THE STORY OF THE WITHER:
Here's my idea of the story:
The paintings and recipe for red sandstone is a warning left by the ancient builders. The painting shows people that the Wither is deadly and possible symbolizes the two dimensions it wrecked, and the red sandstone recipe tells the player to build it in a Mesa where there aren't any souls around. This could also mean that this is what they did, however it is unlikely considering how strong the Wither must've been to destroy a civilization. It probably had many animals around to kill. The red sandstone tries to warn the player not to make the same mistake. The chiseled nether brick recipe will be explained later.
What you have to realize is the Mobestiary is a player/settler trying to figure things out. There can be red herrings that aren't true. However, I will have to obtain it in the future for research as to see just how accurate it may be.
If the Mobestiary is right about the scholars, they were theorizing the Wither's existence before the painting. The painting likely was from somebody who has been to the nether as seen by the red sky. But wait a second, how could people theorize about it if it was never seen before? And that is where we deal with setting. This book likely talks about a time AFTER the ancient builders. and the painting could have come from a survivor that saw it's creation. I have opened a tab with the page about the Wither, and I noticed something: the writer acts as if the Wither has been summoned multiple times. The writer states, "There are many ways to fight the wither. Some use trickery, summoning it in such a place that it suffocates in bedrock." Notice how it said that some people use trickery when summoning it. Not "will use," but "use" This means that summoning it is not an uncommon thing. Everything points to this being after the extinction event, where a few of the ancient builders survived. This is the story of the first Wither.
The original Wither must've taken over the Nether, unable to fit through the Nether portals and instead destroying them with such force that both sides are broken with purple goo leaking out of some blocks. The ancient builders in the Nether created fortresses, and in the recipe books they depicted wither skeleton skulls to mark the purpose. This might have later been what allowed scholars to theorize about the Wither: the depictions, and the wither skeletons. Anyway, though the fortresses survived, the builders did not. They were instead turned to more wither skeletons and now are a means for the player to summon the Wither. The Wither then went to the Bastions, which were used as stables, treasure rooms, and mining outposts. The ancient builders, knowing of their impending doom, armed the piglins with golden weapons and crossbows. All of the better gear was in the fortresses. They fought with the piglins against the wither, but fell. The ancient builders were turned into wither skeletons and helped the Wither, so the piglins learned that these black bones were no longer their friends. Now they fight any wither skeletons, knowing what they are capable of making. Eventually, the piglins killed the Wither, and any surviving builders escaped to the Overworld, and depicted the scene of death in a painting to try and frighten others from creating this monster. The piglins that were withered but not killed were zombified, with their skeleton showing. Builders that withered away without being killed directly by a skull were turned into skeletons in the Nether. The warped forests emerged from the soul energy in the Nether. Somehow the Wither's rampage must have mutated the mushrooms into soul powered mushrooms that have enough energy to show it in their color. These fungi are what make a warped forest instead of a soul sand valley.
There are whole biomes that are filled with the souls of the fallen builders and piglins.
The painting inspired the settlers to try this in the Overworld out of curiosity, not caring about the fear of the scene. This is what caused the extinction from Withers to continue in the Overworld. The Wither caused the builders to seek something that would kill it. It had taken the souls of many animals and builders, causing the few that remained to put the recipe on chiseled red sandstone to show any person crazy enough to try this to make it in the mesa, where there are few if any animals. Nothing seemed to kill it, and each death made it stronger. Over time, they went underground to the Ancient City. As seen, or rather heard in Disk 5, the builders tried to make a portal to another realm to escape. They probably used some warped materials knowing that they had soul energy. They also used soul sand and soul fire, having learned of it's potential. Finally, they needed a hard material, unlike the weak netherrack. They used the deepslate around them and even possibly some bones from the skeletons, which are withered builders, to make a frame, and soul fire underneath to give heat to it. Then, the Wither approached, and they had to try it. They went from the deepslate caves to their barracks to gather forces to... wait, The Game Theorists already explained this. Anyway, they open the portal and something similar to the Warden comes out. They are silent and still in fear of this new horror, but the Wither knows no fear. it thinks it's the apex predator. But this new creature begs to differ. In one shriek, the strengthened Wither is killed, and the souls from it are used to corrupt the ancient city and much more of the underground. Shriekers and sensors rise from this skulk, and Sculk Catalysts are formed. The bones of the Wither fall into the sculk, and this monster goes back to where it came. The portal closes, and the builders feel a sense of relief, with this new plant-like thing. They don't know what they've brought, however. The unearthed bones make up the shriekers, and after they set enough off, the sculk feels endangered and creates an abomination out of bone for structure, skulk to hold it together, and souls to power it away from the sculk. The Warden. This thing starts killing, and after they try to adapt to it with wool, they realize they need to find an escape. They see the zombies from the illagers, skeletons from the Wither, and Wardens from the Sculk, so they try to find somewhere else. They build the strongholds, with winding corridors and numerous staircases to escape the Warden if it were to get to it, and a portal room where they worked on their final escape. They tried to build the End Portal, and would meet their end. But that's a story for another time, as is the tale of the illagers. The story about the Wither is over.
Also, chiseled red sandstone does not spawn naturally, however it is in the recipe book. That makes it viable proof, just not in the way that you stated.

And fun fact: The Wither Storm, which is an official mob but cannot be summoned in the base game, actually turns all of the blocks into obsidian. This is proven by the texture of the tentacles. Also, the Command Block might be a sign that there is some force that turns everything into blocks to be similar to the Command Block. Even the Wither Storm, which changes the atomic structure of blocks to change them into different blocks, follows this rule and always has cubic or non-curved parts.
I have a controversial but interesting idea. Look at the center of the wither (the bones) and look at the warden. Other than the small hole the warden has which has souls in it, it looks like they are formed from the same kind of creature. Since they have the same kind of body structure
if the ghast tears are Made out of souls, then the end crystals are crafted using them, because the souls give energy to crystal. Aj overwhelming energy that IS the reason the explosions of end crystals are so powerful.

Theory: The wither the settlers created might be different from the wither we create using 4 soul sands and 3 wither skulls. The wither skulls of the wither might be the skulls of the dead soldiers that got burned in the nether. The soul sand is a collection of souls. The nether didn't have soul sand before the wither died.
So what happened is that a sorcerer painted the wither painting and knew that the painting he created will eventually lead to the destruction of the ancient builders. So the sorcerer painted it, placed it outside where some people can see. A group of settlers saw the painting and got curious. They attempted to make the brown thing and made a prototype. The prototype soul sand they made might be very strong because they have never created soul sand before. So they might have used the souls of the fallen soldiers thinking that the creature they are gonna build will be like the iron golem. So they used the souls and the 3 skulls of the fallen soldiers. They placed 4 soul sand in a T shape, and placed 3 scorched skulls on top. The souls powered the new creature, the wither. The wither might have bigger than the wither today because the ancient builders didn't know how many souls they should put on the experiment. The giant wither shot skulls everywhere, killing many people. The wither eventually escaped the lab and started destroying cities. The wither has the power to apply the withering effect on anything. So when a creature gets killed by the wither, the creature's soul gets absorbed by the wither and leaving a flower with the effect withering. The withering effect happens when the withered plant is stuck on the creature with it. That's why when a player gets hit by a skull, a plant with withering stick into the plaher, causing the player to wither away. The wither continues to destroy cities. The ancient builders had another problem. While making a prototype of the soul sand, they brought back the dead. The dead infected the ancient builders. Somd ancient builders went into the nether and infected the nether. That's why there are zombie piglins. Then some of the ancient builders escaped to the end, while others stayed in the nether. They defended against the zombies and the wither. Finally, the ancient builders got killed and they coded that the blazes should kill the ancient builders so that the wither will not follow them in the future. The ancient builders killed themselves using the blazes. The wither found a bastion, where the piglins are, and the wither tried to get souls from them. The piglins finally killed the wither and when the wither died, all of the souls got crystallized into a nether star. But the nether star couldn't handle the compression and exploded. All the souls got flung into the different regions of the nether, leaving only the large fossils of bones and soul sands becoming the soul sand valley. The modern wither has less souls in it than the first one, so the nether star doesn't explode because of compression. The nether star is like the sun. When the sun can't handle too much pressure, it explodes into a supernova. The remainding ancient builders got stuck in the end and never returned.
And that's my theory.

Matpats theory says that the wither skeletons were ancient builders, but you get normal skeletons in the Nether. They aren't black either.
My theory is that there were actually two different races of ancient builders. One race was taller, the ones that were killed in the Nether become wither skeletons.
This ancient race eventually evolved into endermen. Either this evolution took place before the outbreak of undead creatures in the overworld or this ancient race was immune to the plague and carried that immunity into the endermen.
This explains why there are no tall zombies or skeletons. The wither skeletons remain because it's not a virus that's keeping them alive. It's the evil force which permiates the Nether.

My theory is that the builders easily managed to defeat the wither, they even started farming them. But one day, a certain command block found it's way into the world, and that caused a certain... Storm you could say.
There's no evidence for this but it sounds nice that the minecraft storymode world is somehow connected to the actual minecraft, and besides, the world would have been destroyed had it not been for Jesse destroying the command block.
The Minecraft we play could be an alternate world where Jesse lost, the Wither Storm was killed by the ancient builders after they used stuff like the elytra and other lost weapons
Mojang studios and Telltale games have both said that Story mode is NOT cannon to Minecraft

Red chizeled sand stone may be the only signs that left from an even older civillization that got destroyed by the wither. Dragonic sayed that it generates rarely in mesa biomes and because there is no rain in some biomes like desert it may stayed without being turned into sand by the air conditions.

You have to look at the advancements and achivements.they give lots of clues as to what happened like the one called "free the end"
Bedrock wither is the one you should really be afraid of. They explode on death, have more health, and they have a charge attack the destroys a lot of terrain and does a lot of damage.

Only problem with this theory is that there seems to be no evidence of things destroyed by the wither in minecraft worlds.
maybe the wither when killed drops it's nether star because when they are kill, they're corrupted souls get sucked into a single star to be kept in until summoned again

My theory: The painting was made by a corrupt adventurer, once he made it the people decided to make it, thus born the wither. For my theory enderpearls are crystallised souls which power certain things, so the withers soul powers beacons made of soul energy.

The thing about HUMANs creating bastions is a thory, however on the blocks it has the piglin on it , so i believe the builders made nether fortress's and i hve loads of evidance

I think that the Wither actually met it's demise to the Warden, because maybe the Wither escape through a portal that was accidentally lit after the ancient builders closed the portal and then killed a ton of creatures and managed to go down deep into the ancient city and that's when the Warden was summoned and tour the Wither to shreds

wither skeletons are actually the dead bodies of the ancient builders that were buried in netherrack but the netherrack under the Dead ancient builders which were in the grave the soul Sand formed in the soul Sand sucked up the souls of the Dead ancient builders and then the dead ancient builders decomposed into wither skeletons
