You forgot "Both-side Blocked Door Death" which is used in some advanced troll levels. How it works: Normally, when you enter side A of a door you will appear at side B. if side B is blocked with an obstacle, then you will return to side A. Setup: we place a tracked solid object (we call it Doorblock)that block the door side A at the beginning state of the level then it will quickly fall off the track. when you enter side B and get forced back, The level state will be reset and Doorblock will appear again and block the side A of the door. When mario cannot exit both sides, he automatically dies. this secret death is shown in some of DGR's troll level videos. For short: when mario get blocked by solid objects in both sides of the door, A and B, he dies.
