The meaning as far as lyrics is literally about fisting but the underlying meaning is addiction and how it starts with just a little and then evolves into needing more and more. Hence why he says "Finger, Knuckle, Elbow, Shoulder". Its always going to take more to get you the same feeling.
I’ve always thought it was about addiction as well.
The music mimicks this too. Repeatedly the song goes from light to hard, harder, then back down.....over and over.
Interesting that all the comments share this interpretation. I always interpreted as pushing some so far past their comfort zone in a relationship to the point that they don’t know who they are anymore
It can be interpreted as addiction but also applies to many of the things in life or life itself. Things become boring and you need more to get the same feeling. We become jaded, unappreciative and constantly seek out the next thing to grab our attention. It is the monkey mind that never rests and can never be filled. "Not enough, I need more, nothing seems to satisfy" - It uses the analogy of fisting to portray the seeking to fill the void that can never be sated.
Hence: 'It's not enough! I need more!"

The favorite bit of irony concerning this song is: all the lyrics about how we, as a society, have become numb and calloused to the world around us, so the title is a little bit of a joke about how you have to go to greater lengths to get peoples' attention. Then MTV refused to play the song title with the video ("track number 1"). I thought that was funny.

Seeing Tool live in January, this’ll be my 2nd time seeing them. They were my first concert and still probably the best I’ve been to. Yes my pfp is Blue Maynard.
BRAD: I highly recommend “Right In Two” for some great songwriting in these polarized times. Its great storytelling and as usual it rocks like only TOOL can. In a catalogue full of bests, this one is right up there AND it just hits different. Lyrically, its on point from the beginning of civilization to our current day. Timeless classic. Can’t wait.

Always been my favorite Tool song
This song, on the surface, is about fisting, but it can also be about a lot more. Another meaning could be about falling deeper into any form of addiction (not just drugs).

Nothing better than being at a Tool show and hearing Danny doing the intro to Stinkfist, and again in the middle section breakdown(live only), the whole place goes Bonkers!! Finger, knuckle, elbow, shoulder deep inside the borderline, relax, turn around and take my hand ;)
Maynard (the singer/songwriter) came up in the comedy-standup and music scene of LA of the late 80’s and early 90’s, so his lyrics are often a mix of comedy/sarcasm and existentialism… and yes, this song is very much about needing to ‘up the dosage’ in order to feel something/anything.
This is one of my favorite Tool songs because of the meaning. Maynard, lead singer, is deep thinker. He sings about all the dark side of our mental state. In this song he’s talking about as human beings we are constantly looking for some type of mental stimulation to take us out of a state of anxiety/boredom. Stinkfist is exactly what u think it is. He’s crossing the line to feel a new different type of stimulation. As human beings we can all relate to needing some type of stimulation help us feel different n alive

the song is actually about overstimulation, and how that numbs your feelings and thoughts, so u need bigger and bigger stimulations like a...stinkfist so u finally feel something.

Here's two quote from MJK that my shed a light on the metaphor used in this song : ""Tool is exactly what it sounds like: It's a big dick. It's a wrench. ... we are ... your tool; use us as a catalyst in your process of finding out whatever it is you need to find out, or whatever it is you're trying to achieve." and I can't find the second quoute but basically MJK said in that quote that if you want to read this song as a song about fisting that's fine... If you want to find deeper meaning it's there too, it just takes a little digging.
Personally, I think that he lyrics are about the need to always go farther just to feel something and where it may lead someone.
One could and can read this as a metaphor. He touches on the intimacy that people people get into. Especially in this day and age of technology. How far are you going to go into something so deep there's no turning back! Regardless of the pain/pleasure!

When I was 19 I did not like King Crimson. After a decade of being a Tool fan, I rediscovered them 10 years ago. Then I found about about their 2003 tour together. Three words: Level Five live.

Favorite band, all-time. Listen to them almost daily for long runs or sprints, as well as hittin the gym. The band has stated that they are a ‘tool’ for getting the message that they are putting out. That message is always the same: it’s whatever you interpret it to be. Yes, some lyrics were based on MJKs struggles with his mother and other personal hardships but he’s written them with intentional malleability; easily conforming to various interpretations.
When this came out in the 90's and I was going to clubs and bars this was always the pre funk jam in the car.

The lyrics in this song are not silly. They are a great personal and social commentary, if you really listen. It's about what you think it is too. It's lovely, explicit poetry.
I remember when this song came out MTV would refer to it as song #6 (I think that was the track # on the cd) because they weren’t allowed to say Stinkfist.

Prob my absolute favorite of Tool. It was the first one of theirs that I really was literally stopped in my tracks by the lyrics.😳😳😳🔥🔥🔥
You hit the nail on the head about the lyrics. Tool sees the entire song, every instrument, is telling its own part of the same story. That's why the lyrics are rarely a focal point in mixing and production. And until the recent album, official lyrics for a current album weren't released by the band till the next album came out. Maynard has said this is also done so people focus on the entire piece and not just the lyrics.
Part of what has made them so enigmatic over the last 30 years
