If I remember what I read correctly, when the author of Ghost Rider decided to include the Caretaker in the story line, they based the look of him off Sam Elliott. So, when they went to make the movie, they decided to ask Sam if he wanted the part since he was the inspiration for the look.
He was the Ghost Rider before Johnny Blaze became the latest Ghost Rider. I think the Ghost Rider is passed on, that is how I see it, but maybe I am wrong.
