I focused on that because patient kept talking but I remembered this video and listened to the patient since I had completed the documents. I did not have time to ask about ADL. I was so disturb that I didn't have time for that but I passed bc I was focusing on the patient rather than being scripted. Thanks again your videos are very helpful
Also I wanted my patient to say he has chest pain but patient said zero pain even though he was admitted for chest pain and SOB. I remembered you saying that do not create your own problems for the care plan hahaha. I had to explore another need

Hi Nery as long as you are doing all required tasks that should be fine. Personally, you do it as per marking criteria. If you do it before then it defeats the A to E process
