I can’t tell if you’re concluding this is acceptable or not ?!
Would it come loose if it was properly torqued?

I'd bet a paycheck those wired wouldn't "come loose" in 100 years

Don't underestimate heat's ability to loosen things.

Is there a lug adapter that would allow you to split off that to a shed?

I am not sure which way they will go. Many licensed electricians have the skills and protective equipment to perform this. Not sure if their local code will allow them to perform work.

Do electricians normally have power company come out and temp disco service to remove the double tap? Or is it recommended to and most do not?. I have seen much work done on live main lugs. Seen many houses double tapped.

Electrical company will not need to come out. Also, the lugs in question might be rated for two wires. But it would have been better to install a two outlet lug for each spot. The main breaker is still capable of shutting down all after it but in this case shuts down both panels. As far as lazy electrician goes.. this is allowed under code, as breaker is installed to control the small panel. Only question is.. is there a main breaker in the actual main panel? I have seen worse in some apartment setups where main breaker is tapped using Xilco?? taps, and taped.. with as many as eight wires leading from there to interior main boxes bu all the boxes contained main breakers of lower value than the breaker in the service panel outside.

You don't explain by tapping the main feeders there are unfused wires which is a potential hazard. This should be explained to the client. Any wiring exiting the panel needs to be fused despite what the codes say. The 10 ft. feeder tap rule 240.21 (B) (1) is crazy in my opinion. Solar connections which back-feed the panel which are tapped into the feeders should not be allowed as well.
As A NJ Licensed home inspector, I routinely call out this. If a fire starts because of the unfused wiring, I do not want to find myself in front of a jury.

my panel was approved for dble tap at the main lugs before the breaker permit was $3.25 and $0.78 tax from 1957 always made me facepalm but never was touched until removal in the 2010's it was a square d panel to
