  • In this video I discuss scarlet fever and the bacteria that causes it, known as strep A. In winter 2022/23, there was an increase in infections in England, including in the most severe form, which can potentially become life-threatening.
For most children, scarlet fever is thankfully a mild illness, easily treated with antibiotics.
Here’s what you need to know, so you are aware when to worry and when you may need to seek help.

My daughter had Scarlet fever 25 years ago. High temp and sickness but no other symptoms till the skin fell off her hands. It took ages to get her diagnosed and she was really unwell for around 5 months. I wish you G.Ps all the best in spotting these cases as i know how hard it can be. X

Thank you for that kind comment. Sorry to hear your daughter had a difficult time with it all those years ago. We GPs do have to keep an eye out for the rare worrying cases in a sea of straightforward ones, we are kept on our toes everyday!

Is a 'Sand Paper Rash' a rash that looks like you've rubbed sandpaper against the skin, like 'Road Rash' the name for the injury where bikers are scraped along the road, or is it a rash that looks like sand paper. I know what sand paper looks like very well, using it daily, but cant smush together a visual idea of what skin and sand paper combine together like. Tiny, tiny bumps maybe? The most coarse sand paper that you can commonly buy has grit about 1/5th of a milimeter across, or put another way, a cm contains 2500 grains, I'm pretty sure my skin is a lot bumpier than sand paper on a good day. So it must be skin that has had sand paper scraped across it. In which case is it just a quick rub of the sand paper to leave some fine scratches, or are we talking red raw, just slipped and leant on a belt sander (I've done that, it doesn't look pretty)?

It’s not the look of the rash as much of the feel, like fine grain sandpaper, rough to touch. Sunburn with goosebumps is how it looks on pale skin. Hope that helps.

@@DrSophieGP It does, thanks! I just keep hearing people use that sandpaper phrase and so asked friends and they were 50:50 split on "looks like sand paper" and "skin after being sandpapered", so really helps to get some clarification :)

My daughter has been prescribed antibiotics for scarlet fever in liquid form as she's 2 but I can't get her to take it no matter what I try. Any advice?

If it’s penicillin V is really does taste revolting. Try bribing with a piece of chocolate straight after, or mix with something sweet like honey. Coating their tongues with peanut butter is a trick some try, or squeeze syringe to side of mouth so not touch tongue. If none of this helps, spk to your doctors to see if you can get an alternative.

What if the child’s fever has gone (4 days fever free) but he still has the rash and also a poor appetite. Does he still need antibiotics? Thank you.

If you’re worried they are unwell with it, then best to contact your GP surgery to get some advice. No clear cut answer I’m afraid.

My sixteen month old daughter is having Amoxycillin atm for scarlet fever. It's been four days and the course is for seven days. She's all the time tired, hardly plays by herself and even though her temperature shows 36.2, her face, hands and legs feel a lot cold than normal. Should I ring the gp again or not please?

I would say yes. To be safe. And make sure it's the right antibiotics that she needs. How is she doing today

With a sore throat, soft foods like bananas and yogurts are the best bet.
