Have seen more than 5 doctors who told me it’s nothing and go home. Thanks a lot
It’s so sad when doctors don’t listen to their patients. I’m grateful the podiatrist I went to was able to diagnose my PTTD right away, although his means of treatment weren’t the best, I still at least knew what I was dealing with.
Same i have been in some much pain for past 1 year
@saraa7117 same goes to me?😢
How are you feeling now, tips or something to get rid of this pain 😢
Samee! 6 months with terrible pain and nobody listened to me :(
@@mariiiaestevez how to get in touch with you? I’ll tell you some tips
I seem to have a chronic issue with this. Never seem to be able to shake it completetely.
agoforth59 me too , might need surgery
Me too. Two years now. Can I ask if you've had any progress with yours?
@@kateotokhs I've been using a brace which does help to walk without as much pain problem is if I walk around too much it just flares up and I usually pay for it that night, I've been using kinesology tape because I can put it where I need the most support only thing is its expensive to buy but I find it more comfortable to wear than the brace ...
@@SimoneMaree81 Same here. I've been using a pair of bespoke insoles which seem to work but they ruin my shoes as they're very hard. And just like you, they hurt when I walk a lot which I like to do. I've learned to live with the pain. I found some strengthening and stability exercises yesterday which I'm going to start doing. I did it yesterday and the effect was instantaneous.

What do you suggest for navicular extra bone attached to tibialis posterior ? I dont want to do surgery .
Hi guys - I was wondering if you could post a video which describes in further detail how to do the cross fiber massage specifically for this issue - what we should be feeling, how to tell if we're correctly getting to the tendon, and how to not grind on the bone. Your videos have been so helpful! Thanks so much.
Physiotherapy via dad jokes. 👍🏽
Yes! Grampa jokes is what we need nowadays, really.
I travel all over the web picking up information and always return here 'cause you make me laugh and things don't seem so bad. Thank you.
You guys are the best. Between you and my Gray's Anatomy book I was able to diagnosis, rehab, and heal. Many Thanks!!!!!
Most helpful video on Interior tendonitis of ankle I've seen: heard that it can last forever and intermittently crops up when you think it's healed.
Ugh ain’t that the truth. Didn’t work for about 1 1/2 years thinking it would resolve overtime... nope! Day one of working again, even with appropriate inserts, tons of pain. Feels like it’s never going to go away. 🙁
@@thecreatedvoid117 You're not alone. It started for me when I was 19 and I'm 26 now. It's slowly getting worse. My feet have always been extremely flat so it set me up for always hurting this tendon.
I’ve been living it for over two months, feels great one day then hurts the next. It’s slow to heal but I’m going to try the massages they show.
The struggle is real. I injured mine running for exercise. I’m icing, compressing, elevating, massaging and staying off my feet for ten days to heal up. I am hoping for the best.
I’m in agony. Going to do what they say and hope for the best. Two weeks ago my foot dr said i would probably need injections, but i really really don’t want that
"I'm not straight."

Great video
You guys are way better then DR phill 😂
It's always clear & simple solutions

Great visual w/ red marker & explanation of PTT. This was an old injury that popped back up, and your simple clear instructions delivered w/ a humorous approach were helpful. I’ve followed your advice from other videos on neck / shoulder pain & incorporated your exercises into my daily routine. You guys rock! Thanks a million
There are so many videos on You Tube for treating PTT but in my case, where my PTT is exacerbated by two stress fractures and oedema (swelling) I find this is one of two TOP sites - really helpful (the other is "Ask Dr. Jo"). It is easy to follow and effective - just one question: how many times a day do I do the execrcises and how many "twists and turns" do I do.
THanks for a great site.

Thank you SO much for this video! It described what I was going through perfectly! Did some of the suggestions and affected me immediately! Thank you!!
Hi Bob and Brad! Quick question about these treatments. Should the icing be done for however many days it takes until some of the pain subsides, and then move on to the cross tendon massage? Or should they be done together, i.e. one after the other? Basically, what would the progression of the treatments be? Thanks!
Very good question.
"The most famous physis therapists on the internet, in our opinion" always makes me lauhg
When you research this injury it gets very scary. Talks about the foot going flat and and needing surgery come up a lot. Is there a chance that if treated early it will go away? Is going on walks about 2 tp three miles a day bad for this?
My podiatrist told me that if the normal treatments didn’t work (rest, icing, good inserts, etc) then I would possibly looking at surgery in the future. That would be for PTTD. If you have tendonitis, it can certainly be temporary and cured. But PTTD feels like a losing battle.
I’m a lifelong endurance athlete (bike racing) at age 45 stsrted running. Ramped up mileage way too quickly and now my left foot is flat and constantly painful. I went from an athlete to debilitated in a week. It’s awful.
@@jaymueller2418 Hello am late to this thread but Im in the same boat currently and having to take a break from distance running. Have you healed ever since?
Hi guys
I've been experiencing shin splints (posterior side) back and forth for 3 years. I've noticed along with the shin splints, I also experience pain in the area just above the inside part of the ankle. Is this actual ankle pain or somehow related to my shin splints?
I rolled my ankle (lateral) sprain several years ago and received a brostrum reconstruction. Now I have pain on the underside of my arch in the area where the tendon shows to connect in your video as well as instability and discomfort on the lateral side of the ankle. Additionally, I have pain on the lateral side of the knee same leg resembling itb pain. I'm virtually certain the two are connected, but don't know what this is called and/or where I should start before going to see the surgeon again. Please help.
GREAT summary. Complete with references to the literature. As a primary care physician, I highly recommend this video.
These techniques actually help me a lot in my rehab following Post Tib Tendon reconstruction. Had the surgery in June 2020, boot off at the end of July, and it’s taken me this long to get at least 98% of my full functionality back.. I still struggle with single leg calf raises on the surgical foot.
But I find that once I’ve massaged the tendon and the muscle a bit, I am able to do more.. my physio obviously does the massage better 😂
Can we have a video on exercise induced compartment syndrome if possible please? 🙏
Thanks Bob and Brad for this video. I used cross fiber massage on a different tendon a while ago which I saw on one of your other videos. I knew I had Posterior Tibial Tendonitis but didn't know how to use cross fiber massage on it, till i watched this. After only doing it 2 times it has made a huge improvement. The tightness has nearly gone, more movement and little discomfort. THANK YOU SO MUCH! I have learned a lot from you guys!!!
One of my achilles feels slightly inflamed (my left foot can walk but i can feel it a bit compared to right) is this just purely down to overuse? (i have really bad flat feet) please can someone help me with this

Hi, I have had PTT for a year and 4 months caused by years of kickboxing and flat feet and it doesn’t seem to get better on its own with rest. Is it too late to start these exercises/stretches? I really want to get back to KB, is it possible because I am really running out of hope?
did it ever heal?
I'm in the same situation, going to do a MRT to see if it's broken or not
I'm deal with the same thing. They said it's not broken.
You shouldn't stretch a tendon that is inflammed. That will make it worse. Your goal is to improve tendon tensile strength. A tendon in the tendonitis state is already loose / worn away and stretching it makes it worse.
I cannot toe off anymore causing me to limp. Very active. Just started 6 months ago. Whole ankle stiff and foot numb. If do range of motion get cramps.
You might need tarsal tunnel surgery
You both are fantastic! Just watch 3 of your videos back to back. Genuine knowledge, simple self tests, with possible alternative causes of pain facts dont line up with your symptoms. Before watching, other online doctors had me convinced that I had an arch issues. Come to find out its a muscle group i didnt know made that big of a difference while causing so much pain. Cheers and bless you both!

I have Patellar Tibial Tendonitis (multiple ankle sprains in the past years, over-training before fully healing, and maybe some postural/structural issues) and may have sustained some serious damage from a PRP injection. Do you think you could answer these questions based on general knowledge?
1. Is it possible for permanent nerve, tendon, or ligament damage to occur from a PRP injection in or around the tarsal tunnel?
2. Can BPC-157, TB-500, or HGH injections heal serious nerve damage?
3. What else can be done besides surgery, traditional PT, and RICE recommendations? I’m trying everything to get this back in shape because it could honestly be a life altering injury, given my career and dreams.
Hello Bob and Brad: I've been there and done that with respect to TT therapy. Why do I continue to feel some discomfort there after five months since my injury? Does this tendon take a long time to recover?
I would like to know too.
It’s been literal years since my surgery and it still is painful
I sustained a high ankle sprain last year playing hockey. Received excellent physical therapy and your advice was sound and supplemented what I was already doing. Many many thanks gentlemen.
Hello, i just came across your video, wish I had seen a year before. I have been diagnosed over a year back with complete rupture of posterior tibialis tendon in my right foot! Being a nurse I am on my feet 12hr times three days a week, along with my household! When I am off I cannot stand on my feet, but have to complete responsibilities. Now that my podiatrist suggest that I need to go for surgical repair which I am not for as my left foot is being compensated with over stretching of PTT. I got IPRF therapy 5 days back to strengthening tendon on left. I am trying to lose weight being on low or no carb diet as I am unable to do any normal exercise. I am approaching you guys to get any advice regarding the same. Thank you!
Oh my. No carb diet works?
My tendonitis calcaneus talus (that's my medical officer gave me the diagnosis and I think it's exactly the same like what Bob and Brad described ----- posterior tibialis tendonitis!) somehow keep giving me 'signals' ----- I'm totally craving for wheat-based noodles LOL and it's always feel better when I eat all the cooked carbs LMAO
Would this cause me alot of pain? debating on going to the doctor tomorrow. been hurting for 2 days below my inner ankle. very stiff, warm to the touch, swollen, and can't walk at all, hurts very bad 😭 should I try these for a few days or should I go in just too be safe? I have insurance but it would be a choice between paying utilities and paying the copay to see the doctor.
So i used to play around 12 - 15 hrs of intense basketball a week then i brought it to about medium intensity maybe 2-4 hours a week around 5 weeks ago until about a week ago but my ankle does not seem to have gotten better. Maybe slightly but the pain is still there. I recently went to the park and played about 9 hours over 4 days and sure it does hurt but I'm able to play on it. The test didn't hurt as much but when i point my foot downwards and far as i can i hurts or when i rotate it extremely clockwise. I'm not sure what to do and whether to completely stop basketball for a few weeks. I had about 4 days off and it felt much better but a sudden movement caused the pain to come back and i played 2 hours of basketball the day after and the pain is back to where i was before.
Sorry for the essay, will appreciate any help :)

Is it common to have sharp pain when adjusting to a new insert and shoes, after being diagnosed for posterior tibial tendonitis?

I'm so grateful for this video! I had a calcaneal spur removed in 1989, along with a chunk of the plantar fascia which the doc said was so thick and immobile it was not useful anymore. BUT, after that surgery my navicular bone dislocated chronically without the support of the tendon. This was discovered by a chiropractor and confirmed by a bone scan grudgingly done by my podiatrist, who would not believe my pain was where I told him it was, since it didn't make sense to him. I've had this tendonitis in ebbs and flows for 29 years and did not know that's what the pain was coming from, I thought it was just that my bone was out of place again and I had to let it move back. UGH. Now I can treat it!
Hello,
I have very sharp and extreme shooting pains in my posterior tibialis tendon. My PA said it was my Achilles and I got an MRI to the Achilles. The MRI does not show any issues with my Achilles.
Question 1: Would the Dr be able to see a posterior tibialis injury on an Achilles MRI?
Question 2: The Dr assumes that I have lumbosacra lroducolpathy or crps. He only looked at my Achilles MRI.
Thanks.
@hanselxy thanks for the reply. It wound up being a nerve. I was prescribed Gabapentin and was walking the next day.
@hanselxy I did 300mgs 3x per day for 5 days and stopped.
Lesson learned: if muscle relaxers or pain killers like Oxycodone are not working, ask the Dr for nerve pain meds. Gabapentin is very safe and easy on the body. If it works then you know you have nerve pain. I couldn't walk for 5 months until I researched then requested Gabapentin. I literally was walking the next day.
I broke me ankle around that area now it’s just pain and popping even after surgery
Not sure if stretching is a good idea. This condition is progressive and, if serious enough, surgery is the only solution.

@@btugrul What type doctor diagnosed it for you? I think this is what bothering me👎
@@juniorsarmiento7292 I saw sports medicine as well as an orthopedist, and both diagnosed the same. It seems the key is to be very patient during the recovery and begin to do exercises really slow. Otherwise, it reoccurs very easily.
@@btugrul Sounds crazy, my orthopedic doctor hasn't diagnosed me with anything, but I feel like my punture wound got to this tendon. How long she ago did your injury happen?
And what doctor is the one helping you with therapy?
Thanks
@@juniorsarmiento7292 posterior tibial tendonitis has 4 stages, the first stage causing mild symptoms and the fourth being the most severe one with arch collapse and requiring surgery. In case you have it, I assume that yours should be stage 1 or 2 which normally heals itself. If you have pain around the medial side of your ankle, you'd better take sufficient rest stopping any weight bearing physical activity. Apply ice some 3-4 times a day and take antiinflammatory drugs like ibuprofen. After couple of weeks you can gradually begin to do stretching and strengthening exercises. Never hurry otherwise you flare up the tendon and it begins to hurt again. Do you have any sports medicine specialist in your country? Normally tendon-related problems don't appear in x-ray, but a physician or sports medicine specialist easily diagnose it with a MRI scan.
Is it ok to run on this? I have track coming up, and I've just started feeling pain in the area of this tendon. It must not be bad yet, because I'm able to do those calf raises(as many as I want until my calf gets tired) with really no problem. It only hurts when I run and for a short time after. Is it possible that I can still run and have it gradually go away?
Don't injure it further. But you posted that comment 2 years ago. I'm a little late
Thanks for the vid! Do you think once pain subsides that a runner could try to ease into running with an orthotic in the running shoe?
I have touch tenderness in the area of the PTT. But no problems with the one leg stand. Does the one leg stand test work show results even in the early stages of a PPT issue ?

I have an accessory navicular bone and have having lateral ankle swelling for 5 months.
See podiatrist tomorrow.
Pain in ankle is severe and I want to know if this helps for this extra navicular bone
Thanks!
My podiatrist told me that my accessory navicular bone was most likely the cause of having PTTD to begin with. You can still manage the pain but as far as the long run, this bone may cause more damage which may lead to the need for surgery. It really sucks. I’ve had this issue since I was 25 and most people who experience it are over the age of 40. Had I not been “blessed” with the genetics of having the bone, I most likely wouldn’t even have the issue.
Bob and Brad THANK YOU!
Over a year ago I thought I “sprained” my left ankle. No big deal I thought. 🙄 Mistake. Insert long story. I finally saw an orthopedic surgeon a couple of months ago and was diagnosed with tibia tendon insufficiency. More search for help after the doctor’s prescribed orthotics were not helping. I found this video a few days ago and WOW! Finally pain free! I’m now working on stretching and getting the ankle stronger. Thank you!

I am very curious about the surgery, as I may need to have it done sometime in the future. How long was the heal time? Were you bedridden during the duration of healing?
Could we have more on this topic, please? I have a stubborn case of this.

How can you tell a difference between a torn tibial tendon vs inflamed?
You get a diagnosis from a Podiatrist or a Sports Medicine Orthopedist. It may require an MRI to get an accurate diagnosis. Don't wait because if it is going to require surgery the longer you wait the more nasty the surgery is and the longer it will take to get decent function back.
These gentlemen are amazing!
"Cah Tok" for the body. Alleviated my condition to some degree by wearing flip-flops with orthotic support around the house, instead of going barefoot. Hint on bent knee, great! I was using my plantar fasciitis stretch (straight knee) and not getting good results.

Hi, thanks for the helpful tips. I just had a question regarding shoes for running. Does a high heel drop help alleviate some stress on the post tibial tendon? I switch from an 12mm drop shoe to an 8mm drop shoe and started having post tibial tendonitis after 2-3 runs in the latter. I was using the same custom orthotics in both. do you think the lower heel drop had something to do with it?
My husband seems to have this, and what with that and his Neuropathy he can hardly walk.
Would you recommend those heel patches you can buy.

Thank you! my Chiropractor recommended this video, what I'm looking for is how to address the connection between waking up with pain in the pelvic floor and the bottom of my feet. I'm pretty aggressive with rolling out the bottom of my feet at night, and the PTT/IT band areas, but I would love info on understanding and addressing the "root chakra" area. Thank you!
Do you have a tape technique for Post tip tendons problems. The arch of my left foot has fallen and give me terrible pain. I do wear custom orthotics and I wear a brace at night.
I rolled my ankle playing basketball coming down on a rebound. The ankle rolled inward and i have common ankle rolled pain but way worse pain on the inside of foot and arch. I think I might have teared the Tibial Tendon. I can't even put weight on the foot, it kills. What should I do?

Please help, I had tenderness and pain so much I couldn't stand a slight touch to the area under the ankle bone, where you describe. I also had pressure in my heel. I rested for 2weeks but then danced on it. It felt so so, and then I felt pain in my arch. The next day I rested it. I have also been arching that foot to get comfortable playing my new ukulele. I think that has a lot to do with this. I was arching it constantly for an hour or so mostdt days for a two months. I woke up the day after and my arch hurt too bad vto walk. My heel was swollen, toes were too. If unelevated my foot turns red, purple or bluish within a minute. It was also cold from the knee down. I went to Dr who said she didn't think I had an injury, just Raynaud's. She ordered ultrasound to test blood flow. The right hurt foot was lower at big toe then the right but vein specialist said numbers are acceptable so he's not worried about it changing colors. It worries me. I have been resting it and its slowly improving and less cold. I have been stretching and trying to walk on it a little, wearing arch supports. Now it hurts bad again in the inner arch. Epsom salt took swelling away, but heel and back of foot hurt a little and look bruised. It will be 4 weeks since I danced on it. I have been resting all day.
Thanks, you two are saint saviors.
I'm new to the pain, even changing my super flat shoes to some kind of commercial ready made orthopaedic shoes but the pain isn't going away completely yet (maybe the insole arch is too high LOL) and am planning to go see a doctor.
But first of all, I must bring my overweight body weight down to normal =/
The first time I had this, I just sort of let it go and it eventually felt like I was walking on broken glass. I had to have three weeks of physical therapy. Now I get on top of it right away.
it only hurts when I lift my toes
