I'd like the idea of deceptive dialogue options when your character is drunk, like the option on screen may read "It's a pleasure to meet you." But when selected your character says "Noicce Titties."
Also if you smoke weed in a game, all your quest markers and objective menus should temporarily disappear, as if you forgot what you were supposed to to be doing.

Condemned 2: Bloodshot had a mechanic in it where Thomas would need a drink every so often, otherwise his aim would get super shaky and unstable. The only way to fix this was to take a sip of alcohol every so often, and it'd settle his nerves.
We need more of that shit in games.
But I'm guessing it goes the other way around too, where he can get drunk and f*** his aim anyways?
Well just like fall out 3-4. Certain drugs would effect things. Not as in depth. But I agree mechanics like these could not only make playthroughs unique, but raise awareness.

The Witcher 2 hangover missions are better. And both of them are freaking hilarious.

I like to think that the example with alcohol in Max Payne is it's his crutch for coping with loss, and it allows him to function through all the crazy gunfights and shit. Some people with alcoholism need it to get through the day, to endure the sober parts of their life, and "function". Doubled especially if it's a stressful situation, like you know, a horrible bloody one-man war in an airport. Point is, that person becomes better and better at hiding the drunk face by growing a tolerance and can seemingly "function"

I was the mother hen at the parties who was inseparable from the roll of paper towels I clutched the whole night while cleaning up the spills and blood and putting my friends to bed or chasing them down the street and bringing them back home. I don't know how I managed all that while drunk. I'm too old for that shit now.
I look back on all the shit I did and I’m amazed I didn’t stop sooner.
I’m over it now but sometimes I do miss getting beyond stupid and just letting loose. But I’d wake up the next day mortified by my behavior and what people must have thought.

When you drink in Prey (The 2017 one), one drink will mess up your vision a bit, but multiple drinks will make your movement a little more janky. Drinking also prevents the Fear debuff, which some enemies can cause that really messes your movement up. It's always a good idea to carry a couple bottles of wine around in case you run into a weaver.
I really loved that no fear thing. Get liquored the fuck up and kill an alien.

Kingdom Come Deliverance has my personal favorite portrayal of alcohol consumption in a video game. The effects of alcohol kick in over time similar to real life, and different drinks have different alcohol percentages. Drinking alcohol also has its own skill tree in that game, with perks you unlock, but if you drink too much you can also get addicted to it. Beer is also described as liquid bread in KCD, which I love.
