That was definitely a sorcerer; just one of the more mutated, exotic types: Tzeentch sorcerers. Chaos daemons do use warp magic, but that level of magic requires an actual psyker or some kind of incredibly powerful daemon (like a daemon prince, a daemon primarch, or a greater daemon). Chaos worshippers are commonly mutants as well; especially psykers who serve the dark gods. Exposing your fragile mortal form to the incredible energies and corruption of the warp through not only psyker powers- but chaos magic specifically, and you will definitely be affected in some way. I think it depends on the whims of your patron deity though. If it serves them more for you to blend in to an unmutated population, then your mutations might be subtle or non-existent. Tzeentch is 40k's god of magic, and his followers are usually heavily mutated. The sorcerer depicted here is definitely unique in its design- but it shares common traits with Tzeentch mutations- the coloration, the hidden flaps of the sightless mutant (who probably sees through the warp) disguising a single powerful void surrounded by small eyes- all are heavily "Tzeentchian" in style. When you think Tzeentch, think more like crazy H.P. Lovecraft horror mutations. Nurgle sorcerers are usually walking disease hives; Slaaneshi sorcerers commonly sport hooves, extra/exaggerated genitalia, horns, spines/barbs/claws, etc; even chaos undivided sorcerers (those who worship all 4 chaos gods equally) usually have something weird about them (a third eye, an extra tentacle arm, etc)- though not always. And Khorne is a dichotomy- he hates psykers and considers warp magic to be dishonorable... as you know- the magic slaughter god from the magical realm of blood fountains.
