After a gig they walked out onto the stage in Butler the bass players saw all sorts of needles all over the floor in front of the stage and he was freaking out about there were so many Vietnam vets that were at the concert and he freaked out over all the needles that were laying around you know on the floor in front of the stage that's why he wrote this song if you want to hear Naomi's jazz or jazzy musical prowess check out planet caravan
“Hand of Doom” is about the experiences of soldiers. We played at an American army base in Germany and it was sort of a half-way house for soldiers coming back from Vietnam. And they were telling me that to get through the horrors of Vietnam they were turning to drugs over there, which, you know, was never broadcast on TV or anything like that back then. So it was all new to me and I thought it was a good subject to write about. -Geezer Butler

Yes, this song is about drug addiction but was inspired by soldiers returning from Vietnam as addicts.
