    Writing a paper for NP school on what the most interesting part of medicine is to me, I have been quoting you regarding rotator cuff repairs! Thank you for helping me so much through school.

    This is helpful-I have been around the block with a shoulder fracture, osteonecrosis, frozen shoulder, cuff repair, another frozen shoulder, and finally a reverse replacement on my dominant arm. The no -dom arm suffered a dog pull about four years ago and the second round of PRP injections have done little. I had several second opinions and a couple of them indicate that surgery for AC joint arthritis, bone spur under the acromion, and biceps tendinitis would be the next step. Needless to say, I am reluctant to jump off that high dive, but preventing a rotator cuff tear seems to be a good idea. Good food for thought here!

    🙏 Thank you so much for this reality check video. You know, when you've been injured, you often get your hops high and kind a dream that all will get fixe, while that's not always the case... I know what I'm talking about... so to hear it the way you just explained it, bring one back to reality, if I may say.

    Thanks for these videos. I'm 7 months post op Rotator Cuff surgery and doing well thanks to you.
    The best advice I can give is be patient - it WILL get better. Do your PT - even at home. Before surgery get an ice machine that wraps around your shoulder. Rather than a using a recliner to sleep in, get a raising platform for your mattress that raises your head and feet. All mattress stores sell them. They make them now that fit in your existing bed frame and your mattress fits on top of it. You may be able to rent one but I bought mine to use well after I am healed.
    Remember, if you rest, you rust! Qi Gong videos on UA-cam help me tremendously!

    Love your videos I wish you were my surgeon. I'm almost 64 years old. I was at the top of my game last summer feeling strong, feeling really good healthwise until I fell down the stairs on my hand and then my left shoulder in October. X-ray looks good nothing broken. MRI in November said, full retracted tear of the supraspinatus and a SLAP tear. First surgeon said I would not perform surgery on Mike because of the tough recovery. The pain was starting to go away, and my mobility has been very good. Second surgeon in January said I would do arthroscopic surgery and use the Stryker in step device, which he called a pillow if I couldn't pull the tendon back all the way to the bone. He said it was retracted 4.5 cm. Saw another surgeon about two months later and he said I would have to do open surgery to get behind the muscle and there's no way you could tell by the MRI how much it was retracted because of the angle that its viewed. I went back to the second surgeon and after recommending surgery the first time around he says well it's up to you. What do you want to do. And I'm trying to figure out what I should do because I really don't have any pain unless I move a certain way, and my arm has been getting stronger. But I'm so stressed out about this because people in a Facebook forum are telling me it's not gonna get any better and then you'll reach a point where you will need a reverse shoulder surgery. So right now obviously I can't do push-ups, but I can do pull-ups and curls with weights and other exercises and it's not hindering my work at all. I'd say I have 95% range of motion I can even reach around my back and the pain has subsided and like I said it's getting better every day. I've been doing some physical therapy. and the physical therapist said "no way would I get surgery. I've been doing this for 35 years". I did tear the same rotator cuff slipping down a grass hill in 2017 and that healed on its own. What are your thoughts? THANK YOU!!!!

      @mikee535, that is indeed a tough one. I am going to link to a video I did on RCT and muscle atrophy. That is often how I decide if a RC is fixable. If you are not having much pain or dysfunction it is certainly hard to recommend surgery. BTW the pillow is call InSpace. I have a video on that as well. I will link to them in the next reply. Good luck and let us know how you are doing.

      @@rotatorcuffexpert thank you so much, Doc. The good news is after seeing three surgeons that gave me three different opinions. None of them being shoulder experts. I finally found a highly acclaimed shoulder specialist here in New Jersey that takes my insurance. Dr. Francis Alberta from Paramus. Going to see him July 29 and I have very specific notes questions to ask him. Thank you so much.

      @@rotatorcuffexpert yes I've seen your grading system video. Judging by my MRI from November 2023 it is not infiltrated with fat. Not sure how long the tendon is. I wonder how outcomes have been with that balloon technique. Stryker in step. Thank you so much for your help.

    My right LHBT is causing a fair amount of pain when swimming. Shots don't seem to help for very long. I know friends who had LHBT tenodesis and the recovery is forever. About 10 years ago, my left LHBT detached on its own. I have no pain anymore and seemingly no consequences. Yet, articles suggest possible instability. Is there a clear path forward in this situation?

    Thank you for your very informative videos! I have a question: According to an MRI, I (m,45) have a small, full-thickness tear in my right supraspinatus tendon, as well as a SLAP II lesion. I think this lesions came fro years of boxing and other sports. I only have pain after intensive training (e.g. swimming), but the pain goes away after a few hours. The pain radiates into the arm and hand and comes from deep in the shoulder joint. I have no pain at night, no restriction of mobility and no loss of strength. I have had problems with my shoulder for a few years now. The doctor advises me to have surgery, he says the tear in the tendon has a 50% chance of getting bigger and my pain could get worse, also the defect is still easily repairable now, this could be different in a few years. He says I am too young to live with a full thickness tear, even if it is small, because I do sports and use my arm a lot. Do you advise young athletic patients with full thickness tears and SLAP lesions to have surgery as soon as possible regardless of the level of severity of the symptoms?

    • @rotatorcuffexpert
      @rotatorcuffexpert  2 місяці тому

      Hey Andrew great question. This is one of those situations where there is no right answer. You could wait and then have an irrepairable tear later on or you may be fine for years and then have increased pain with a rotator cuff that is still repairable then. I would have a good discussion with your surgeon and maybe even get a second opinion. Good luck and let us know what you decide

    I would love if you could post a video explaining partial-thickness intrasubstance supraspinatus tears.
    I have a small partial-thickness intrasubstance tear of my rotator cuff in both shoulders. I am so confused on whether or not it will heal. I am doing physical therapy and even after 2 months still so much pain. It’s only 4mm by 3mm by 2mm. I am now debating on PRP treatment. Would love so more information on whether or not this type of tear will heal on its own. To tears repair themselves and close?? Will the tear always show up on an ultrasound even years later as scar tissue?
    Thanks so much for all of your videos!

      I had surgery last Thursday and when the nerve block wore off it was extremely painful. It gets less and less painful by the day but that first night was really hard. Just keep up the pain meds

      I had a full shoulder replacement in Dec of 2022, and it was my 1st surgery ever. I thought I was going to die from all the pain that I endured. Sleeping was the worst. I couldn't get into any comfortable position to sleep. I used multiple pillows to try and makeshift, it helped some. The key is time and patience. The medicines for pain, stopped working after a while, even the anti-inflammatory. The body becomes resistant to them. Gotta ice it down. Wished I'd considered the ice apparatus, but I didn't, despite the controversy surrounding it, meaning some swear that machine aided in a faster recovery. I did physical therapy for 6 weeks, and that helped tremendously. I don't know that my healing process would've gone as well as it did, had I not done PT. Nearing 2 years post-op, I'm at 90% of being able to do the things that I was able to do, prior to my shoulder injury. So, hang in there; y'all have got this. 💪 💪
