    Evening, I’m 58, semi retired and last fall I was dx with osteoporosis. Two months later my husband (M.D.) moved me from Alaska and to Arizona. My grandkids are here in AZ. Always a plus. Yes, osteoporosis can be reversed. I’m doing this thru diet, supplements, weight bearing exercises, weight vest and vibration plate. I’ve lost 12# in 6 months. I had a dexa scan in May 2023. It was the best news! I was worried that all this hard work was for nothing. It wasn’t much of an improvement, about 2% in 6 months.

    1/14/23, 1pm EST, what baffles me about all the osteoporosis videos I’ve watched given by Physical Therapists & MD’s is that no one mentions gastric bypass RNY surgery & malabsorption as causing Osteoporosis. They talk about scary drug treatments or supplements & excercises. People who’ve had the RNY & other gastric bypass surgeries cannot take oral bone meds & from what I’ve read, these drugs have serious consequences & can’t be taken forever.

    • @Dr_DougLucas
      @Dr_DougLucas  Рік тому +2

      Gastric bypass surgery (especially Roux En Y (RNY)) is a known risk factor for malabsorption which puts you at risk for osteoporosis. This isn't discussed often. Bariatric surgery doesn't seem to get much criticism but in my opinion should be avoided. My work with PHD Weight Loss and Dr Ashley Lucas (The Doctor Ashley Show) proved to me that there is an efficient way to lose weight for nearly everyone.

    Excellent information!. As a petite 71 year old, and watching my mother shrink to the point of her ribs sitting on her pelvis, I'm desperate to maintain my bones. I lift heavy weights, walk, take supplements and try to eat right. I would love to hear to speak about Thyroid medicine and bone loss. I believe that is a double edge sword, you need it, but it causes damaging side affects. Why can't a thyroid medicine be made that doesn't damage bones? I would love to hear a whole segment on that. Also, please do a segment on Calcium. Not all calcium is created equal and it is important to know which one to look for at the store. Thank you! If it is safe is it effective?

    • @Dr_DougLucas

      Hi Paul. Strontium citrate has not shown the same risks as ranelate in studies I've read. It is used in combination with other supplements like VIt D/Ca/K with beneficial results in intervention trials. I offer it to patients as part of their stack but let them choose. Many people don't want to take it and I don't think it's necessary for success. It can also have an impact of DEXA follow up which I'm not concerned about but has to be taken into consideration.

    My doctor won’t prescribe estrogen for me because I am more than 10 years past menopause and the medical establishment, going on outdated studies, thinks it’s too risky for a woman of my age. She did approve of me taking DHEA and bioidentical progesterone cream, so I’m hoping these will help boost my hormones in 6 months when I get retested.

    • @Dr_DougLucas
      @Dr_DougLucas  8 місяців тому +1

      DHEA will help a bit but progesterone cream is not well absorbed and not as good as oral micronized progesterone. I bet she also didn’t suggest testosterone which would likely have benefit as well. Starting women over 10 years out from menopause on estrogen does have risks BUT so does having osteoporosis so a risk/benefit evaluation is required from someone that understands both.

I’m -1.8 on my left hip and -5.1 on my right hip, -2.9 L1-L4. Is there any reason you know of, why such a disparity would occur between both hips?
    And what kind of pull back can I expect from a score like this, best case scenario. Not happy about taking drugs, but not sure I’ve much option. Been told to do no exercise except walk until I see a specialist. I’m pretty active and not sure that as wrapping myself in cotton wool will be good for me either. Any advice is very welcome.

Without a reason, injury, surgery physical disability there is no reason for the discrepancy other than error, machine or operator. Osteoporosis is a systemic disease. We also will switch people to REMS when available. This seems to be more consistent even when repeat DEXA shows the same discrepancy.

    I'm 76, lumpectomy for breast cancer 1/23, DEXA scan 2/9/23 with T score of -2.5, completed 6 wk of radiation 3/29/23, Rheumatology Clinic offered only pharmaceutical answers. This obviously impacts taking Tamoxifin or Aromatase inhibitors. I'm so frustrated. I've exercised for years, weight bearing exercises, walking 10,000 - 15,000 steps 5x/ wk, taken C and D3 for years, BMI is about 27.1. I'm looking at your video with a lot of hope. Hi Jayne,
      This is definitely a challenge. Tamoxifen effectively removes estrogen from cell signaling which reliably increases osteoclast function. This is a scenario in my patient population where matching pharmaceuticals may be an option if you can't hold back bone loss with natural methods. I wish there were a better way but you have to pick your biggest battle and right now it's probably breast cancer over bone health. We work with people in this situation and it's always a challenging conversation. There is hope but some options are limited.

      HI Doug. Bummer to hear about your UC. Steroids are a major risk factor in osteoporosis development for some. Celia, Crohn's and UC all can lead to significant gut dysfunction and immune dysfunction which will impact your bones. In addition prednisone directly causes bone loss. I suggest you reach out to our team at Optimal Human Health. www.optimalhumanhealth.com We run two companies in parallel and Optimal Human Health is the program that would directly address your UC in addition to your bones. Good news is prednisone induced osteoporosis can be reversed quickly (relatively) as long as we get to the root of the problem (your gut).

    Insurance won't cover most of this kind of medicine. Are functional medicine doctors actively fight for insurances to cover them?

    • @Dr_DougLucas
      @Dr_DougLucas  Рік тому

      HI Susan,
      Some may be fighting. Most are too busy treating patients.
      The issues with the insurance industry run deep. If we wanted to we could contract with insurance companies to get reimbursed for our services. The problem is they see our services worth pennies and no business can stay open with that level of reimbursement. That's why most of use a cash pay system.
      To evolve our system to medicine 3.0 it's going to take more than better insurance coverage. It's going to take a complete overhaul. I'm not optimistic that it's possible without a two tiered system of care. I think the level of care we provide will always require an investment from the patient. The question is what's the value to the patient? What's it worth? What's the cost of inaction?

      Hello Golnoosh,
      I can't give you direct recommendations but treating high urinary calcium with a potassium sparing diuretic seems very much like treating a symptom. That would have no impact on your bones but may make your labs look better. I'm not familiar with and wouldn't use this technique in my own practice. Instead I would recommend my patients explore WHY they are losing bone and correct those issues. Urinary calcium loss may be a sign of bone loss or secondary cause of bone loss but not necessarily and in general we have stopped using it because urinary calcium loss in the face of adequate calcium intake is NOT associated with bone loss.
      Consider watching our free master class and reaching out to our team for resources we can offer over international lines. I cannot treat patients internationally but we can direct people to resources available to them.

      @Dr Doug Lucas I've bern taking thyroid for 15 years. I'm now 61 and just had my first DEXA scan because I've developed near chronic back pain. (My husband insisted on it after he fractured two vertebra attempting to get out of the water on a slalom ski, basically water pressure, and after he asked for scan, discovered he is osteoporitic.) I knew I was at some risk because my mom and grandma both had OP, and I had earlyish menopause, but I also have done weight bearing exercise since forever, running or walking, plus weight training (though not as consistently,) and compared to my mom, I'm chubby. So I was shocked to learn my lumbar t-score was -4.6, and hip -3.3/3 5. I immediately jumped to the thyroid supplementation, but you say if "properly dosed" it shouldn't be an issue. Presumably I've been properly dosed based on yearly blood tests, but...almost as soon as I started taking it, I started having heart palpitations, which I now know are PVCs. I didn't want to say anything then because I felt so much better after months of feeling like I was walking under water, but it did make me wonder if there was a connection. Can a dose be too high yet not show as too high on blood work? I know, you're not an endocrinologist, and I should probably see one, but the one I contacted can't see me for like 6 months. 😞

      This is a very common scenario. At lease your doc is using newer drugs and not Fosamax. However, it does'nt sound like there has been a discussion of WHY you are losing bone, HOW to stop that bone loss and natural ways to improve your bone quality. There are likely alternatives to consider.

      @@Dr_DougLucas Thanks for taking the time doc. I appreciate that. No, he did not give me any ideas on how to try to improve my bone health. I do work out on a regular basis and do not have too many "bad" habits regarding eating or substance. My mom was on for while on Fosamax when that was the only option but she did not like it and stopped. She never had a break at all. My sister has pretty bad numbers on the bone scan but her health isn't as good as mine. I am not too worried about my bones but keep an eye on my future numbers and made some adjustments to my diet and my workout regimens.

    The only test they haven't done is the magnesium.

      That’s an easy on to do on your own. RBC Mag. Alternatively for those that don’t have it we know that most of us need supplementation. Question is how much.

    What about fasting???

    • @Dr_DougLucas
      @Dr_DougLucas  Рік тому +3

      Fasting is a great tool for many things. There is value for weight loss, longevity and more. However, if you push your fasting window to the required therapeutic dose for those mentioned items it becomes difficult to hit the primary goal of diet for bone health. Getting adequate protein requires more than 2 meals per day. So, you have to clearly define your goals and adjust accordingly. Very unique to each patient!

    I am on hormone blockers for treatment of breast cancer. I cannot do HRT. I am going to have to rely on supplements and exercise. I do not want a pharmaceutical solution.

      Hormone optimization is a risk/benefit conversation for everyone. It's a great tool when you can use it but for many the potential risks may prevent it as an option. There are many other tools to consider.

      @Dr Doug Lucas When I went on hormones briefly, not for bone health (though I knew there was some benefit to bones, they just weren't on my radar at that time,) but for energy/improved quality of life, and I think, more dollars for the guy who prescribed/administered them, I definitely had a wild ride. He utilized the patch for delivery of testosterone and estrogen, a cream for progesterone. I definitely could tell a difference in energy and strength, but also started growing more facial hair, had acne, was angry and thought about sex all the time (approximately 8 years postmenopausal.) Four or six months later, whenever the next patch came, he agreed to reduce testosterone, which left me with very lumpy breasts, back to normal energy, and my husband was way more irritating. The lumpy breasts made me nervous so I just said no to a third round.
      About two months after the patch was supposedly no longer delivering the product, I started to bleed, not heavily, but daily. I kept thinking surely it will stop on its own at some point, but it didn't, and finally I broke down, went to see an ob-gyn. (A naturopath had suggested hormones, was managing my hypothyroidism at that time.) It was incredibly awkward, particularly when the ob-gyn, who appeared about 14 years old, was literally clueless. Sure, she was young but this isn't something they teach as a possible side effect of HRT ? After a biopsy, she agreed I was probably right, the HRT had bumped me into more of a perimenopausal state, but 6 months of even light menstrual bleeding is...not great? 1 D&C later, which worked, I...wonder why that happened.
      Lots of things seem to happen to me, though, and that's part of why I'm not super excited to start another regimen of medical intervention, even though I am in pain pretty much 24/7, (though thankfully, it's mostly a dull ache that I barely notice if I'm sitting now.) *Yes this is the continuation of another comment, but just because there's so much I have been ignorant of, I'm throwing out my hormone story, in case it ... provokes a question/discussion for someone else. Too late for me to try to get THAT right.

      @@cindylutz7442 Thank you for sharing. Your story is not uncommon, unfortunately. Because hormone replacement has been largely ignored by the traditional medical model the demand has been met by providers with various levels of training that exist outside of traditional care. This can be a good thing but with hormones I see a lot of what I would consider mismanagement. I can't give you direct advice but I can tell you based on your experience that your levels of testosterone were probably too high and your estrogen was not adequately balanced by the progesterone cream resulting in hypertrophy of breast tissue and endometrial tissue leading to the D&C. So, failure to test and dose appropriately led to you stopping the hormones which could have had other benefits.
      Your thyroid question is more simple. You simply need a comprehensive test to evaluate blood levels on replacement. This can be done by endocrinology or primary care. It sounds like you would be a great candidate to work with us too. I previously commented on a prolia shot I was given without any info from my doctor and got a response that you were sorry I was not satisfied with my doctors style. Style? Style is how you decorate your house and yard, Style is how you dress, Style is something you can stop and change at any point, Style is not invasive, Style cannot dissolve or break your jaw and bones, Style does not stop your body cells from building more bone Style does not stay with you for years after you quit. STYLE?

      Hi Carole,
      Clearly that comment didn't sit well. Please remember we are all human and how we treat people can be described in many ways. I always consider medical practice an art and yes there is a "style" to how clinicians discuss management with patients. It's how I choose to describe it. I'm sorry you don't like it.

    Hi! My Dr recently diagnosed me with moderate osteoporosis -2.5 and discussed options for pharmaceuticals. A retired Dr. friend used the Frax risk assessment to determine if moderate cases required pharmaceuticals or if the practices you talked about in this video would help. I suspect I may have a gut health problem due to other medications I am on such as carbemazepine. I am frequently nauseous. I live in Canada and currently we have a shortage of family physicians so they seldom have the time/opportunity to really explore root cause other than vitamin D and calcium bloodwork along with regular bloodwork. You have given me some good suggestion Re parathyroid and exploring gut health. Any suggestions on how to approach my Dr to explore root cause, which is time consuming work she may not be paid for? She does care about me but our family physicians are stretched to the limit. Our national health care system is based on the 1960s and does not reflect the changes over the past years for family doctors due to aging populations and a great deal more administrative and “after-hours” work. Some provinces are experimenting with primary care teams but our province is not very progressive. I recognize my Dr works very hard and cares a great deal so how do I approach getting to root cause with her? I am learning so much from you.

      I’m glad you’re having a good experience with the channel! Unfortunately, exploring the root cause it’s not done in the traditional medical model, regardless of the country. If you haven’t already, considering joining our free master class, where we talk about all of the things that you are interested in.
