In this video, I'm sharing some surprising updates to the way we treat and manage soft tissue injuries, like sprains. For a long time, we’ve been told to use ice and take anti-inflammatories.       If you’re struggling to swallow food and drinks, you should get a medical review. When it comes to tablets, people are just usually overthinking it, so trying to distract yourself can be helpful.

    Yeah yeah i cured my rhumatoid arthritis by cutting out gluten whereas the specialist that i went to see at the hospital gave my an imune suppressor that made me loose weight vomit diarrhea dizziness no consentration at all moody for 3 months after that i went back to her she said we need to tast you for cancer because of the side effect of that rhumatoid arthritis treatment, so if you are curing us for 1 thing to give us worse, oh my goodness, i stand up proudly and louedly i will follow some alternative medicine 1000time better than conventional medicine and my sister is a professor in nephrology 🤨😤

    I’ve had asthma for years and the worst part about using an inhaler is when your coughing and then have to hold your breath, it’s so hard to not cough right after

      Ideally, your preventative inhaler means your coughing should be under control and you only rarely need to use your reliever inhaler. I know that’s easier said than done!

    My A1C is two tenths of a point from being type 2 diabetes. My blood test came back today. I’m 43. My BMI is 20.4 so I ain’t fat. I’ll have to cut all sugar from my diet and get more exercise

      No, it doesn’t sound like it. Sometimes there is a genetic disposition to type 2 diabetes, but it might be worth discussing this with your doctor.

    If in the low numbers of type 2 diabetes, can this be taken down to pre diabetes? Im not overweight and just found out my fasting blood suger was high.

      We don’t diagnose type 2 diabetes with just one reading, unless there are symptoms. Usually we repeat the test to confirm it is still high. If someone is diagnosed, it is possible to go into permanent remission, back to pre-diabetes levels.

I was really excited yesterday when I was almost within normal range....but had switched to my right hand when measuring for that reading. This morning, 119 with right hand, then 131 with left hand....only a minute apart....So happy, then so disappointed

    This is a great video. I get bad brain zaps, tinnitus, dizziness and severe confusion which has put me off tapering for a long time. This has caused me more anxiety actually. I do think before prescribing anti depressants the patient should be made well aware off the possible withdrawal effects and dangers! Currently along side my tapering plan I'm using prescribed 'alternative' medication to help cope with the anxiety and symptoms of withdrawal which for me personally has worked well so far

    Thank you for speaking out about this. I recently found out that the ''GP'' I thought I was seeing the last two times I sought medical attention from my practice was in fact a PA. He blurted out to me he's a PA after I suggested he orders a scan for my leg but he had to explain he doesn't have the power to do that and needs to consult the GP. I assumed PA was a junior doctor or something along those lines, but when I googled PA soon after I left I was flabbergasted. Your are absolutely right that their title is misleading and I'm certain there are probably hundreds of patients who are fully unaware their ''doctor'' is actually someone with a 2:1 in biology (best case scenario) and a 2 year masters masquerading as physician.

    Please help, i need to take iron capsules and i tried it method, and i still can't swallow the capsule, only the water, like i drink all the water and capsule just left in my mouth, it just melts😢 I already wasted 2 capsules and im very scared :( Maybe this capsules are just too heavy so i can't swallow them? They are also very big :(

      If you’re trying this method and it’s still not working, you might be able to see if you can swap for an alternative iron supplement that is easier to swallow.

    Dr Sophie can you provide research data/ literature evidence where patient safety was at risk because of PAs being in practice ? In addition there is lot of literature out there where patients are aware of rile of PAs and the care which PA provides, especially in USA and Canada. Also PAs can prescribe and order imaging. They don’t merely draw bloods or take histories. I think there is a scope to educate within medical profession about role of PAs.

    hey! I just recently started using an inhaler, I've been doing it for three days now, and I kinda got the hang of it? But I'm struggling to keep my tongue down and not touch the roof of my mouth while trying to inhale, it's causing the medicine to not go down and just get caught on my tongue, if anyone had some advice that would be awesome!<3

    Last year (2023) I had polymyalgia. Within days of startting on a course of steroids, I had shingles as well as covid 19. I was 80 years old and was told that, after age 79, I was no longer eligible for shingles vaccination. If I felt I wanted one, (which I do,) it would cost me around £440! I am, today, a healthy and fit 81 year old but may be in need of a further course of steroids. Is there any means by which I could have free schingles vacintation on the NHS?

      The rules do change, but if you are out of the agreed age range, then it won’t be available to you on the NHS I’m afraid. Glad you’re well and didn’t have any lasting issues from the shingles.

    PA school teaches you to ask for help! PAs don’t just do a two year course, without having a BSc - 3 sometimes 4 year course. Many medical schools only do 3 years of theory with placements staring in year 4 & 5. Stop misleading people to think PAs have just done a 2 year course. You are saying they aren’t doctors yet all you do is compare them. 8:15 you’ve just described your F1 colleagues who have done the equivalent amount of time in university. They see patients and gave but had that many examinations. It seems you think PAs are trying to be consultants? Cause what kind of unnecessary comparison are you making?! And have you bothered to explain that all juniors work under consultants - the consultant makes the decision mostly in the hospital during WR. The junior team typically do the jobs. Get your facts right. Nothing but a biased agenda here.

      Ask any PAs who have gone on to become doctors, they will tell you the training is worlds apart and yes I do compare them, because PAs are being used to replace doctors. I’m not going to comment any further. It is a tricky situation and I’ve said many times, this is not the fault of hard working PAs, but rather the senior staff who have set up this new role without any proper evidence or planning. I wish you well in the future.

    This video is a bunch of prideful and arrogant nonsense! It sounds very personal to me when you accuse PAs of taking the role of doctors. PAs work under the supervision of consultants- what you fail to mention! They aren’t just working in specialist roles autonomously the way your biased and misleading video has suggested. There is definitely not a 100% pass rate on the PA course. PAs don’t work as PAs until they pass their exams! The exam is far from easy.They work as healthcare’s or band 5s. Where on earth did you get your information from! 6:27 and what do F1s do? Finish 5 year course, many of them have only had 2 years of clinical experience and they go straight into hospital, managing patients. No experience and having to manage patients alone on nights or weekends. Why do they call it ‘black Wednesday’? Again, your biased video fails to mention that part. That is far more unsafe- many don’t even know how to prescribe and are constantly needing help. But I think you assume they are safe just because they graduated with a tutor. At least PAs always have that supervision. Absolute nonsense. Spreading false information. Seems you get your info from Reddit and X Please mind your business and focus on your job.

    It is normal to expect these sort of hate campaign during the early days of physician associate profession. It is very unfortunate that a patient died as result of receiving medical care from a physician associate. However, everyone within the health care establishment and many legendary consultant doctors who supervise Physician associates and their training trust the training and the qualified physician associates ability to practice medicine under a consultant doctor as delighted. Alike doctor's physician associate also undergo continued medical training throughout their career and work very hard to improve their clinical competency to provide safe patient centered care to their patients. Besides every 5 year a physician associate undrgoes a comprehensive examination to Keep up their professional registration. When it comes to medical errors and patient safety, Honestly and statistically speaking in and out hospital fatality related to medical care happens all the time because of medical ignorance and negligence from physician of all walks of training.       I cover lots of these points in my video. It’s ridiculous to call this a hate campaign. I, like all the royal colleges who have spoken out about the issue, am concerned about patient safety, as well as PAs who are being misled about the future of their profession. PAs are now struggling to get work and I think it is unfair to allow the courses to continue without the appropriate structures for nationally approved training, supervision agreements and scope of practice in place.

      @@DrSophieGP sorry if my comment disturbed you doctor. Physician associates are not doctors and they almost never replace/ replaced doctors anywhere. The training program and national standards are set out in the USA, UK, and Ireland. Besides, there are many physicians and medical school faculty members involved in the training of physician associates. In almost all areas of healthcare physician associates work within the scope of practice of their consultants.

      @@sibycherakathilchacko7392 Please do more research before commenting. There is a reason the royal colleges and BMA are all speaking out. PAs are indeed being used to replace doctors both in primary and secondary care in the UK. There is no nationally agreed scope of practice.

    No no no. The inhaler should never be used in the mouth. Use space or a spacer to get the medicine into the lungs. Your incorrect method directly contributes to oral issues. Not correct! 😢 your video should be removed stat!

      This is incorrect, evidence shows most adults who are using the inhaler correctly do not benefit from a spacer and it just adds to plastic waste.     Proper use for best effect is to use it with an aero chamber, otherwise a majority of the medication hits the back of the throat and never makes it into the lungs.

      Nope, studies have proven for adults with correct technique, the use of a spacer makes no difference and just adds to plastic waste.

    I can get these cartridges through my prescription so can’t be that bad 🤷🏻‍♂️ plus it helps with anxiety and many many other health issues physical and mental so it isn’t bad overall considering it’s been medically legal since 2018 in the uk

      It is only licensed for a very restricted use, including intractable nausea and vomiting, MS related spasticity & severe treatment-resistant epilepsy. Anecdotally, many people find it helpful, but we don’t yet have evidence for most other uses.
