Not ATC fault. They warned the chopper pilot twice. Both times, chopper pilot said he had visual on the plane and would use his discretion to let it pass. The chopper pilot was looking at the wrong plane. 3 planes were lined up in sequence for arrival on runway 33, chopper pilot was focused on plane 2 at his 12 o’clock, not plane 1 at his 10 o’clock. There is a structural pillar on the blackhawk located at the 10 o’clock position, so it’s likely the chopper pilot only saw planes 2 and 3. Seeing as plane 2 was over a mile out, he decided to cross the approach flight path. Terrible accident.
It will be blamed on the 1 air traffic controller doing 2 jobs. It will not be blamed on the individual that was not there doing the 2nd job, nor on the individual who allowed 1 person to do 2 jobs. I now understand better why the helicopter pilot was not instructed to stop forward movement and hover out of the landing path.

Obviously human error. Likely the Blackhawk pilots, or ATC. Interested on how the military will spin this.
