There is increased signal seen in the anterior fibers of the biceps femoris tendon at the insertion on the fibular head partial-thickness tearing. I was injured years ago doing ladder drills with our Vol Fire Company. It happened when practicing stepping up/down with all weight plus gear & hose on one leg while wrapping the other leg in & through one ring. I felt the lateral ligament give way. Never had it examined or x-rayed, just wrapped & to allow it to heal. And the turn-out gear was stiff and acted as a support to that ligament when having to kneel. But now, 20 years later, I re-injured it.... Just wanted to explain the scenario since it did not occur from typical tramaor sports injury, only excessive weight.
If your ACL is not functioning properly or gone, your LCL/FCL will be more prone to tearing as it has to work a lot harder to stop tibia and femur from moving too much appart (varus forces). Had an ACL injury 2 years ago which i only rehabed, so muscles and other ligaments are compensating. No longer able to play football as the LCL/FCL keeps popping out constantly when i break, twist and change direction towards the outside of my knee.
Nice job. I suspect a grade 1 LCL injury left knee- no fall but “stepped off” a high stool onto left foot ( had ACL complete tear & repair rt knee in1987; since then have considered my left leg the “stronger one.” Trying conservative rx first, if still problematic in ~6 weeks will see my orthopod. R/ monica
I have had both of these tears or both of my legs. One was when my teammate crashed into the inside of my right knee during soccer. It took about 2 years to heal. A few months later, an opposition player fell into the outside of my left knee. It’s been 4 years since and I still have the same symptoms. A loud pop in my knee, swelling, pain, and inability to stand on my knee without pain. 2 weeks and I get back to playing again. Rinse and repeat. I will eventually get surgery.

I'm an active 68 year old. I twisted my leg in a bike accident - hit a pothole. The bike fell to the left and with my left foot planted, my left hip twisted when my torso turned towards the left. Massive swelling without bruising. No pain when palpated. 30cc fluid removed from the knee with a steroid injection initially and another at 4 months. LCL and IT band both stretched and twisted it seems. That was 8 months ago. PT and quad strengthening have helped. Compression intermittently helpful. I walk without gait disturbance but ascending and descending stairs is difficult when alternating legs. No MRI. It seems like something else could help but I can't find an answer or a practitioner to help me. Suggestions?

UPDATE: While seeing a chiro for a diff problem, he examined my knee. He suggested deep massage of the lateral quad of the effected knee. It has helped so much. Stairs are only a minimal challenge now.
Hi Farah. When I use the term pathophysiology I’m referring to the general mechanical forces or physiological response to those forces. In the instance of lateral collateral ligament injuries, pathophysiology would be a virus force or load to the knee that exceeds the strength of the lateral collateral ligament. When I refer to history, it is more of a specific mechanism or example of the pathophysiology, such as a soccer player that was tackled and struck from the inside of the knee. Great video. Even a MRI and varus stress test aren’t enough for knowing the LCL injury (partial for example). The best option is using dynamic ultrasound OR better, DMX. I don’t agree with the term of mm opening for determining the structural damage as most are for the general population but does not take in account lax people. Apart from that, it’s great documentary explanation. -- Edit: NASIDs are the worst thing a caregiver can offer as it stops the natural healing process (e.g. - natural healing of the ligament)
Hi! I do have LCL grade1. What mean Rest? Should I completely immobilise my joint and walk with crutches or sit home and do home routine walking? If first, how many days? Then I will get muscle wastage and dissaper of 180* leg extension. What step next: muscle tonus or extension 180*? How can i Know that walking without crutches is ALREADY not damaging my LCL?

In general, “rest” is referring to avoidance of physical activity, aside from activities of daily living. You are correct that complete rest and immobilization would result in joint stiffness and muscle atrophy. Physical therapy is important to maintain range of motion and muscle strength. I, personally, do not completely immobilize grade 1 LCL injuries. I felt my knee seperate horizontally before falling. 1. Can that be considered a "pop"? 2. Can a grade I or II tear cause significant instability? 3. Does a grade III usually present with significant pain?

To answer the questions in general terms...there are several conditions and injuries in the knee that can cause any number of mechanical symptoms in the knee and can be described with many potential terms such as pop, snap, click, clunk, etc. A grade III LCL tear does result in instability to varus stresses to the knee. In many cases of ligament rupture in the knee there is acute pain at the onset, which can be variable in severity/intensity from person to person, but the pain may subside considerably or completely after a few days to weeks. I injured my knee 2 months ago with a direct blow to the medial aspect of my knee, I can walk, and carry some things, but I just wonder why my quads,hamstring is constricted? Im having occasional knee pain with it most of it when i walk long distance.

I injured both of my legs for running 13.5 miles smh.. Never again!! Recovered 95% in 4months. Self diagnosed and self treated. However, I need to see a Doc cause I have pain in the cold weather.

I have injured the medial ligament over 100 times ( stopped counting) ! I have no acl sense high school football 1972 an intentional blow to the outside of my left knee from behind to take me out of a 2nd in the league winning season ( 1st string linebacker with the most un-assisted tackles ) ! it was discovered during medial meniscectomy in 1976 in the Army Infantry ( EIB ) after bending it backwards near 90 degrees at night stepping into a golfer hole up to my knee totally over loaded with equipment I am certified timber cruiser grader / certified wildland fire fighter ( forest service ) / surveyor crew /head chain man ( they probably don't have those anymore private ) I work all summer with my knee swollen like a basketball and healed in the winter off season !I loved working in the woods !!! but it got so painful that I couldn't be nice to people ! haven't worked sense 88 now I have a military pension but didn't get that till 20 years later ! they wouldn't fix it and wouldn't say it was disabling ( yet any lying scumbag could get disability for any stupid reason ) ! the military gave me 30% but refused me 100% (couldn't deal with pain and ass holes at the same time so they would fire me )!

Is it true that if you hear a pop it is a complete tear, ie. a Grade 3? Or can it be a Grade 1 or 2? I'm hoping. And people with poor connective tissue (such as EDS ) can tear by just stretching-not kidding. This is a clear and concise presentation. Thank you.
Thanks for viewing and the comment! I wouldn't say that hearing a pop would indicate a full-thickness tear 100% of the time, but that is an important piece of information in the history of injury. It would certainly need appropriate medical evaluation and imaging to determine the extent of injury. You are correct. For someone with a connective tissue disorder, it is much easier to sustain a ligamentous or tendon injury, and it is also more difficult to treat. Thanks, again, and take care!
Thank you for your quick reply. I was hoping you wouldn't say that. I have EDS and I have been having problems with that knee for some months. I heard a loud pop after stretching and now that knee is much looser on the LCL than the other knee. Is there any other symptom that says definitely Grade 3 tear bc it is going to create a fuss. The PCP (here in Iceland) is screening for COVID and there is no appt system. I already need a wheelchair to get around and I need a carer to push me. I want to avoid surgery if I can. TIA
So sorry that you are having issues with your knee. There are several potential causes of lateral knee pain that can involve a pop or multiple pops, an LCL injury only being one. It would be best to have a local qualified medical professional evaluate the injury. Best wishes for a speedy recovery.

Is it true that if you hear a pop it is a complete tear, ie. a Grade 3? Or can it be a Grade 1 or 2? I'm hoping.
And people with poor connective tissue (such as EDS ) can tear by just stretching-not kidding.

Thanks for viewing and the comment! I wouldn't say that hearing a pop would indicate a full-thickness tear 100% of the time, but that is an important piece of information in the history of injury. It would certainly need appropriate medical evaluation and imaging to determine the extent of injury.
You are correct. For someone with a connective tissue disorder, it is much easier to sustain a ligamentous or tendon injury, and it is also more difficult to treat.

I was hoping you wouldn't say that. I have EDS and I have been having problems with that knee for some months.
I heard a loud pop after stretching and now that knee is much looser on the LCL than the other knee.
Is there any other symptom that says definitely Grade 3 tear bc it is going to create a fuss.
The PCP (here in Iceland) is screening for COVID and there is no appt system.
I already need a wheelchair to get around and I need a carer to push me.
I want to avoid surgery if I can.

So sorry that you are having issues with your knee. There are several potential causes of lateral knee pain that can involve a pop or multiple pops, an LCL injury only being one. It would be best to have a local qualified medical professional evaluate the injury. Best wishes for a speedy recovery.
