5:10 To me, this statement is more of an acknowledgement of a shift in her and Tomose's relationship/dynamic than a confirmation of her always knowing he was in love with her and ignoring them. Their relationship seems to rely on both of them avoiding talking about things that could cause conflict and brushing it under the rug without really resolving it and understanding the why. This can be seen when she goes to him about her voice at the park where he is dubbed Sugar Daddy Banjo and when he goes to her room. She even mentions this as a part of his personality. Due to being in the otherworld stream, he seems to be acting more vocal of his desires since he views this environment as a good thing for him, even if she disagrees with the notion. She seems dense of his feelings from the start, but she seems to finally be understanding that this new shift is occurring and may result in a rift between the two due to this new vocal behavior (it already seems to be causing them to drift apart). She is afraid of them no longer being as close, but doesn't seem to know how to understand his perspective since they're on totally different pages.
