Today i wanted to do a video on the best test to capture your palpitations
Palpitations are an extremely worrying symptom for many patients even though the majority are not dangerous. The way you know they are not dangerous is to record what is happening to the heart rhythm when you are having the palpitations and if the heart rhythm is normal then its not a heart rhythm disturbance.
The patient gets the palpitations - he goes to the doctor - after a few days the doctor does an ECG and tells the patient this is ok. the patient goes home and gets more palpitations. The patient goes back to the GP. The GP says you need a 24 hour tape and unsurprisingly the patient doesn’t get any palpitations and therefore when the GP says that the holter was unremarkable the patient does not feel reassured and the palps continue happening and the patient is lost as to what to do next.
So today i thought id talk to you about the various different tests there are what the advantages and disvantages are of each
A simple ECG. This only captures 6 seconds of time and if during those 6 seconds you are having your palpitations then you dont need any other tests but more likely than not, you won’t be and therefore the ECG is useless. We do it because it is easily available and may give insight into any other abnormalities with the heart which may make it more likely that what you are complain of could be heart related.
A 24 holter test. Usually useless because the pick up on these is only 8-10% at most which means that out of 100 patients whom we give a holter to, only 10 will have symptoms during the 24h period and the only reason to have it is if you have your symptoms daily or if there is anything you can do to bring on your symptoms. In which case do whatever brings on your symptoms. A holter records continuously for 24 hours so it will record everything that goes on in the heart and the only way, we will know whether what we see is incidental or related to your symptoms is if you fill in and return a diary declaring the exact time when you had your symptoms. A lot of people also get anxious about the extra information that the holter shows up like number of PVCs and how slow the heart goes but truthfully as these things aren’t causing symptoms they are not important. 48h/72 h holters are better but inconvenient because patients can’t shower etc with them on.
An event recorder. This is a device which will only record what you ask it to record and therefore you can keep it on for longer. The good news is that they have a memory which means that when you press a button after you develop your symptoms, it will record upto 30 seconds of what was going on before you press the button. Problem with these are that they tend to be really taffy to operate and the recordings have to be sent down a phone line which many people struggle with so although the pickup is better than a holter it is not great.
A wearable patch like a zoo or a hardy patch. These are excellent and i have one right here which i will show you. You keep these on for 1-2 weeks. The pickup is nearer 50%. Not only do they record every single heart beat but they also have a button which puts a mark on the recording when you have symptoms. You can shower with them etc and finally when you have worn them for the duration, you simply take it off and post it for analysis and a report appears by email to your doctor who checks it and can send you a result. It minimises having to travel to the hospital first to have it put on and secondly to have it taken off. I have a few patients who were scared to leave the house because of anxiety and who were getting palpitations and i posted one of these to them and they worked a treat. These are not available to individual patients but they are to healthcare professionals and i certainly prefer them to any other monitoring device for my patients. They cost about 380-400 pounds.
Alivecor. This is a really innovative bit of kit which is increasingly being used. Basically if you have a smartphone you can download an app on your phone and as part of the app, you get a case which has a couple of sensors and if you hold the sensors, the app will record a single lead ECG for 60 seconds and this can then be sent and interpreted for a small charge. The advantage is it is there with you. The disadvantages are firstly you only record events after they have started.
I experienced, this situation, and it was very frustrating. I am again, learning from you, and feel less anxious. You are so wonderful.

York Cardiology hello tomorrow I will take my hooter monitor off do you know if I have to turn of the telephone or do I keep it on? I have to ship it but I'm not sure what to do exactly with the phone that comes with the monitor

I have proximal AFib. I have an Alivecor. It's brilliant! Very reassuring. Ive been able to monitor a fast heart rate while at work. Took my meds and could check that it was going back to normal. The advantage was it allowed me to stay at work and not have a trip to A&E. Yes I played with it a lot when i first got it. But that gets a bit boring after awhile. Also helped me to see what the different heart feelings looked like on the ECG, again very reassuring. Having a bubble test this week!

Maybe it's because I live in America, but you forgot one. Mine was put on a few weeks ago. It's an event monitor that comes with a cell phone that records all the time AND you push a button when you feel something. So, it records constantly and sends the info constantly to the monitoring center. It will auto capture if it senses something wrong and notify the monitoring center. It sends a summary to your Doctor daily. The cell phone transmits without having to call anything in. So it's an event monitor ,but a million times better.

I bought a KardiaMobile from Amazon and it sounds similar to the AliveCor. It was $ 80 U.S. dollars and is easy to use but has the same limitations as the AliveCor device you described. But it’s reassuring that even though my heart pounds it’s not AFib. I’m taking the kind of magnesium you recommend and bought a wedge pillow. My heart pounding seems related to lying down. I’ve also been reading on how to stimulate the vagus nerve as it seems very important. Your videos are great !

Very helpful thank you! Suffering from possible long covid, with supraventricular tachycardia (SVT), palpitations, up and down high blood pressure even though on meds, horrible sudden onset anxiety, exhaustion on exertion, insomnia, recovery relapses going on 15 months and counting.

I've been having palpitations now for about 2 months. I have about 20 a minute, every minute, every day. It's driving me insane.
Thank you, I have learned a lot from your videos. I am an ex-pat living in China. I had heart valve surgery last September and everything seemed to be going well except that I developed a very bad cough. That has cleared up but now I am told I have developed afib. My symptoms seemed to be different. I have sharp pains in my chest but I have been told that is just the healing process from my surgery but it does not seem like that. I also get dizzy spells and double vision.
I currently take Betaloc to slow down my heart rate but some doctors have been pushing me to take Amiodarone but have offered no way to monitor. Can you perhaps talk more about drugs to treat afib? I have seen several doctors and all give me different answers.
Really frustrated and confused.

dr sanja I'm having pvcs and pacs my electrophysician said he can go and ablate those...... can you do a video of you support that ablations can get rid of pvcs or pacs???
With the patch recorders, will the whole timeframe be analyzed? Or only the times you press the button will be analyzed? Sometimes I may not feel my symptoms but want to make sure it will still show up.

I'm getting a holter monitor for Tachycardia recording this monday. I have to wear it for 48 hours. Kinda nervous about it but I hope they'll find some answers. Your videos are very helpful!!

You had me worried now because I live in Serbia and I cannot find the things you mentioned (that are better than ecg and holter). When I had my palpitations I've done an ultrasound of my heart, ecg and 24h holter and all of them were fine, but now I am worried because they are obviously not reliable enough. What do you suggest me to do?
What are your thoughts on ecg watch like 5th or 6th generation apple watch?
If I'm dealing with POTS, will the monitor be helpful to diagnose?? Or do I need to do the tilt test?
The 30 day monitor also activates when it detects an abnormal beat

I'm 22 and have been worried about palpitations since I was 15. They've recently become so frequent but although I've had ecg's and an echocardiogram in the past, no palpitations were ever recorded. Thanks to your videos I've realized what I have are ectopic beats. After looking back at my ecg results I realize there was a note made that said "IRBBB". My cardiologist never told me about this, he said everything was normal. However I was wondering if IRBBB can cause ectopic beats or other issues?
that's a normal finding, often found in young patients and then it might even go away. Just get a new EKG a couple of years later to see if got worse. If it stays the same or disappeared, then your fine. It might cause a few ectopics, but so rarely that it is very probably not what causes your ectopics, unless it got worse in the EKG.
Hello Doctor, I would like to know your opinion on the QardioCore monitor as I am considering getting one for palpitations and my workouts. Is it as reliable as the ZIO pacth for instance?

Hi Doctor, I got my holter results but I realizer that I was on metoprolol during holter test. Doesnt that mean that the test could be false since I was already on a medication that controls palpitations? Please do advise. I am currently suffering with sever heart beat issues but apprently my doctor thinks nothing to worry about.
Just found your channel and finding the information very helpful. I was recently diagnosed with afib while wearing a heart recorder. Just finished an echocardiogram and an echo stress test this week. I will see my cardiologist soon to go over the results. However, I received a call from my primary doc's office telling me my echo indicates I have pulmonary hypertension. As a cardiologist, what do you do with that information? Sounds like a scary diagnosis
How are you doing as of today?
I’m doing fine. The pulmonary hypertension is very minor. I had an ablation over 3 years ago and have done exceptionally well. I have an inserted loop recorder in my chest which keeps me off anti-coagulants. Thanks for asking how I am doing.

I can't wait to have my 24 hour ecg tomorrow I do get the worst symptoms everyday nd I've had about 36 normal ecgs from going to hospital so can't wait to rub it in everyone's face that I'm not just a hypercondreact

ps. but I don't get it??? the monitors I had for three weeks and had to transfer the recordings over the phone, didn't record anything during each episode at all, I couldn't press the button while it was happening , and when I sent it back and saw the doctor he told me that it records two minutes back???!!! ,and still he said he didn't see anything like what I was having??? ,but I'm still having the systems. I was also told that it records on its own even without pressing the button, is this really true?
those event recorders are really difficult to use and some can record minutes back... i dont think they record unless you press a button but i might be wrong
That's what my heart did went from 70 to 180 suddenly it feels so awful

I need you to be my cardiologist ! I've been suffering from palpitations non stop all day for almost a month ! I was referred to a cardiologist that says it's all in my head ! I have a long history of having palpitations and was on beta blockers .. I feel lost and scared 😨 . .
It's not in your head it's in your chest! I hate when doctor's are dismissive oh you issues i just a holter monitor today for 24 hours and it's recording everything good luck to you.
@@laneyd5519 Thank you ! When I initially went to see the cardiologist I literally had to beg him for a beta blocker . I also requested copies of my heart monitor readings and I can visually see the irregular beats in the readings !!! I can see that every minute my heart was beating Irregularly !! When I talked to a second cardiologist he said there was only two irregular readings and that there was nothing serious going on !!! I've done a little research and I'm now taking 500 mg's of magnesium daily to see if that helps any along with a beta blocker .... A lot of cardiologist treat their patients like garbage 🗑 !!!! I've been suffering with irregular heartbeats since I was in my early 20's 💓 !!!
Not sure why but I trust your information. I have trust issues with doctors. Always pushing pills on me saying "here try this". Last pill I was taking to prevent and episode didnt so I switched to magnesium taurate with much better results. I know a doctor personally. It seems it just about getting money. I'm wearing a recorder now. I expect my cardiologist will get the results and prescribe a pill regardless of the results. But I'll do what I have to do because I need to go back to work at least partime at a job that's regulated.

I disagree with a lag time in results. There are devices out there that do real time telemtry and results are sent to the doctors daily....

I went to the heart doctor they put a monitor on me and I had a rapid heart rate they was able to record it now I have to go to a specialist to try and get abrasion done for svt or is it afib the recording said afib does ablasion work please respond I have anxiety also
it depends on whether it is an SVT or AFib..for an SVT ablation is an excellent treatment. For Afib not so much
The doctors said svt very fast heart rate reaching 250 sometimes it can be 60 beats then shoots up to 200 suddenly I hate it I'm going to electrofisuogist may 5 my doctor made me an appointment to see if I need an ablation done
