    A women in Italy was dying from Covid..He family came to say goodbye..Husband asked if anything else could be done..Doctor said I'm gonna turn her on her stomach but her heart could stop..Family said do it she's dying...They turned her and she started to get better..So they continued to turn her to get pressure off her lungs...12 days later she was released and is doing fine.

    "I'm arguing for evidence-based medicine..." You're asking for trials. It doesn't take an MD to understand that you have to practice some FIELD HOSPITAL medicine because you've never seen this crap before. Your insistence on clinical trials to support the decision to treat a patient outside of conventional norms is insane. While you insist upon peer-reviewed double blind studies to support a treatment, docs like Vladimir Zelenko are saving hundreds of lives by intervening early with a protocol that doesn't need to wait on peer review.

      my understanding there is NO VIRUS, see Dr.Kaufman and others researching the raw data, at least none isolated therefore no effective testing for a virus. something is causing low O2 levels in blood and someone knows exactly what it is. these doctors are the heros we need, looking outside protocol to save lives

    I personally would not want to be intubated. That's why I stay home. Blessings to all the doctors who are making decisions in very uncertain waters. The nurses and the doctors and all the front line people are the true Superheroes of 2020.

      Oxygen therapy, according to what I've read from doctors across the country, is one of the first treatments for patients of covid-19. The problem is that the O2 saturation levels continue to remain low for some patients, which is why the next step is usually intubation on a ventilator. Some of the questions being asked by these doctors however is if the typical protocols, including pressure settings, are too much for the patients body to handle, which in turn could lead to damage of the lungs or death. It sounds easy to adjust on the fly, adjust settings per patient, but with so many patients doctors rely on established protocols to treat all patients the same (more or less) if they are dealing with the same disease. The biggest problem is that we don't know much about the virus or the disease and there is not much time to figure it out, especially as one doctor put it... By the people on the front lines caring for patients, it needs to be researchers and there needs to be clinical studies done. Time is of the essence. Pandemics have ravaged humanity before, and it is happening now. I pray that we have learned from past pandemics and will come together to defeat this, but so much of our world is so divided. So many countries looking to blame another country, cutting off trade including PPE supplies and medication and so much division within the United States.

    I've heard that ECMO has been more effective than ventilation in some cases. Probably because it mainly focuses on oxygen saturation of the blood, while simultaneously taking stress off of the heart and lungs, which are the organs most affected by this disease.

      I was wondering that too but with problems they are having just getting ventilators and basic supplies they may not have enough resources. It seems ECMO may be underutilized even under normal conditions, so unfortunately I doubt it's being used for very many if any patients (I hope I am wrong about this) even if it could help.

      ECMO is going to be in even more short supply than ventilators. A June 2019 article I found on the Net ( "Miracle medical machine ECMO makes heroic rescues, but leaves patients in limbo " USA Today Melissa BaileyKaiser Health News) says only 264 hospitals have ECMO machines. The sitation, I have read elsewhere in the UK is even worse, only 15 machines in the nation.

      These are brilliant doctors that do not understand this disease. Whoever created the coronavirus is still in the shadows. And whoever it was is also very brilliant. I would worry greatly because I believe that something much more powerful is coming. Most doctors understand the make up of DNA and cell binding and separating yet they do not understand what they are looking at. This makes me feel even more afraid. I have had a slight case of atypical pneumonia. I was so afraid and sick because I couldn't breathe, my throat was tight and I couldn't catch my breath. And this was just atypical pneumonia which could be treated with antibiotics. I did recover luckily. COVD19 sound much more complex. I am very afraid

    I think this shows that many doctors are seeing things that aren’t making sense to them. Makes me think the pathology hasn’t been completely discovered yet. Can there be a problem with the RBCs not being able to carry the needed oxygen to the body’s cells and not just the pathology in the lungs?

    So can a rapid diagnosis of either Type 'H' or Type 'L' lung distress be determined using ultrasound to know whether it's ARDS related and thus determine the treatment type? It seems as though some doctors might be sold on the idea of intubation as a default course of action? Whereas these doctors seem to be questioning all possible avenues of treatment protocols. Which is their job, so good on them for asking difficult questions.

      It looked as if they were using lung xrays. I'm also interested in knowing what the quickest and most accurate way of verifying which kind of presentation there is of the lungs. I think the bureaucratic Dr. who accepts 30% living through intubation is being stubborn. He doesn't want to be sued and is being overly cynical. If it's possible to do better then we must.

    These Doctors are practicing the art of Medicine. This virus is new, they need to have the time to find out what it`s all about. I know people are dying but, these guys are doing the best they can. They will find out about this virus and will save lives. Kudos to all doctors,nurses and all hospital employees.

    I would like to see what the effect of proning is for at-home patients. I was sick with covid-like symptoms a month ago. It felt like but lungs were full that I couldn’t clear because I could only produce a dry cough. I had heard that the Chinese were proning patients and doing tai chi with them, and I decided to try a version of this. I would go on a slow walk for 30 minutes and then would lie down on my stomach for several hours. Even if I didn’t sleep, lying on my stomach (and sleeping on my stomach) seemed to help more than anything. I have asthma, and I think that lying on my stomach and forcing myself to do mild exercise (even though it hurt) made a great difference. I don’t know why this is not being recommended for patients with breathing difficulties/shortness of breath who are at home. It seems too simple and unsophisticated to be true. But lying on my stomach worked like magic.

      ive been noticing my lungs hurt a little becuz a year ago i had PERICARDITIS PLEURAL effusions.. which took 9 months to heal but now that im.considered a high risk..i am using oxygen when my lungs feel low oxy is at 94...thats not bad..
      i dont have covid..thank GOD.
      but i keepin watchful prayers over me..
      BLESSING to our nation to heal fast..

    Could it be possible that categorizing the patient as positive for the coronavirus after a test and using that as a determining factor in diagnosis and treatment be a contributing factor to the confusion?
    What if the coronavirus test result would be set aside - keep it out of the picture - and diagnosis and treatment for a patient proceed as it normally would? Would that make a difference?
    Could the coronavirus be the virus in the way a patient is diagnosed and treated?

    The depth of commitment to providing the best possible treatment to their patients is awesome! It has been a long standing discussion in Medical treatment that one treatment does not fit all.
    I applaud the courage of these doctors for looking closer at the prescribed treatment protocols. Because of their commitment we will provide a more diversified set of protocols to treat COVID 19.

    So far the whole of Africa and India have reported less cases than the whole of America, is the hospital environment actually killing more people? mMost deaths have occurred in the hospital environment. The recovery rates are high in Africa as a whole and those who self isolate at home have a higher rate of recovery? Something odd is happening

      If you look at the death rates for the U.S. v. other countries, it started out lower but is now higher: 1.5% v. 6%. Is this because the U.S. is treating it agressively, getting people on ventilators early? Maybe the "give me lots of ventilators" [thank you, Gov. Cuomo] one-size-fits-all medical movement is not right? I'd love to hear Dr. Fauci on this topic!

      India is virtually not even testing their people yet. You have to remember that the numbers you see are REPORTED numbers, not actual real life numbers. If you think countries like China, Russia and Korea are reporting their real numbers then you are mistaken. I've been to S Korea, a huge portion of the population leave in skyscraper type of buildings bcs they have so many people in such a tiny country. And there is rampant poverty there, outside of Seoul and Busan.

      Exactly. I was a respiratory technician back in the 70s and worked with a lot of ventilator patients. Also had a lot more COPD patients and they had breathing machine treatments with a face mask, a certain O2 level and a bronchodilator that reduced the surface tension of the mucous buildup in the alveoli and eased expectoration of it. I'm hearing that the severe COVID patients have stiff lungs when breathing and they are put on the vents. It's been 40 years since I was in that arena.

      Hemoglobin they say it starts to accumulate the ferrin and messes with your iron so as the hemoglobin won’t bind to the oxygen

      Larry I am a bit concerned, never in the past so many Ventilators have been used for patients that are suffering from an unknown disease like this. What if the ventilators are the reason for these deaths. It's almost like blowing in the soap water. Someone needs to investigate into this matter. Rather than forcing a pipe, maybe a mask system should be developed where the patient can stay conscious.

      Agree, he is very serious and compassionate. As a newly retired nurse, I didn’t truly appreciate the struggle the docs were going through. Unfortunately, in this lawsuit crazy world, it’s hard to speak up. The one doc was pushing pretty hard to stay with the plan. I like the comment by someone on this thread about treating this like a field hospital. Of course I am writing this after the fact, but this peer review stuff in the event of a crisis causes people to not question. This video is very refreshing to see different opinions being recognized.       It's not really arrogance. In medicine, the reluctance to embrace new evidence because it contradicts norms is called a Semmelweis response/reflex. Its a type of medical bias that all doctors are aware of. I think you have to understand it from this context. Most of what they're seeing is completely new to them, these are not the typical ADRS symptoms but doctors will err on the side of caution and follow the existing protocols simply because there is no alternative. Caution is very much what we need because these are observations by some doctors. There needs to be a dialogue between the medical community before protocols can be changed. You don't just suddenly shift something without further research.

    A friend of mine believes he had covid 19 about 5 weeks ago. He's a 57 yo. healthy male. It was different than the usual flu in that there was more dizziness and complete loss of balance. If he went from sitting up in bed to lying down, the room would be spinning for half a minute. These symptoms went away over the next 1-2 weeks. He never had shortness of breath.

    My grandpa died because of corona. He started with pneumonia and the doctors wanted him to stay in the hospital but as stubborn as he is he didn’t want to be admitted. I wish the doctors tested him and made him wait before letting at least go home. He started get worse. He developed double pneumonia,a lung collapsed,he needed a ventilator,he couldn’t get him blood pressure up without medicine and the worst part is he had to die alone. So please stay home if you can. It’s very important to protect yourself and others.

      my mom died. her oxygen was 65%. doctors made my brother ( who brought her to hospital) panic and fear that below 90% oxygen it can be dangerous and they have ti put her on ventilator or else organs can be damaged. he immediately signed and gave green signal to doctors to put her on ventilator. and she died. i am so sad and devastated... ! even if my sis in law had warned me..to tell my brother to discuss with doctors and NOT put her on ventilator ! should my brother have insisted to doctor to put her only on manual oxygen therapy ( nasal cannule and face mask oxygen) ? would oxygen therapy through nasal cannule have been enough for her breathe to become better ? or 65% is so less that doctors HAD TO PUT HER ON VENTILATOR ... i am so sad and shocked and looking for answers.

    I thought the intubation takes place when the patient can no longer breathe on their own. I am a very shallow breather. Clinic aides get worried when I come in and make me take deep breaths. When I can reach what they consider normal then that is what they write and then they leave me alone. But I can be at 81-71 and feel fine. Maybe if a person can talk ok with no problem then the treatment needs reevaluating.

      People are far too sanguine about the use of mechanical ventilation and the massive sedation it requires. There are horrifyingly high levels of injury and long term detrimental impacts both physiological and psychological which worsen each day someone is on ventilation. There can be organ damage. Approximately 1/3 of patients suffer brain injury, irreparable cognitive detriment, delirium, PTSD-like symptoms, long term depression, etc.
      If it is at all possible to avoid mechanical ventilation and still preserve the life and likelihood of recovery, then they should be avoided.
      Why aren't all patients in prone position? This is now the normal protocol in Italy. It has been proven to facilitate breathing, both natural and on ventilation.

      At my former hospital ( just retired), they are definitely doing everything to keep patients off the ventilator if possible. Everyone gets proned.

    The public is far too uninformed and sanguine about the use of mechanical ventilation and the massive sedation it requires. There are horrifyingly high levels of injury and long term detrimental impacts both physiological and psychological which worsen each day someone is on ventilation. There can be organ damage, permanent lung damage as a result of the pressure on the lungs. The high oxygen levels also can cause serious damage. Approximately 1/3 of patients on ventilators suffer brain injury, irreparable cognitive detriment, delirium, PTSD-like symptoms, long term depression, etc.
    If it is at all possible to avoid mechanical ventilation and still preserve the life and likelihood of recovery, then they should be avoided.
    Why aren't all patients in prone position? This is now the normal protocol in Italy. It has been proven to facilitate breathing, both natural and on ventilation.
    What about the use of ECMO? Even with its risks the Chinese found it gave better results with the sickest patients. These machines are expensive and unfortunately too rare. Why is there no broad discussion on the use of ECMO?

      evidence based medicine tends to be designed for "average" patients with typical, specific, disease/condition presentations. Long before Covid 19, evidence based medicine often failed patients with atypical,complex &/or co morbid conditions.It's not at all surprising that Covid is confounding rigid evidenced based protocols. I hope these doctors not only make it through this horrendous time but come away from it in a way that allows them to expand this original thinking approach beyond pandemics into main stream medicine.Evidence is vital but it should serve as a guideline not holy doctrine.

      This fear mongering video uses local anecdotes to highlight one specific aspect. Yet as the data comes in it's showing more and more the models were wrong.

    Those same discussions went on a generation ago regarding ARDS, the syndrome is not homogeneous. The debate regarding invasive care vs a trial of conservative care has gone on in many ICU situations, and the risk of delay is often balanced by treatment associated toxicities on the aggressive side. It wouldn't be the first time we would conclude that less is more. Also bear in mind that lead time bias can dramatically shift results, so comparisons between institutions needs to bear that in mind. Results often say more about inclusion criteria than treatment efficacy.

      Also the public is not aware of the dangers of ventilation and sedation to the patients, as part of the equation. The patient may exit the ICU with terrible injuries and deficits.

Probably because we are no longer a 1st world country (or even a real world country for that matter) ,when even basic supplies are unavailable state of the art treatment is probably not even on the radar. ECMO was used here in 2009 for H1N1 patients, but that was before our country signed it's own death warrant on November 8,2016.

    "Can you put a patient with a pneumonia like symptoms on a ventilator" given that if his/her lung is full of fluids. I have been asking that to myself. I remember how a doctor drained the fluid from my dads lung and gave him the chance to breathe properly and this happened 10 years ago. If the intention is to let the patient breathe properly and increase the O2 saturation then should we be looking into the condition of the lung as well, a constant monitoring and making sure there is no fluid buildup? Also staying still in bed and not being able to move atall can lead to more fluid buildup in the lung due to lack of movement and no sweating?
    Maybe I am thinking wrong.

      These patients don't just needs O2, their lungs is so full of fluid its basicly turns into a wet stiff sponge and they laterally cannot breath properly, they need to have O2 force into their lungs, the question is about what setting to be used and what other things is required to do like draining the fluid or lying on the stomach to help clear fluid.

      It's not loose liquid, or they could hang them by their heels and drain it out. Covid-19 produces an extraordinarily viscous mucous as part of the cell breakdown and immune response that coats the lungs alveoli preventing oxygen and CO2 membranous exchange. The surface area available for exchange is progressively reduced, decreasing oxygen levels, affecting brain and organ function, especially the heart. Also as the lungs stiffen from a gossamer softness to cardboard, they become harder to compress and expand. Breathing becomes labored, eventually exhausting the patient.
      The mechanical respirator forces air into the lungs with a higher percentage of oxygen to compensate for the reduced exchange surface area.
      The other option is an ECMO. You can look that up.
