Maximus’ big mistake was not insisting on being physically present when Marcus Aurelius told Commodus that Maximus and not Commodus would inherit control of the empire.

If the emperor had just told Commodus that he wasn't going to be the next emperor in the room full of witnesses and Praetorian Guards, the entire film wouldn't have happened in the first place. Which, while boring, would have made a lot more sense.
Well.....knowing Praetorian Guard's track record, Commodus probably gonna bribe them to support his claim to the throne, and have Maximus killed.
