To save his son, he had to do something that is equivalent to someone else. And Johnny chose himself as the target for the rock disease. He then fell off the horse and agitated a stand user which is a tree/bunch of leaves by falling on their leaves. The tree then teleported a huge boulder which killed Johnny.
