If the backrooms were actually 600 million square miles in surface area (referenced from like 3 google searches lol), peppino running mach 4 would take (VERY ROUGHLY) 27.1 hours to run from one end to another. It would most likely take us MONTHS or YEARS to do half of that, considering we can't just run through a wall as if it didnt exist (in the games he can just pile through a WALL OF MARBLE with no speed loss). I have no clue what's at the edge of the first level, but peppino would find out after a day and 3 hours.
