents them from thinking clearly. Dr. Paul Mason is another person who is quite easy to follow, and has some great videos about cholesterol (see in particular: "High cholesterol on a ketogenic diet"). Peter Attia has good information, but it's pretty advanced stuff. For debunking the diet heart hypothesis and the purported benefits of statins, David Diamond does a great job. On the subject of CAC (coronary artery calcium) tests and their role in diagnosis, Ivor Cummins is the guy to listen to. On the mechanisms of how insulin and glucagon control blood sugar, Ben Bikman is the expert. For general issues related to nutritional biochemistry, Chris Masterjohn is another great source of information.

Thomas...you spoke with great passion today and I like that! So my HDL came in at 73...LDL came in at 143, triglycerides at 62 and total cholesterols is at 228. And my A1C is 5.0...But my doctor wants to put me on Lipitor...but I told my doctor before that I changed my dietary lifestyle to low carb and no sugar...he says “risk of major adverse cardiac events is elevated at 8.5% should I be fearful of his comments?🧐
You're remnant chlorestral is (if I've worked it correctly). At 12. So look at a video on remnant chlorestral - personally I'd be happy with those figures.
My figures are as
Total 188
hdl62
Ldl 112
Triglycerides 70
Remnant 14
A1c 5.1
I think it's this obsession with 200 that's the problem- let's say I was 188 but with a much lower hdl and a higher ldl- I'd be more worried but the doctors would still probably say I'm ok-
you know its funny.. i went hard and lost 30 lbs with low carb IF and i eat very clean. My bloodwork says my LDL is slightly elevated, Doctor thinks nothing of it bc my triglycerides are very low.. but now i know why. Ty bro
Be lucky you have such a doctor :-)
I knew tom in high school.
Definitely a good guy
As long as A1c, Triglycerides and HDL are in a good range, then high LDL is likely okay.
Are you kidding? HDL means nothing. LDL means everything. If you have high LDL your chances of getting sick or dying from heart disease is enormous. Triglycerides are bad too of course. Lots of fat is unhealthy. It doesn't matter what kind of fat it is. Some fats are just less unhealthy. Eating large amounts of dietary cholesterol increases your LDL levels unless you already have high cholesterol. Thomas is off the rails. It has been proven a zillion times that high LDL leads to heart disease. "Fluffy" LDL is less harmful then the small LDL but it is still bad! This is crazy. Lowering it lowers chances of getting heart disease. Drug companies have to prove this to show that statins are effective. They've done it a zillion times.

The best explanation out there Thomas. Thank you. Being on keto and happy to say all the tests are perfectly normal. Have a small request for you. Can you please do a video on keto for high blood pressure?
Jude Perera I agree!!
I can't tell you scientifically why, but my systolic blood pressure has dropped 25 points since keto, and that's after cutting my BP meds in half. I've had high BP for probably 25 years. I'm ready to drop them altogether. Would love Thomas to explain this phenomenon as well.
My triglycerides are 110 and my HDL is 55. Both within normal range. However, no where on my lab bloodwork is my A1c...??? But my LDL is 167. Like you said, I'm not worried and would never take a statin! The doctors try to get me too, but I refuse. Great explanation and I have determined that I have the good LDL thats just doing their jobs!
Thank you Thomas! Between you and Dr. Berry, we know TRUTH about our health now!
This is so informative. Thanks for making it simple to understand.
Wow, this is really good. After losing 37 lbs and doing ADF... my cholesterol went through the roof. 450 total 350 LDL. My doctor then wanted me to see a Cardiologist. I have decided against it. The only problem I had was my HS-CRP was elevated as well, and this is not the first time it has been when I have been on an "aggressive" diet. In 2012 I did a liquid diet and my HS-CRP was exactly where it is now. Though my doctor did the little particle and big particle test and my LDL was big particles.
Great video... I've picked up bits and pieces in recent years and this puts the mixed up puzzle together in a coherent manner. On another note and indirectly related, as important as vitamin K2 is, a video on its importance and how it works and its relationship with D3 and calcium would also be terrific if it comes out as well as this one.
I actually had really bad numbers ( dangerously high triglycerides). My precious cargo was sinking the boat.
The keto woe brought my numbers to an almost "normal" range. My body knows what to do without sugar confusing it. Something that all of the meds I've been on couldn't do.
Absolutely the best video that I have seen explaining this in a way that I can understand. Thanks so much!!
Amazing information 👌 👏, my daughter is 17 years old and she is on low carb diet lost more than 15 kg and her ldl so high, the doctor after she asked me about her diet she thought cause my daughter reduced the fat and cutted off the sugar and carbs with this ldl numbers that's mean its genetic and put her on medication, but I refused because I know she's healthy and I'm on keto so I had information helped me to understand, she told me that you'll take full responsibility by not giving her cholesterol medication for the rest of her life.
I'm appreciated there's people like you, May God bless you 🙏 ♥
Remember the time I went to the doctor and got a blood test at the end of a 7 day fast. A few days later I got an urgent call. I need to go on statins. I told the doctor before hand and after I water fasted for 7 days. Went Over his head. I needed to go on statins

Same here. It's too bad. Finding a doctor NOT behind on their reading is proving difficult

Just received my physical results and the first comment the Doctor left was to consider taking meds to lower my LDL so glad I watched this video before calling her back!
Thank you Thomas. Recently, I was able to convince my 76 y.o. mother to get off her statin drug. I bombarded her with your videos (and Dr. Berg's). I'm sure she was prescribed it as a "preventative" due to her age and not being on any medication. She has also been on Keto for 4 months now. She's lost weight and feels great. I'm sure she can run circles around her drug pushing physician. Next task, find her a new one...but not a drug pushing one😉

Great explanation! One more thing:
You can also calculate your remnant cholesterol by substracting (HDL+LDL) from your total cholesterol. You want your remnant cholesterol low cause unlike LDL, this is strongly associated with heart diseases!
what do you consider low remnant cholesterol? mine is 17.
Isn't this the same as VLDL
no, the calculation is just a crude approximation, of not that much value
At last someone who talks to be understood. I watched so many videos and couldn’t understand a thing … I watched this video and now everything is much clearer . Thank you so much Thomas

That sounds like my case. This January my Hba1c:5.3, total cholesterol: 226 triglyceride:84 LDL:125 HdL: 84. My primary doctor said my total cholesterol is high cuz my HdL is high and not concerning ldl since I have healthy lifestyle. This was before low carb high fat diet. But I’ve been regularly exercising at least 2-3 times a week. Now I’ve been doing low carb and high fat for 4 months now. I’m going to doctor next January so I’ll compare the difference. Lisa Gordan Great for you! My Mother in law took statins for years and deteriorated her health daily. Had a stroke and just went downhill from there, mostly brain issues 😢
Good on you. Just avoid the sugars as much as possible/ fast and you will be fine.
Go on KETO and Intermittent Fasting and say goodbye to all your meds, including that stupid Doctor.
Thomas, this is brilliant. I received my blood test results today and low and behold, my HDL and LDL were high. Once I viewed the parameters you listed (HDL cholesterol, triglycerides, HBA1C) on my report, those numbers indicated they were well within healthy range. Not surprisingly, I believe the high numbers are as a result of my (generally) keto diet and high levels of activity/exercise.
Let's see a video on triglycerides. I never hear information on them. Other than it's bad, but its keto energy? So how bad is it?
Search Dave Feldman. He's done plenty of lectures and podcasts. Leader in the field. Kinda dry but has all the answers you are looking for. Thomas breaks the research into bit size pieces for the masses. And kind of a jack of all trades. Doing very well at it.
The triglycerides are bad because it causes inflammation of the body and more over it oxidizes the LDL cholesterol which are transportation for the cholesterol. Remember there's no much thing as "good" or "bad" cholesterol because they are lipid proteins.
Triglycerides are not bad _per se_ -- they are metabolic fuel. However, if you have a *fasting* blood test (12 hours fasted) and your triglyceride numbers are still high, that is a bad sign. There should only be low levels of trig in the bloodstream in the fasted state. This is because if your body is working properly, trig is getting transported and _delivered_ to cells that need energy, and only limited amounts are in transit at any given time in the fasted state. If there's still a lot of fat in the bloodstream, it suggests that there is probably a lot of damaged LDL particles (oxidized/glycated LDL) carrying around this fat with no place to "dock" to deliver the triglycerides. This pathological condition is almost always caused by excess sugar, and especially fructose (fruit sugar) in the bloodstream.
Note that only a *fasting* blood glucose test has any evidential value at all. For several hours after a fatty meal, your blood will be carrying around huge amounts of trig that's been picked up from the digestive system and is being transported in the blood via chylomicrons (the supertanker of lipoproteins!). This is perfectly normal and healthy.

That was a very good explanation. Just had a blood test and hdl is high (75), triglycerides low (45) and ldl is pretty high at 145. Statins are being recommended. Your conclusion has me feeling better.
Went to my doctor and he was freaking out over my high cholesterol ( 300+) and informed me I am a high risk for having a stroke or heart attack anytime and wanted to put me on cholesterol meds. I asked him what was the HDL and LDL levels, but he said "it doesn't matter, it's combined over 300." In the past few years I've dropped 70 pounds on a KETO OMAD 75/25 meat/veg lifestyle. How yah been doing now?? Did you find a new doctor?
True science is starting to win out in the human diet world.
Weird how this "true science" is not backed up by any studies ever
Unfortunately it is not. We still have the plant based and vegan pushers on high that deny the science of fats and cholesterol. Most nutritionists and cardiologists still follow the dis-proven diet heart hypothesis. Even the much recommended DASH diet is not based on any real science.

It’s cholesterol that enables us to convert sunlight to Vitamin D. So, it’s kind of like chlorophyll for people.
Bingo thats why theres vit d deficiencies and sun sensitivity ...docs telling people to avoid fats and placing em on stupid statins...
But as we age, our ability to make vitamin D diminishes. See Dr. William Davis. If you're past age 40, you probably aren't producing much vitamin D, so may want to supplement D3.
What a great video . I just got a call from my doc telling me that everything is great with the lab except my LDL , she was even shocked cuz she knows that I literally worship working out and I’m a marathoner plus I don’t eat junk 🤷🏻♀️
“...did you know this?” ... Thomas, I was taught in school that I should eat brown bread at night as it clears the body of unhealthy fats... it’s fair to say that I had been aware of 0% of the incredible information you share. 🙏
I have been on 3 hypertension meds and a statin for about 20-25 years. I have been keto for 6 months and went 5 pounds lower than the goal my cardiologist set. My EKG, BP, and in-office exam were all perfect, but he won't take me off any meds. I'm compiling a list of before & after meds blood pressure readings, but I am sure he will want the dreaded lipid panel before making any changes. I don't really know how to talk about this complex science to him, but I have to try, (rather than just switching doctors) because if he is open-minded, it may help other patients.

I have verified your information based on physiology and biochemistry and trust me, you have the right information.

I listen to this channel as well as read and listen to Dr. Robert Lustig. He says don't worry too much about total cholesterol and LDL. Its best to calculate the ratio of triglycerides to HDL (TG/HDL). You want to be below 2.5. 1.5 and lower is very good. So try to reduce TG and raise HDL.
When doing a fractionation test, what are we looking for? My over all cholesterol and triglycerides are extremely high, in he 300 markers, and my A1C is 5.6, is that good or bad?
