I’d use form 5 because it has defensive and offensive qualities

Form 3 for me because I prefer defense.
Palpatine hates using a lightsaber but I wonder how he would do against in lightsaber combat against Tulak Hord, who never lost a duel and died by his apprentice who literally stabbed him in the back knowing he couldn't win head to head. My money is on Tulak, also my favorite two Sith lords are Darth Malgus (his lightsaber is badass) and Lord Momin who is the one that built Darth Vader's castle, he is a unique Sith that prefers to create than destroy.

For me, I would go with Form 2, Makashi.
It is the same as Count Dooku and since it is more elegant and also focused on combating most forms.
Also Count Dooku took on Obi Wan Kenobi and Anakin Skywalker and won and managed to prevail against Yoda!
