Less than a month before a door plug on a Boeing aircraft blew off midflight, employees at Spirit AeroSystems, a subcontractor for Boeing, had tried to warn corporate officials about serious safety problems with parts for 737 MAX jets. But those warnings went unheeded, and the employees were told to falsify records, according to a new investigation by The Lever on a federal complaint filed by workers at Spirit. "In some cases, workers were retaliated against for trying to raise those alarms," says journalist David Sirota. "These workers in this federal complaint are alleging essentially a culture of defects, a culture of fraud, a culture of retaliation."

You know its bad when the shareholders file a complaint that you are prioritizing profit over product safety and quality.
So all of the BOEING MAX airplanes 7,8,9 ALL have continuous issues unbeknownst to the public. Bowing internally removed many of their safety inspection procedures. And spirit employees were pressured to falsify safety inspection documents. Anyone who dares DEFEND Boeing or spirit in these comments is absolutely 100% an airline PR person. It should be a 20 year mandatory minimum prison sentence for going after a employee for reporting safety issues
Unless that employee is lying or has ulterior motives. Then the employee should be fired. More and more often we see people making up allegations against others all the time in the media for personal gain or for malice, taking advantage of the benefit of the doubt that's given to a self-described "victim".

But if you look at how rare incidents are compared to the amount of safe and successful use their products have had , it's a fraction of a percent. Boeing and Airbus are the 2 competitors in the market, Boeing being older which accounts for the higher # of accidents. For Boeing to be declared unsafe, there would have to be a large unaccounted for discrepency in it's # of fatalities per unit of flight time compared to the same metric of a competitor.@@jameschalkwig787
On the flip side , Boeing is a big company that's aware of it's responsibility. Whether it's motivated by profit or by passion, both circumstances are heavily incentivized to avoid any kind of failure or mistake. There's simply no scenario where it would make sense for shortcomings to be tolerated. Employees voicing their opinions in the press will likely have more to do with how strict/loose company policy is with non-disclosure agreements and how harshly they sever ties with employees that aren't measuring up to their standards, rather than any legitimate misconduct. @@phillmckill5562

I lost my lead position "pay cut" at Boeing last Nov after I pushed back on a serious safety issue against my 2nd and 3rd level managers. Safety Org brushed me off, the Machinist Union brushed me off and I've been warned that Ethics is a huge waste of time and just put a bigger target on your back. That's manufacturing today. It's shameful, I've worked at Boeing almost 40 yrs and it's a sad and sorry thing to see it's decline. It's 100% The leaderships fault. They're not leaders, just takers.
It's also those structure assemblers who have no self accountability. They are just there for a paycheck.

They need to change to laws making it a felony to do what Boeing is doing with safety reports... If a employee reports a issue and that effects there job the next guy probably won't report ..there needs to be a major way to make bosses second guess doing that...that don't care back fines that's the cost of doing business but if it's going be there freedom on the line I bet none of them are going pull what they are doing now... And make it like a 20 year mandatory minimum so they know there isn't going be a sweetheart plea deal
That’s a shame. I work in BDS. It’s not that way on our side, but BCA drives our stock price. I’ve done some work on the BCA side. I’ve not been impressed. The culture is top down there.
This is why I sold my Boeing stock years ago. Engineers used to take pride in the safety-oriented culture of Boeing.

The accident is a good reminder to all of us that when the seat belt light goes off, you don't loosen the seat belt. You just leave it on unless you have to get up out of the seat. You leave that seat belt on to protect you from things like this

This isn't new, I worked as government oversight at Boeing for years and they repeatedly refused us access to their facilities and the facilities of their subcontractors. Subcontractors, when we were allowed to inspect, were frequently caught producing subpar products with subpar materials in subpar conditions. The amount of FOD (foreign object debris) left on planes coming out of the Boeing plants was absolutely astounding. The response from the DOD and congress was, and still is, to just keep funneling money to Boeing. In any reasonable world Boeing would be out of business and the executives would be on trial.
It was never as bad as it is now. It used to be past incidents would cause mass improvements over the whole industry. Similarly the long bow helicopter accidents caused a transformation in every areospace plant to measure and monitor electro static that might damage electronics. But it's just the last 10 years I noticed regs loosen for FOD, heat and humidity, and ESD causing defects.
In addition. In the past the US government would ground planes and issue immediate actions with these defects, and the manufacturer would face financial hardship and even bankruptsy. Now the lobbies bribe the government and the manufacturer continues to operate.
The airline industry is just as good at self-regulation as the financial industry is!
The Ducommon scandal where they mislocated flange holes then Boeing ovalized the holes was covered up with FAA complicity. They viscously attacked the buyer (yes buyer) that reported it. Of course things just got worse.

I worked on a Locheed Aircraft C-141 Starlifter jet plane. The doors opened up on a set of rollers, on both sides of the doors up vertically. Unlock by turning handle pull handle inward, the lift up until it locked. Other escape hatch on ceiling, opened inward. Cannot blow out accidentally by design. This design is from the 1960's. Zero incidents involving these door designs.
Thats what you get when you hire on skin color and gender rather than merit.

If workers and shareholders were raising concerns, several people had to have reported the plane was safe, when it wasn't. I pray God neither you or anyone you care about falls victim to something like this, because unethical people exist in every industry - food, drugs, vehicle manufacturers, etc. We all rely on quality control officers to do their jobs with the public in mind, above all, rather than profit.
There used to be inspectors. Now Boeing saves money by having the same worker self inspect their work. Couple this with an environment where workers are pressured to move things along and it’s a recipe for disaster but it saves money so HQ could care less

Spirit WAS “Boeing Wichita KS.” The factory was sold to a private investor group. All (or many) staff were fired or retired and then folks had to interview for new jobs if they wanted to work for Spirit. Then, investors took the company public a few years later to make a boat load of coin. This was in part motivated by Boeing’s desire to reduce the power of the unions, among other practicalities.

Could be. But the evidence seems to be that any assembly errors were done by Boeing unionized workers. It's not just "Union Busting" that has Boeing farm out this work. Many overseas sales are contingent on Boeing purchasing some major components by the country of the customer.
What a quandary? What is worse, management or unions. My experience in both private industry and in government, management was worse, but union didn’t help. Together it is toxic.

I ran the C-17 factory floor systems. There were 40,000 Work Orders per airplane. Each Work Order had 30 to 100 steps. The mechanic has to “stamp” (recorder in an electronic system) each step and a QA inspector has to “stamp” inspections for many (not all) steps. It’s very difficult to be 100%. I am guessing in this case that the mechanic left out the 4 bolts and mistakenly stamped them off as complete. I don’t know if there were inspection steps for this door. It should have.

My first thought was exactly that: a shoddy subcontractor. I've heard wild, first-hand accounts from people working within contracted companies who witnessed, or participated in, falsifying inspection reports in order to meet deadlines and this obtain bonuses for the companies. We're talking about military contractors.
It needs to be understood that Spirit AeroSystems of Wichita (maker of the 737 Fuselage) WAS Boeing until 2005. Up until that time, these fuselages were made by Boeing at their Wichita plant. When Boeing corporate decided that production costs in Wichita were too high, they "closed/sold" the plant, laid off all their workers and created a stand-alone entity known as Spirit AeroSystems.
I had many friends who worked at Boeing Wichita during that time. Everybody went home sweating bullets, looking in their mailboxes for that magic letter offering them "new" employment with the Spirit, doing the same job they did last week, but at significantly lower wages and with less benefits.
Spirit AeroSystems is a vendor/supplier for other manufacturers, but the overwhelming majority of work in Wichita remains the 737 fuselage production.
Boeing may claim this is not their problem, but the reality is that they created this spin-off company merely to decrease their manufacturing costs and increase their bottom line.
Thank you for this. In the video, Juan Gonzalez asked David Sirota why Boeing spun off the Wichita operations and Sirota never gave a direct answer.

Read how the FAA allowed McD to take its time carrying out vital airframe-safety mods to ALL DC-10 airliners after structural weaknesses were demonstrated in a 1972 depressurisation incident. Hundreds of people died in 1974 because feet were dragged to protect the DC-10's sales prospects.

This has been a known issue in the US airline industry for decades. So much so that NTSB managers tell people that whistleblow to be prepared to never work in the industry again.
I think Airbus should pick up these skilled persons. The whitleblowers care about safety.
German aircraft mechanic here. I‘m proud of our safety culture and workers rights. My company ordered MAXes recently and I hope I dont get to work on them.

There's suspicion that the door plug was not properly bolted. The plug is installed in the fuselage during manufacturing at Spirit. However, when Boeing receives the fuselage, they remove the plug to facilitate installation of the interior. The plug is then reinstalled after installation at Boeing. The aircraft had been delivered October, 2023, so the plug bolts were mostly likely last torqued at Boeing. The two hour voice recorder: The two hour limitation hass been the standard for US airliners for decades. Boeing airliners delivered to European airlines have 25 hour voice recorders. The FAA needs to upgrade the US standard to 25 hours.

Fact: 2 small bolts that don't have to be "tight" to function properly are retained by clips. These are installed after interior is loaded into plane at Boeing, not at Spirit during construction.
Why not have all ecordings uploaded to the Cloud, or something similar?
Tell me, why would the plugged door be removed to facilitate installation of furnishings? Once the plug is in place then the interior trimmings can be installed not before.
Lying isn't a fiscally costly act, so people who run businesses lie. Until lying costs the people with money more resources than lying generates, the behavior won't change.

7:16 I don’t think the cockpit voice recorder overwrite is as nefarious as it might seem.
Apparently the checklist in question went missing due to the pressure disruption and the pilots forgot to take out the circuit breakers on arrival
The ground engineering crew also omitted this step.
In my opinion, the real problem is the FAA has been diminished, and FAA leadership lacks authority & talent.
This is another consequence of prioritizing profit over safety.
It is nefarious on the part of the FAA to leave the tape length at only 2 hours, many years after the technology has advanced to make it easy to have 24 hours

I want to address the woman claiming that the plug door is designed to open under rapid decompression. That is only true IF it is an active emergency exit. The plug door on this flight was supposed to be decommissioned, and four safety bolts in place, preventing the door from opening under any circumstances.
this sub company is just a way to push off responsibility on purpose.
And a way to have non union workers. I would be willing to wager that the workers at Spirit do not have the same benefits as the Boeing workers that used to build these fuselages. I'm happy to be corrected by someone that might know better.

It's ownership of politicians by the corporations because we have a system of campaign finance regulation that allows legalized bribery. Corporate dollars fund the campaigns of politicians, who then do the corporations favors once in office. Our votes don't make much difference.

I would only add that once those politicians leave office, the corporations give them "gift" jobs like a seat on the board where they collect huge salaries and stock options if they have been "friendly" to said corporation. This is obviously another form of bribery as the politicians know this kind of payoff is in their future. The fact that the bribe is deferred makes it less obvious and easier to deny.

Spirit Aerosystems - a Boeing spin-off sub contractor - may well be guilty of not signing off correctly on quality control reports. The plug doors are manufactured and initially mounted in place by Spirit. The airplane then goes to Boeing for fitting inside the fuselage. Boeing removes the plug doors during this inside fitting procedure and then replaces the plug doors after the inside fitting has been completed.

I was working as a contractor at Boeing in El Segundo in March of 2020, right when the Covid-19 pandemic struck. There were morons coming into work SICK. I complained to Boeing for puting not only MY LIFE at risk, but my 95 year old Grandpa who I lived with at the time.
They told the "regular employees" that they should work remote if they can, but they also SAID NO to us contractors working remote. Doesn't MY LIFE count as much? So I complained to them about that too. Soon after they retaliated and fired me after only being there for 3 weeks. Boeing DOES retaliate, I'm here to tell you!

Well that's it for me. Airports have become a pain in the butt to deal with and to think one contends with that only to be placed in a machine that could fall out of the sky at any moment is just beyond the pale. Shareholder value be damned. The profit over everything culture that leads to this also leads to companies going under. We've all been subjected to the rip off from shoddy goods but in the case of Boeing, how do you ask for a refund after falling from 30,000 feet. If it's Boeing, I ain't going.

and its really frustrating that the US government and lobbyists have hobbled our ability to have high speed rail or ANY alternative to air travel for long distance, reliable transport in country.

I hate to say it but I recently did a lot of flying and was low key relieved every time I flew on an airbus. The 737 Max debacle lost the public trust.
The fact that the "full" plane miraculously didn't have anyone there means someone knew something was wrong with it.
No excuses, should never have had happened.
Profit ahead of safety and failing inspection are key contributors of this disaster. Luckily no one was seriously injured.

I got chewed out once for making a shield over a chain and sprocket that was tearing peoples clothes by a plant manager. Rich people don't care about safety, just money.
My dad used to be an Aero space parts inspector. Certified in everything.... seems like since he died nothings been safe. This will come back on the parts inspector , weather he or she works for the company that made them or not. The inspector put his or her stamp on it.. every stamp is a federal number that is monoroted. he or she will be the one held responsible.
This is extremely concerning to hear about. The involved companies need to be held accountable for risking our lives. They're fortunate that this latest incident wasn't fatal. I think more airlines might begin to consider buying Airbus planes.

Good Lord! This is horrifying! Just how pathtic IS Boeing at this point?! They only have ONE competitor & that's Airbus. Why does Boing feel the need to have a subcontractor with the unsettling name Spirit? I mean, Spirit Airlines is one of THE WORST carriers currently in operation. Moreover, when was the FAA targeted to be the Quality Control arm of a major aircraft producer? IF that really is their primary function, I cannot help but feel they're understaffed & underfunded. 💙

Boeing removes that plug door when the fuselage gets to their factory in order to install the interior,they then reinstall it. That it was not reinstalled properly is on Boeing,not the supplier.
And the FAA is supposed to be there inspecting along the way like building inspectors do at different phases of construction.

Space Shuttle Challenger explosion was also because of a culture of fear and deadlines at Nasa. Employees had tried to shake the tree and they were shut down, many of them retaliated against. Vertical and Horizontal communications is critical, as well as serious quality controls that should be verified by third parties…
And that's why they're always four times over budget and 4 years long on their project finishing.
I have not worked in the industry for 30 years. A lot has changed people. I am so disappointed to find this out. It’s not the Boeing that I know.
I worked for Airbus and many Boeing manufacturing procedures were derided as not being fit for purpose. The formation of Spirit was seen as a way of avoiding Boeing going bankrupt in the inevitable disasters to come.
That was definitely the culture when I worked on the 737 line, push push push, numbers numbers numbers is all that my manager cared about always showing us his stupid laptop and telling us what he looks at and cares about where just numbers on his screen, we hit 52 planes built that month with constant supply shortages, I didn’t want work for a company with that mentality so I quit shortly before the first plane went down Boeing sucks…….

I worked in quality control in the auto industry. One rule was you never record a bad result. You fix the problem, and record a good result. I've seen thousands of door lock components sent out that would shatter like glass because they cheated on heat treating.
it is scary to think companies like this would prefer profit over human lifes just to edge out over the competition..
I don't believe the shareholders care about safety, they ONLY care because of the potiential lawsuits.

Amy, how could you miss this : The NTSB official says that the cockpit door was « designed » to open in case of depressurization and Boeing « forgot » to mention this in the manuals. She then brushes this away by saying that Boeing will update the manuals. So basically, she is telling highjackers that if you blow out a window, you get access to the cockpit. When will be the next 911?
Spirit isn't building coffee tables for living rooms. They are building critical parts for an aircraft that could kill 200 people. Quality has to be of the utmost. It will definitely make me think twice about getting a window seat in the evacuation aisle.
Spirit - A company "headed now by a former Boeing executive" - That right there is your problem. The leadership at Boeing has been destroying this once noble engineer focused company from the inside for decades now.
That happened when McDonnell Douglas bought Boeing with Boeing's money. The Engineering/Quality culture was immediately changed to a Financial one.

@@ccanoles7717 it used to be that seeing a Boeing Jet on my itinerary instilled a sense of security - now it's just the opposite. Sadly I'd rather be flying on an Airbus, not they aren't without issues, at least it seems like they are trying to put safety first.
The airlines always cry they have no money. All those airlines can single handedly borrow from each other the $$ they need to operate.They are always given govt welfare along with the outrageous amounts they charge.

This kinda thing is super rare, no one was seriously injured. Yeah they should do an inspection and be a bit more on alert but who cares beyond that? This is like the first commercial accident in a few years. If you wanna go after air people go after the small and single user aircraft that go down ever couple months.
I'm surprised this is still going on. I worked for the company that replaced SPAR aerospace. SPAR had an issue with "Foreign Object Distress" which is things like metal shavings, lint, dirt, left in assemblies for aircraft and satellites. When a crashed satellite was found to be the fault of FOD from a SPAR factory, the whole aerospace industry in Canada was devastated and never recovered.😮

@@mpmpm aerospace is a cost plus industry. If aerospace companies what to increase profits that either ask the government to pay more for military hardware, or they ask the government of incentives to selling to airlines. It's not like your job flipping burgers.

Please look at the design structure of the plug door and the way the bolts retain the plug door in place. As long as those bolts are present the castle nut is on and the cotter pin is in it, they can be loose as hell that door will not leave that aircraft. It doesn't have to be tight it just has to be on there. My problem is that the only way that that door could have left the aircraft in question is that somebody either put the bolts in and never put the castle nuts and and cotter pins on. And all four of them fell out of their spot, or nobody ever put the bolts in in the first place. There are Springs underneath that door that are held in place by two bolts that run through the spring purchase and there's two bolts at the top that keep you from being able to push the door up to release it from its mounting points. The only way that that could have occurred is somebody stuck the bolts in the place and never put the nuts on them and the cotter pins. Unless there's something else to the design that I didn't see in the blueprints that I saw, that door isn't coming off of there without somebody intentionally walking away from it just after installing the door and throwing a couple of bolts in place and leaving it unfinished but they had to sign off on their actions. I've worked on aircraft, I know a little bit about what I'm talking about. Which is why I looked up the actual design features of that door plug.

Bolts break under stress if they are improperly handled. For example if they are over-tightened.

remember that wing job with no rivets, but since you couldn't see the job from the ground, someone took a break, another didn't sign off the incomplete job to the next shift, and the plane imploded. easy enough to happen. and a lot of liability for a nut jockey to have.
Pretty sure this was installed by a unskilled worker and "inspected" by an IA, who probably is overseeing 30 other worker's and never bothered to actually check the job, these corporations hate hiring skill and experience.

I've been a quality engineer since 1995 working in various industries, mostly automotive, and I can tell you that money drives how many pass/fail decisions are made.
And this is how you end up as a miles long debris field in a foreign country.
Profit is the GOD of America. We need to reassess our values and hold all BIG corporations accountable.
Those corporations are how they are, because of how conservative North-Americans are.
We need to hold the unity party accountable since we no longer have a 2 party system in America.

It's been like this for years as most companies are cutting on maintenance saying that ´´It´s an expense´´...!!! Those people were very, very lucky that day.
I am wondering what the purpose of designing a door to pop off during rapid decompression. As David Sirota pointed out, it would have been a disaster for those passengers sitting in those seats had they been not wearing their seat belts. That is, is this design worth the life of those and possibly other passengers and flight crew nearby the missing door? I am really interested in an explanation for that design?
Its laughable that the recording were not saved. The plane landed twenty minutes after the blow out and the recorders go for two hours. Nobody thought to pull the breaker on the recorders to save it. Wow
the new information is there may have been intermittent warning indicators (internal cabin pressure sensors) that have been alerting many crews to this related depressurization issue "leaks" over many months or years. That is recorded on the 24 hour recorders, and I have learned now those warnings have been erased as "false" warnings....but they were not !
The pilot unions do everything they can to minimize the length of recordings.
There was some minor note at the end of the appropriate checklist about flipping the breaker to the CVR after the incident but it's hard to fault the pilots for that minor oversight given the rapid depressurization reportedly blew the checklist out of the cockpit and they had a lot on their minds at the time. The real screwup there lays squarely on the ground crew securing the plane after the incident.

Airlines should be required to state what airplanes are used in every flight , at booking of flight . Before paying.

Actually that info is located on a passenger's ticket receipt and on the fuselage by the boarding door.
They already do this. But other models get subbed in on routes somewhat frequently. So it's not a guarantee and I'm pretty certain that the carriage contact states as much. I don't know for certain, but it's likely that you forfeit the value of your ticket if you refuse to board a flight due to another plane model being subbed in on the itinerary.

So all of the BOEING MAX airplanes 7,8,9 ALL have continuous issues unbeknownst to the public. Bowing internally removed many of their safety inspection procedures. And spirit employees were pressured to falsify safety inspection documents. Anyone who dares DEFEND Boeing or spirit in these comments is absolutely 100% an airline PR person. The MBA's of Aerospace companies probably couldn't tell the difference between a bolt and a nut, they think of airplanes are the same as washing machines.
100% agree! They can't. They have no fucken clue what any of the parts or aircraft types are because their head office is 3000 miles from the shop floor in Seattle! Management even the CEO need to be walking the shop floor daily asking questions and talking to the employees. Ask them what they need from you to improve quality and a better workplace. Put words in to action, "Walk the Talk". Companies like Boeing need to be run from the Bottom - up, not from the TOP - down. Take a page Arthur Blank and Bernie Marcus playbook and you'll be making the worlds best quality and safest planes!
There has to be mass resignations and criminal charges after this. This cannot continue.

This isn't the first time. The 737s (not MAX) used a subcontractor who was supposed to use CMC machine cutting tools on the fuselages in the '90s and the early '2000s. Instead, they were drawing the lines by hand with permanent markers and hand-cutting the fuselages. That led to bolt holes being misaligned and new extra holes drilled in the fuselages to work around the poorly cut main part of the fuselages. The whistle-blowers went to the FAA and the (FTC?) and their complaints were ignored and they were outed by the government to Boeing. They were subsequently fired by Boeing. The MAX door incident is a repeat performance by Boeing. My confidence in them has been eroded.
During my 20-years USN CAREER, after a 4-day turnover, I spent about 18-months in AIMD calibrating torque tools, pressure gages and aircraft cable tensiometers. I test the tools on devices that were certified by a lab. It was a simple job, I did need to reject a few items and educate some sailors on proper use and storage of Periodic Measuring Equipment. I left the lab due to promotion to AE1 a supervisor position.
Very odd that 26A and 26B were conveniently vacant. Thankfully no one was sitting there, but very very strange, almost like they were blocked off by the airline intentionally.
I think I have an idea why. It is not a coincidence or result of conspiracy. You know unless you pay for them, seats are assigned by an algorithm. Possibly these seats are defined as a kind of service access point for this aircraft variant and programmed to be assigned latest.
Another possibility : this plane had some presuurization alarms days before. Maybe they supposed that door plug was the problem and decide to seat nobody near it (the investigation will certainly reveal this if it's exact).

Sure, and considering 171 out of 178 seats were occupied, and likely the last row of 3 right at the bathroom wall was empty, means out of 4 empty seats available on the entire plane, an algorithm randomly left 26A and 26B empty? Riiiiiiiiiiiiight. Seems quite intentional those specific seats remained empty.

our government has a bad habit of having a ton of 6 figure salaries who are supposed to be watching out for this kind of stuff but allows these companies to self certify. If they did their jobs instead of stuff like taking Exxons word for having enough oil spill containment in the gulf we wouldnt have disasters like this
