Thank You! Finally an explanation of what I am facing! I am probably at the point of needing surgery to fuse the ankle bones. I appreciate the complete description of the stages, causes and ultimate treatment.

Hi Doctor...I was diagnosed with stage 4 PTTD. I was born with flat feet and had to wear special shoes all of my life to correct them, which never took place. I am now over weight and was having problems with my right foot because of the PTTD. Would you inform your patient to have reconstructive foot surgery to create an arch?

Is it possible for Posterior Tibial Tendon Disease to be mistaken for Plantar Fasciitis? Or that it is a symptom of PTTD?

I like this video, Doc. Very informative. I'm currently in a boot for posterior tibial tendon strain and I'll be getting my orthotics next week. I also recently went back to school for my second degree. My major is nursing. I'm using this video for a project that I have to do for one of my classes. I do have one concern. I'm just wondering if my condition will disrupt my career goals of becoming an RN. I know nurses do a lot of standing and walking, so I need someone to just keep it real with me, doc. Should I look into another career path? I currently have a degree in psychology, but I've always wanted to be in the medical field.

What is the specific name of the brace for PTT and it will fit over my running/walking shoe correct? I stand on my feet 8-10 hours a day and can’t seem to find relief in a shoe.

should FDL transfer be the operation of last resort? What the recovery time from this surgery and can one return to marathon running after such a surgery?

I have been diagnosed with PTTD and I have done PT but I haven't been good about doing the exercises at home. I wore some ankle boots one night and now my calf all the way down to the bottom of my left foot is hurting. I started doing my P.T. exercises with bands at home. I hope this condition can improve with the use of my orthodics and the exercises. My father had flat feet but never went through any of this.

Hello Doctor. I’ve had bilateral pttd for 5 months. The left foot is much worse. I’ve been doing band exercises and heel rises, but I’m not really much better. I wear custom orthotics and an air cast ankle brace. I just tried iontophoresis with Dexamethosone, but I don’t know if that is useful. Any advice? How do you know if the tendon has degenerated? An MRI done 3 months ago for my foot (different problem) showed that the tendon is intact with fluid around it. Another podiatrist did a quick in-office ultrasound, and said I have tenosynivitis. Should I be advocating for an MRI of the tendon?
    Thank you.

Can the surgery for PTTD be done on an outpatient basis?

Is this something that would cause the big toe to get "stuck"? Ive tried to explain this & people dont understand. Its extremely painful and feels like whatever runs along the top of the foot/big toe gets stuck and I cant move until the pain goes away. Its been happening for years and Id love to know what causes it.

Had a PT tear 5 years ago, not treated timely but have had injections and physio to no avail, ended up last week having the tendon transferred and a calcaneous shift performed, it is now in plaster and very painful, I have a plate and screws , is there any advice you can give on helping it to heal please. Thanks so much sir.

You are better off contacting your surgeon, as all cases are different and all recoveries are different.

Thanks. I will of course do that. I just wondered if there was anything specific I should do to speed the healing or how it is treated there. Here, I had the Op last week a cast was put on,I won't see anyone until next week when the cast comes off and the staples removed and possibly cast back on for another 2-4 weeks.nothing else. thanks Kim.

    Hi Doctor,
I am a hockey referee and in recent games I have found some intense pain on the inside of my foot/ankle (what seems to be this area of the foot you are discussing). It does not pain me to walk, run, or do everyday things but seemingly only when I am skating and the skate is putting pressure on that area of my foot/ankle. When I apply pressure myself with my fingers, it also causes pain. I am wondering if perhaps this PTTD is what I am experiencing and if so, what can I do for treatment?
    Thanks for your time.

You are better off having a podiatrist evaluate the foot, as there are a few structures in the inner side of the foot/ankle that can become irritated by a skate.

sadly these feet were diagnose 2years ago with advanced Posterior Tendon Dysfunction (TPTD) , tibialis posterior tenosynovitsi, and required
immediate treatment to help prevent tendon rupture. I got orthotics and it did improve but I can not walk bare foot for more than a moment. A lot of inflammation has actually gone but still I get pain. Is it possible to reverse the condition through exercises and if so do you have any suggestions...sad feet,,,,

Hi Doc. I was born with flat feet almost 60 years ago. Have been very active my entire life but the past year I believe my posterior tibialis is definately inflamed and is hindering my walking. My flat feet are pronated and I have worn orthotics for decades. I recently got a new pair from an orthopedic podiatrist. The worst pain is in the morning after I have not walked on my feet for a while. Any suggestions?

See your foot specialist again as he/she should be initiating active treatment. If it is the posterior tibial tendon, self-treatment is often unsuccessful.

Hey doc. Please reply to this ASAP. So I skateboard and I do tricks on mostly parking lot surfaces. I have been skating for two years now and skate almost every day, and a little over 6 months I noticed a slight pain when I walk but I said nothing of it only because it went away after 12 hours or so. But now I'm on the hockey team in my school and after intense practice. That same exact pain came back. I'm not specialist, I'm just some sixteen year old kid but I did my reaserch and I bet this is it. This week I had hockey practice and then the following day I went to the park to play basketball for 3 hours and the pain is very bad occasionally as in I walk up and down stairs slow and steps hurt. But this only last like 24 hours. The pain is gone now, but I bet this is it. Please reply and just tell me what u would do. Ik this is the one. I get pains deep in my ankle in that exact area. Thnnx doc

You should have your parents take you to a foot and ankle specialist for evaluation- many things can cause deep ankle pain.

I dont have diabetes or gout. It happens when Im sitting down or laying down way more than when Im on my feet. Its scary because all I know to do is wait for it to let go.
