Cushing syndrome is a group of conditions caused by high levels of circulating cortisol or related corticosteroids.
A gout attack occurs suddenly, often at night. The affected joint is swollen, red and warm to touch. The initial attack usually involves only one joint, most often in the big toe, and lasts a few days. Subsequent flares may involve multiple joints and can last for weeks, with shorter intervals of remission in between.
Gout is caused by deposits of urate crystals in the joints, which may happen when blood levels of uric acid are too high. Uric acid is a waste product from the breakdown of purines, a major component of DNA and RNA. Because body’s cells are constantly renewed, uric acid is constantly produced and removed in urine. High serum urate level may result from decreased urate excretion, increased urate production, and/or overconsumption of purines.
The most common cause is the kidney’s decreased ability to excrete uric acid, which can be hereditary, or a result of kidney diseases, medications such as diuretics, alcohol use, or lead poisoning. Increased production of urate may occur in conditions with abnormally high cellular turnover or cell death, or in obesity, as urate production increases with greater body surface area. Overconsumption of purine-rich foods can contribute to higher urate level, but is rarely the only cause.
High concentration of urate promotes its precipitation and formation of crystals in joints, causing inflammation and pain. Because uric acid crystals form more easily in cooler temperatures, gout more often affects joints of the extremities. Urate crystals may also deposit under the skin around the joints, forming hard lumps, called tophi, that are not usually painful but can limit joint motions and lead to deformities. Joint damage caused by gout also increases the risk of developing osteoarthritis. In the kidneys, urate deposits may form stones, blocking urine flow.
Gout is more common in men. It often starts in middle age in men, and after menopause in women. Other risk factors include diets, obesity, other health conditions, family history, and surgery or trauma.
Diagnosis requires detection of urate crystals in synovial joint fluid. Because not all people with high serum urate levels develop gout, a blood test alone is not sufficient for diagnosis.
Acute gout attacks are treated with anti-inflammatory drugs, such as NSAIDs, regular use of which may also help prevent or reduce frequency of attacks. Because tophi can be dissolved by lowering serum urate, medications that block urate production or increase urate excretion may be used for this purpose.

Same. Had this for like 11 years on and off but just put up with it. It’s crippled me. Misdiagnoses, extreme pain, I am crippled. I can’t walk, sleep, work, do anything. I’m in excruciating pain. Doesn’t help my ankles are damaged as well. Don’t know if this undiagnosed gout has been a contributor as I have no arch and flat feet, too. I’m bewildered that no medical profession looked into gout first off and checked my dad’s medical history. He has it. It’s hereditary. I’m not on allopurinol and have been for 4 weeks. It’s not really doing anything. Also find out by researching it that I should have been on colchicine as well which no one told me. I’ve started taking this, too but it’s not working and I’m at my absolute worst right now. So I concur. I wouldn’t wish this on anyone.

@@chshaw87 So sorry to hear what you're going through. When you say you're crippled is it the same feeling that gout gives you and it's all the time? because when I have my gout episodes they only lasted a few days. And usual one day or 2 that is the worst where it is actually excruciating pain and it's only in my foot and mainly my big toe where my bunyion is.
It may sound silly but you're also supposed to drink one glass of cherry juice every day when you're a gout patient of patient when you have pain or not. So do you have pain all the time or is it when you're having an episode episode?

@@yobabybubba if a flare up occurs, it will come on gradually and can last just shy of a week. so for example this came on last week and the pain has finally gone today. it doesn't help that an MRI has shown I have done damage to my ankles. I have gout for a few years, I want to comment on how I have ""controlled" it" In addition to taking Allopurinol I have a diet, Ripe banana. (Not Banana) boiled well, then smoothie with strawberries and blueberries. Some Yogurts, (Not all) Meat, Yes meat, but Goat, Also Rabbit. and Tilapia Fish There is a Rice called Basmati. (Low GI), Crab too but from king crab. Tumeric helps too. Flour tortillas a little. Eggs Skim milk..,Some vegetables but not all. Avocado, Cilantro, Celery, Carrots, Zucchini, Eggplant, Taro, Nopal, A little pinto beans and a certain type of lentils. Potatoes (Not fried) If you like them fried, use an "air Frier" Good luck and Blessings!

In my case i have it in both knees and feet. Attacks would start from anywhere around a foot or a knee, sometimes even at the back of a knee. I was a non-meat eater (red meat and poultry) but i ate a lot of fish, so that may be the reason i developed gout. In the first five years only one knee or one foot could be in a flare up and would last from 3 days to a week. But then i started eating meats and poultry and the worst attacks came. Both my knees and feet are swelling and there is no way i can stand let alone walk to the bathroom, and attack duration is longer. The longest one i experienced was 2 months. I eliminated the meats again from my diet for good and im back to one knee or one foot per flare up. Having all four body parts swelling at the same time is the worst unpleasant thing to ever experience with gout. I literally walked with my hands under my butt to the bathroom and almost ate only rice with salt or sugar or any fruit in the fear that It might get even worse

I truly believe that since I stop exercising since I drive a semi, this gout creep up on me. On diet, they say drink lot's of cherry juice to flush your system if it hits you. I think it's a liver issue and diabetes issues starting up.

      Michael Myers,
It is my understanding that uric acid is eliminated from the body through urine so everyone has uric acid in their urine. If your body has a high concentration of it and it is not eliminated quickly, the buildup crystalizes in joints and causes the attack. Perhaps you have a concentration that is within normal limits. When the labs came back from my first attack (one of the worst attacks) the level was "borderline." I would not expect so much pain from a borderline level! I sure hope you don't gave gout, its horrible...

But the remaining fluid is more concentrated, which can increase the risk that you'll develop the crystals that cause gout. Some types of diuretics also reduce the kidneys' excretion of urate, a component of uric acid."
