The thing at 13:41 once happened to me unintentionally because my Wii Remote battery life finally decided to quit on me at just the right time (although I don't know if it was necessarily a game over).

But if Mario or Luigi gets crushed by a Thwomp, Whomp, Whomp King, Ledge Hammer, Rhomp, Millstone, or Tox Box, then it is instant death.
Only few enemies that not give instant death when squash
Like Digga Leg and Bouldergeist are enemies that crush Mario or Luigi but do not one shot them.
